Decreasing incidence of postcesarean endometritis with extended-spectrum antibiotic prophylaxis.

OBJECTIVE: To describe the effect of an extended-spectrum prophylactic antibiotic regimen on postcesarean endometritis. METHODS: This is a cohort study of trends in postcesarean endometritis using data both from prospective surveillance by the infection control unit and from query of our obstetric computerized database to compare three periods of antibiotic prophylaxis: standard narrow-spectrum with intravenous first- or second-generation cephalosporin (1992-1996), clinical trial of extended-spectrum with addition of intravenous doxycycline and oral azithromycin (1997-1999), and routine use o…

Antibiotic prophylaxis in patients with urinary retention undergoing transurethral prostatectomy.

BACKGROUND AND AIMS: The risk of severe infection and complications in connection with transurethral resection of the prostate (TURP) has clearly increased in patients with urinary retention and an indwelling catheter. The aim of this study was to determine whether the antibiotics ciprofloxacin and ceftazidime are equally efficient in preventing infective complications in these patients. MATERIAL AND METHODS: An open randomized prospective study was carried out on 72 patients with urinary retention undergoing TURP, to compare of 500 mg of ciprofloxacin (CF) orally twice daily and 1 g of cefta

Antibiotic prophylaxis for gastrointestinal endoscopy. American Society for Gastrointestinal Endoscopy.

This is one of a series of statements discussing the practice of gastrointestinal endoscopy in common clinical situations. It is intended to aid endoscopists in determining the appropriate use of antibiotic prophylaxis for patients undergoing gastrointestinal endoscopic procedures. Guidelines for the appropriate practice of endoscopy are based on a critical review of the available data and expert consensus. Controlled clinical studies are needed to clarify aspects of this statement, and revision may be necessary as new data appear. Clinical consideration may justify a course of action at varia…

[Antibiotic prophylaxis in cesarean section and voluntary termination in pregnancy].

Infection is the main complication after voluntary termination of pregnancy through vacuum aspiration, with an infection rate of 3.6% in the subsequent three weeks. This rate is comparable to that observed in the so-called clean surgical procedures. However the consequences on fertility remain unknown. Studies using systematic antibiotic administration in all vacuum aspirations (voluntary termination of pregnancy and spontaneous abortion) demonstrated the value of such a preventive measure. Factors of risk are difficult to identify and systematic bacteriological specimen collection in all pati

[Antibiotic prophylaxis in urology. Surgical and endoscopic surgery. Lithotripsy. Transplantation].

The opening of the urinary tract switches surgical and endoscopic urology to the clean contaminated category and therefore for each of them antibiotic prophylaxis has to be considered. Prophylactic antibiotics are only recommended before surgery in patients with sterile urine. Those with infected urine should have curative antibiotics. Prophylactic antibiotics are commonly recommended for transurethral resection of the prostate, transrectal biopsy of the prostate, renal transplant and radical cystectomy with ileal or colonic pouch for urinary diversion. It is worthless in diagnostic cystoscopy…

Antibiotic prophylaxis in trauma: penetrating abdominal injuries and open fractures.

Infection is an important cause of late morbidity and mortality following traumatic injury. As part of a coordinated treatment effort for the injured patient, preventive antibiotic use can reduce subsequent infectious complications. Available evidence supports the use of antibiotic(s) with activity against both aerobic and anaerobic enteric pathogens for patients with penetrating abdominal injuries and bowel penetration. Patients with open fractures benefit from the use of an antibiotic with activity against Staphylococcus aureus. Data on the ideal dose and duration of antibiotic administratio

[Antibiotic prophylaxis in orthopedics with special reference to spinal operations].

We report about our experience was the perioperative prophylaxis with antibiotics. We use a prophylaxis only in operations of the spinal column. In our opinion the prevention with antibiotics in other orthopaedic operations is not necessary. As an example we show our hip arthroplasties with a rate of infection under 0,5 percent.

Candida esophagitis following cardiac operation and short-term antibiotic prophylaxis.

Dysphagia and retrosternal pain are common complaints in patients after cardiac operations, and most often they result from the median sternotomy and/or endotracheal intubation. Although Candida esophagitis is a recognized cause of similar symptoms, it is usually not suspected except in immunologically compromised hosts. This report describes the case histories of five patients, not immunosuppressed or cachectic, who developed persistent dysphagia during recovery from cardiac operations; four patients received only 4 days of preoperative and postoperative prophylactic antibiotic treatment with

[Antibiotic prophylaxis of immediate and late complications of scarlet fever].

Patients with scarlet fever were followed by clinical and laboratory investigation (bacteriologic and immunologic reactions, etc.), in relation with the early application, and the duration of penicillin therapy. The patients were also followed for a period of 30 days after discharge from the hospital. The results show a decrease in the number of early and late complications, as well as a reduction in the number of carriers of beta-haemolytic streptococcus in those patients in whom penicillin treatment was applied early, and when it was prolonged in the recovery period. The clinical and statist

Postoperative Infection Risk in Sacrocolpopexy: Single vs Dual Antibiotic Prophylaxis.

INTRODUCTION AND HYPOTHESIS: The optimal antibiotic prophylaxis for mesh-based prolapse surgery remains uncertain. Although cefazolin is widely used, its limited anaerobic coverage may increase the risk of postoperative infections, particularly in women undergoing concomitant hysterectomy. Evidence from hysterectomy cohorts suggests that adding metronidazole reduces surgical-site infections, prompting evaluation of this approach in laparoscopic mesh-augmented apical prolapse repair. The objective was to compare postoperative infection rates in women receiving single-agent versus dual-agent pro…

Single- vs Multiple-Dose Antibiotic Prophylaxis for Total Hip Arthroplasty.

IMPORTANCE: In primary total hip arthroplasty (THA), studies suggest that a single preoperative antibiotic dose is noninferior to multiple doses for preventing periprosthetic joint infection (PJI). However, this evidence largely derives from older, heterogeneous trials. OBJECTIVE: To compare single- vs multiple-dose perioperative antibiotic prophylaxis for preventing PJI after primary THA for osteoarthritis. DESIGN, SETTING, AND PARTICIPANTS: This comparative effectiveness research study used a target trial emulation approach to analyze Danish registry and microbiology data on primary THA pr…

Antibiotic Prophylaxis Use and Outcomes in Children Undergoing Laparoscopic Orchiopexy.

PURPOSE: We compared clinically relevant outcomes in children undergoing laparoscopic orchiopexy who did and did not receive antimicrobial prophylaxis. MATERIALS AND METHODS: This was a multicenter cohort study using data from 144 National Surgical Quality Improvement Program-Pediatric hospitals. Patients younger than 18 years who underwent laparoscopic orchiopexy from January 1, 2021, to December 31, 2023, were included. Complementary hospital and patient-level analyses were performed to explore the relationship between prophylaxis and 30-day postoperative surgical site infection (SSI) rates

Rifaximin prophylaxis causes resistance to the last-resort antibiotic daptomycin.

Multidrug-resistant bacterial pathogens like vancomycin-resistant Enterococcus faecium (VREfm) are a critical threat to human health1. Daptomycin is a last-resort antibiotic for VREfm infections with a novel mode of action2, but for which resistance has been widely reported but is unexplained. Here we show that rifaximin, an unrelated antibiotic used prophylactically to prevent hepatic encephalopathy in patients with liver disease3, causes cross-resistance to daptomycin in VREfm. Amino acid changes arising within the bacterial RNA polymerase in response to rifaximin exposure cause upregulation

Antibiotic Prophylaxis in Breast Cancer Surgery: A Multicontinental Survey Study.

INTRODUCTION: Prophylactic antibiotic (PPA) usage is a common practice in breast cancer surgery. However, there is limited information on the global patterns of antibiotic usage in this setting. This study aimed to investigate the prevalence and preferences of PPA usage in breast cancer surgery among surgeons across different continents. METHODS: A multicontinental survey study was conducted among 295 surgeons who were actively involved in breast cancer surgery around the world. The survey collected information on PPA usage, preferred antibiotic choice, and factors influencing antibiotic pres

Prospective randomized trial of antibiotic prophylaxis for percutaneous renal biopsy.

BACKGROUND: Infection is a rare complication of percutaneous renal biopsy (RB). However, the questionnaire included in the Kidney Biopsy Guidebook 2020 in Japan revealed that antibiotic prophylaxis (AP) was administered at about 60% of hospitals. The objective of this study was to evaluate whether it is possible to omit AP for RB. METHODS: Patients aged ≥ 15 years were eligible. Three hundred and sixty-four patients were recruited at 6 hospitals. The patients were randomly assigned to receive either a single dose of intravenous cefazolin or no antibiotic prophylaxis. The primary outcome was t…

Enhanced antibiotic prophylaxis and infection-related complications following prostate biopsy.

PURPOSE: Sepsis after prostate biopsy is a costly and potentially lethal complication. We sought to assess whether enhanced antibiotic prophylaxis regimens combining oral and parenteral antibiotics may decrease the risk of post-biopsy urinary tract infection and sepsis compared to regimens with only oral antibiotics. METHODS: We identified men with commercial insurance who underwent prostate biopsy (2009-2015) with prophylactic antibiotic coverage. Our primary exposure of interest was antibiotic regimen: enhanced, oral-only, and parenteral-only. Post-biopsy outcomes of interest included urina…

Low Adherence to Recommended Guidelines for Open Fracture Antibiotic Prophylaxis.

BACKGROUND: Prompt administration of antibiotics is a critical component of open fracture treatment. Traditional antibiotic recommendations have been a first-generation cephalosporin for Gustilo Type-I and Type-II open fractures, with the addition of an aminoglycoside for Type-III fractures and penicillin for soil contamination. However, concerns over changing bacterial patterns and the side effects of aminoglycosides have led to interest in other regimens. The purpose of the present study was to describe the adherence to current prophylactic antibiotic guidelines. METHODS: We evaluated the a

Antibiotic prophylaxis in orthognathic surgery: an overview of systematic reviews.

The purpose of this overview was to assess different antibiotic regimens used in orthognathic surgery and to establish an evidence-based protocol so that beneficial and adverse effects can be determined. A comprehensive literature search for systematic reviews and/or meta-analyses was conducted in MEDLINE (PubMed), EMBASE, and the Cochrane Library until March 2020. Grey literature was investigated in Google Scholar, and a manual search was done of references lists. Two meta-analyses and four systematic reviews met the inclusion criteria. The AMSTAR-2-tool was used to ascertain the potential ri
