Rapunzel syndrome in a pediatric patient: a case report.

This case report describes a 2.5-year-old girl who was hospitalized with complaints of abdominal pain and vomiting for 2 days. Abdominal ultrasound revealed small bowel-to-small bowel intussusception. Diagnostic laparoscopic-assisted exploration of the abdomen revealed 4 separate intestinal intussusceptions along with multiple dark intraluminal masses within the small intestine. Laparoscopic reduction of the intussusceptions was unsuccessful. Laparotomy allowed palpation of the entire small intestine with extraction of the masses, which were found to be human hair (trichobezoars). The intussus

Anesthetic considerations in hepatic porphyrias.

Porphyrias are inherited defects of heme synthesis with manifestations that can mimic surgical diseases; they can be provoked by administration of certain drugs. Manifestations such as abdominal pain, vomiting, tachycardia, hypertension, neuropathy, fever, confusion, and seizures have been described. Management of patients with porphyria is designed to avoid triggering drugs, such as barbiturates, and perhaps, benzodiazepines and ketamines. Nontriggering drugs should be used in the management of patients with porphyria. Because starvation can induce an attack, glucose infusions are important i

Vision loss associated with a stiff neck complicating strabismus surgery.

An exotropic 60 year old suffered a scleral-choroidal penetration and vitreous hemorrhage during and after strabismus surgery on a highly myopic eye when her anesthetic airway management was complicated by her previously asymptomatic and unrecognized rigid cervical spine which placed both the surgeon and the anesthetist in disadvantaged positions both physically and medically. Restoration of vision and binocularity ultimately required vitrectomy and intraocular lens implantation.

Optimizing Anesthetic Management for Laparoscopic Surgery: A Comprehensive Review.

Laparoscopy, as the most frequently chosen surgical technique in the world, carries specific complications and distinctions in anesthetic management. Complications of laparoscopy, particularly perceptible as disorders of the physiology of the cardiovascular, respiratory, and nervous systems, are caused by the specific technical conditions required for this type of operation. To facilitate surgical access and to clarify the surgical field, it is necessary to create a splenic emphysema, consisting of filling the peritoneal cavity with carbon dioxide (CO₂). This results in an increase in intra-ab

Minimum Atropine Dosing in Pediatric Patients: Does CRNA Practice Reflect Current Recommendations?

The purpose of this study was to evaluate certified registered nurse anesthetist (CRNA) current pediatric atropine dosing practices. Emphasis was placed on rationale for dosing and knowledge regarding current literature and guidelines. An electronic survey was deployed by the American Association of Nurse Anesthetists (AANA)'s survey department to a total of 2,905 CRNAs who are current AANA members. The survey was completed by 98 CRNAs, of which 67 selected that they do not administer anesthesia to pediatric patients weighing less than 5 kg and were excluded from further survey participation. …

Integrated pulmonary index in pediatric sedation for endoscopy: a prospective cohort study.

OBJECTIVE: Integrated pulmonary index (IPI™) is a device working with fuzzy logic principle that analyzes patient's end-tidal carbon dioxide (ETCO2), respiratory rate (RR), peripheral oxygen saturation (SpO2), and pulse rate and provides a number between 1 and 10. We aimed to investigate the usefulness of IPI monitor in pediatric patients. METHODS: After the Investigational Review Board approval pediatric patients undergoing gastrointestinal endoscopy under sedation were recruited. Propofol (Group P) and ketamine (Group K) were used for sedation. The primary outcome measure was average peripr

Video laryngoscope versus USB borescope-aided endotracheal intubation in adults with anticipated difficult airway: a prospective randomized controlled study.

BACKGROUND: Video laryngoscopes are approved equipment for difficult airway intubations. The borescope, which was introduced during the coronavirus disease 2019 (COVID-19) era, is placed over a direct laryngoscope blade to provide an economical video laryngoscope. In the current study, we investigated the use of an endotracheal tube mounted over a USB borescope versus a video laryngoscope in patients with suspected difficult airways. METHODS: After obtaining informed consent, 120 adult patients with suspected difficult airways undergoing elective surgery were included in this study. Patients

Anesthesia in Austere Environments: A Narrative Review.

Over 1 million preventable deaths occur each year because of insufficient access to emergency and essential surgical and anesthetic care. Austere environments, such as low-income and middle-income countries, isolated and confined environments, disaster situations, and combat zones, present specific challenges due to limitations of infrastructure, resources, and the environment. Strategies to improve safe anesthetic care in austere environments include mission trips and humanitarian aid, building local capacity of anesthetists and equipment, and improvisation. Telemedicine, mHealth applications…

Hyperbaric oxygen therapy in a case of cervical abscess extending to anterior mediastinum, with isolation of Prevotella corporis.

We present a case of an odontogenic abscess, first spreading at the lateral cervical level and then in mediastinum. We isolated an anaerobic bacterium, Prevotella corporis, rarely documented in literature. The mortality rates of cervical abscesses secondary to odontogenic infections and complicated by mediastinitis vary from 10% to 40%. Treatment of descending mediastinitis involves multidisciplinary teams such as otorhinolaryngology, thoracic surgeons, infectious disease physicians, anesthetists and intensivists. Due to the combined treatment with surgical drainage within 48 hours of hospital…

Two cases of isolated unilateral paralysis of hypoglossal nerve after uncomplicated orotracheal intubation.

We report two cases of postoperative unilateral hypoglossal nerve palsy following uncomplicated orotracheal intubation for plastic surgery. Both patients underwent a long procedure and were installed in a non physiological position. These two factors might have played an important role in the mechanism of nerve damage. Furthermore, other etiologies of neurological injury like a difficult airway or anatomical anomalies were not present. The aim of these two case reports is to sensitize the anesthetists to hypoglossal nerve palsy and to highlight a potential clinical problem of positioning durin…

Anesthesia and airway management in two cases of apert syndrome: case reports.

Apert syndrome is a type of acrocephalosyndactilia that consists of craniofacial synostosis, midface hypoplasia and syndactyly, with an autosomal dominant inheritance pattern. During anesthesia, difficult intubation and ventilation may be observed because of abnormal airways. In one of our patients, visceral anomalies, such as esophageal stricture and post-strictural dilatation, may cause respiratory problems because of aspiration. The second case was a Mallampati grade 2 with craniofacial synostoses, midface hypoplasia and syndactyly. In the case of apert syndrome, anesthetists must be prepar…

Isopropyl alcohol inhalation as treatment for nausea and vomiting.

In my practice as a recovery room nurse, I had observed anesthesiologists and nurse anesthetists wave an opened alcohol preparation pad under a patient's nose when he or she complained of nausea. When asked, "Why?'' the response often was, "Because it works.'' The following article describes the use of inhalation of isopropyl alcohol as a treatment for postoperative nausea and vomiting. Because alcohol swabs are so readily available, and certainly less expensive than some of the newer antiemetics on the market, this simple nursing intervention was worth investigating.

Wound healing and catheter thrombosis after implantable venous access device placement in 266 breast cancers treated with bevacizumab therapy.

The aim of this study was to determine, in a population with metastatic breast cancer treated with bevacizumab therapy, the incidence of wound dehiscence after placement of an implantable venous access device (VAD) and to study the risk of catheter thrombosis. This study enrolled all VADs placed by 14 anesthetists between 1 January 2007 and 31 December 2009: 273 VADs in patients treated with bevacizumab therapy and 4196 VADs in patients not treated with bevacizumab therapy. In the bevacizumab therapy group, 13 cases of wound dehiscence occurred in 12 patients requiring removal of the VAD (4.76…

Segmental thoracic spinal anesthesia in patient with Byssinosis undergoing nephrectomy.

Byssinosis is an occupational disease occurring commonly in cotton mill workers; it usually presents with features of chronic obstructive pulmonary disease (COPD). The management of patients with COPD presents a significant challenges to the anesthetist. Regional anesthesia is preferred in most of these patients to avoid perioperative and postoperative complications related to general anesthesia. We report a known case of Byssinosis who underwent nephrectomy under segmental spinal anesthesia at the low thoracic level.

Anesthetic management of a patient with sickle cell disease for common bile duct exploration.

Patients with sickle cell disease (SCD) may present to the anesthetist in different clinical settings like perioperative care, management of acute painful crisis and intensive therapy for acute respiratory failure. We describe the successful management of a 34-year-old female patient with SCD, posted for cholecystectomy with common bile duct exploration under general and epidural anesthesia. The importance of preoperative stabilization and careful anesthetic strategy is emphasized.
