A patient with mental retardation and mild facial dysmorphism had a karyotype which was considered to be normal before the availability of chromosomal banding techniques. She had a history of a cat-like cry and severe feeding problems during infancy. At the age of 9, she was still found to have initial aphonia on trying to initiate sounds. Repeat chromosome analysis with G banding showed an interstitial deletion of the long arm of chromosome 7.

The formation of tracheal 'granuloma' in the tracheostomized infant, as a cause of aphonia and an obstacle to decannulation has long been recognized. Endobronchial or transtomal removal has been recommended; however, a number of these lesions were resected in conjunction with complete tracheostomy revision and found to be very much larger than suspected endoscopically and consisting of dense mature scar tissue. It is, therefore, suggested that resection of these lesions endoscopically or transtomally may be quite inadequate and an open procedure preferred.

Kaposi's sarcoma of the larynx is a rarely observed manifestation of AIDS. Possible concomitant symptoms are hoarseness up to aphonia, urge to cough or stridor. Expectoration of small parts of tissue of Kaposi's sarcoma may be an additional sign. Similar to Kaposi's sarcoma in the oral cavity Kaposi's sarcoma of the larynx is characterised by a purplish, spongy nodule. Its surface can be verrucous due to deposits of dry secretion. Local extirpation of Kaposi's sarcoma removes the laryngeal signs and symptoms.

Idiopathic fibrosis of different areas of the body such as retroperitoneal fibrosis, mediastinal fibrosis, sclerosing cholangitis, Reidel's thyroiditis and pseudo-tumour of the orbit are well recognised entities that manifest individually or in combination. A case of idiopathic fibrosis of the upper aerodigestive tract is presented, which caused dense fibrosis of the floor of mouth, tongue, pharynx and larynx leading to aphonia and severe dysphagia. A permanent tracheostomy was performed and regular bouginage is necessary. This is the first such case reported in the literature.

From birth, an infant girl had repeated episodes of loss of consciousness, apnea and bradycardia, aphonia, and severe pharyngeal-esophageal discoordination. These functional deficits correlated with a gross and microscopic malformation of the medulla oblongata, which is characterized by profound deficits of motor and sensory nuclei and of the reticular formation. We have classified this as an example of Möbius syndrome. No etiology was suggested from the history. Histopathologic findings are compatible with an early massive insult to developing structures in the tegmentum of the medulla oblong…

Ectopic thyroid tissue is an uncommon anomaly, and the presence of dual ectopic thyroid tissues in a single patient is exceedingly rare. We describe a 69-year-old woman who presented with dyspeptic symptoms, asthenia, aphonia, and chronic fatigue. The patient was eventually found to have a multinodular goiter, cervical goiter, and mediastinal goiter. The patient underwent radical thyroidectomy and dissection, followed by treatment with levothyroxine. This report underscores the diagnostic challenges in complex presentations of ectopic thyroid tissue.

Effective treatment of adductor type spasmodic dysphonia with botulinum toxin injection is presented. Patients showed objective and/or subjective improvement in phonation. The beneficial effect lasted for approximately 3 months. An immediate complication is temporary hoarseness or aphonia, mainly due to diffusion of BT into the adjacent muscles. This is avoided by limiting the injection to one vocal fold only and by keeping the dose at less than 5 units. Insertion technique of the needles, such as percutaneous and laryngoscopically controlled techniques, are discussed.

We present a case of a 73-year-old man who developed sudden onset dysarthria, dysphagia and bilateral facial weakness with automato-voluntary dissociation, which deteriorated rapidly to anarthria and aphonia within a few days. MRI scan of the head showed acute infarct in right internal capsule and an old infarct in the left corona radiata while the rest of the investigations were normal. Based on these findings, diagnosis was thought to be subopercular syndrome. He recovered significantly in a few weeks' time.

Clinical variants of laryngeal lesions in patients with early syphilis are reviewed. A case of secondary syphilis with a laryngeal lesion is reported in a female who developed aphonia. Basing on laryngoscopic findings, an otorhinolaryngologist made the diagnosis "chronic laryngitis, singer's nodules". The authors infer that painless laryngitis of long duration without signs of inflammation and resistant to standard treatment should be tested for syphilitic etiology. Therefore, the patient's blood should be taken for Wasserman reaction while the patient should be consulted by venereologist.

BACKGROUND: Trachea esophageal puncture (TEP) is performed following total laryngectomy to allow speech and communication. The most common reason for long-term speech failure in this population is hypertonicity of the constrictor muscle.
OBJECTIVES: To present our experience with the treatment of aphonic patients after total laryngectomy and TEP and suggest a protocol for treatment.
METHODS: Of 50 patients who underwent total laryngectomy and TEP, 6 suffered from aphonia after surgery. All patients underwent radiotherapy with or without chemotherapy. Delay in speech continued for more than 6…

Recurrent laryngeal nerve (RLN) injury is one of the main complications of total thyroidectomy. If the injury is bilateral, total airway obstruction, aphonia and hoarseness of voice could be precipitated. Hence, it is wise for the operating surgeon to be guided by neural monitoring during thyroidectomy. We present a valuable experience handling a middle-aged man with a huge papillary thyroid carcinoma . He needed an urgent thyroidectomy due to obstructive symptoms. We highlight our intraoperative dexterity in handling his surgery in the context of continuous monitoring of RLN using electromyog…

A retrospective study was conducted in a psychiatric setup of S.P. Medical College, Bikaner (Raj.) to assess the social demographic and clinical characteristics of hysterical patients. The illness was more common in female patients. Most of the patients were young, married and illiterate. Nearly half of them had faced some stress prior to onset of their illness. Fits of unconsciousness and aphonia were the commonest presentation in female and male patients respectively. Duration of stay was 2-3 days and most of the patient responded well to different treatment modalities.

Injury to the thoracic trachea is a potentially lethal condition in a patient with multiple injuries. Several clinical signs are commonly associated with this process: subcutaneous emphysema, aphonia, stridor, pneumothorax refractory to thoracostomy tube drainage, pneumomediastinum, and hemoptysis. The clinical appearance of tracheobronchial rupture may be delayed for hours or even weeks following injury. Standard treatment for disruption of the thoracic trachea is primary repair via a right thoracotomy. We describe a patient with a complex carinal injury following blunt thoracoabdominal traum…

In the effort to develop a non-invasive method for the evaluation of the vibratory characteristics of the vocal folds in patients with vocal pathology, some of the newer methods for evaluating vocal function in normal speech are being considered. Inverse filtering of the glottal airflow waveform and ways of measuring vocal fold contact area are described and evaluated for their potential use in the clinic. Knowledge of the vibratory behavior of a patient's vocal folds is important to our understanding of problems such as spastic dysphonia, dysphonia plica and functional aphonia as well as the …
