The paper presents our experience in correcting with contact lens the children's aphakia. Since February 1990, soft contact lenses daily ported have been used in children's aphakia. The study included 17 children with ages, between 2 and 9 year old, from which 9 cases were of congenital cataract, 6 of traumatic cataract and 2 of Marfan syndrome. We analyzed retrospectively the complications and visual results of 154 cases of secondary intraocular lens implantation. The follow-up ranged from 1 to 4 years (mean 2.8 years). Best corrected final post-operative visual acuity was better or the same as the best corrected preoperative vision in 95.5% of the eyes. Vision deteriorated in 7 eyes (4.5%): in 3 eyes because of cystoid macular edema, in 2 eyes because of retinal detachment, in 1 eye because of corneal decompensation and in 1 eye because of macular hole. We studied 75 epikeratoplasty procedures using nonlyophilized tissue performed by eight ophthalmic surgeons in 70 eyes (47 patients) to correct for aphakia in children less than 8 years of age (mean age, 3.4 +/- 2.1 years). The success rate and complications for contact lens wear in 366 aphakic patients were evaluated retrospectively over a mean follow-up period of 36 months. Successful use of a daily wear contact lens (DWCL) was related to lens handling ability. In the nationwide study of epikeratophakia, 154 ophthalmic surgeons who had attended a training course performed 519 procedures for the correction of aphakia in adults: 310 of the eyes had 30 or more days of follow-up after suture removal. 72 unselected patients wearing aphakic spectacle correction were examined more than 6 months postoperatively. Approximately one third had poor adaptation in spite of satisfactory acuity. No correlation with age was found in the 60-85 year range. Binocularity appeared to play no part in adaptation but a significant sex difference was found, females predominating in the non-adapted group. Four embryos from women infected by rubella virus early in pregnancy were investigated histologically.
