Atypical cause of radiculopathy - Intradural spinal arachnoid cyst.

Intradural spinal arachnoid cysts are a relatively uncommon lesion that may be either intra, or extradural, and intradural spinal arachnoid cysts are even less common. Arachnoid cysts are cerebrospinal fluid collections in the spine that can present with neurological symptoms. The objective of this paper is to describe a rare case of radicular pain due to a spinal arachnoid cyst.

[«De novo» development of a suprasellar arachnoid cyst].

Intracranial arachnoid cysts are fluid-filled cavities that arise within the cranial arachnoid, representing approximately 1%-2% of all intracranial lesions among the paediatric population. We present the case of a 2-year-old boy who presented with instability and episodes of ocular deviation. A computed tomography scan (CT scan) and magnetic resonance imaging (MRI) of the brain revealed a suprasellar cyst and obstructive hydrocephalus. At birth a transfontanellar ultrasound was normal. The cyst underwent endoscopic fenestration with complete remission of symptoms. In the review of the literat…

Cranial nerve VI palsy after dural-arachnoid puncture.

In this article, we provide a literature review of cranial nerve (CN) VI injury after dural-arachnoid puncture. CN VI injury is rare and ranges in severity from diplopia to complete lateral rectus palsy with deviated gaze. The proposed mechanism of injury is cerebrospinal fluid leakage causing intracranial hypotension and downward displacement of the brainstem. This results in traction on CN VI leading to stretch and neural demyelination. Symptoms may present 1 day to 3 weeks after dural-arachnoid puncture and typically are associated with a postdural puncture (spinal) headache. Resolution of …

Benign enlargement of sub-arachnoid spaces in infancy.

Benign enlargement of sub-arachnoid spaces (BESS) is one of the causes of macrocephaly in infants. It is a self-limiting condition and does not require any active medical or surgical treatment. We report a case of an infant aged 4 months who was referred for magnetic resonance imaging (MRI) of the brain as the head circumference of the infant had increased rapidly from the 50(th) percentile in the 3(rd) month to more than the 95(th) percentile in the 4(th) month of age. MRI revealed enlarged anterior sub-arachnoid spaces and mild prominence of all the ventricles. A possibility of BESS was susp…

Endoscopic treatment of quadrigeminal arachnoid cysts in children.

OBJECT: Quadrigeminal arachnoid cysts (QACs) are rare, comprising approximately 5%-10% of all intracranial arachnoid cysts. The management of these cysts is challenging, and their optimal surgical treatment is controversial. This study evaluates the role of endoscopy in the treatment of QACs in children, focusing on some factors or technical aspects that might influence the outcome. METHODS: Eighteen children with symptomatic QACs were the subject of this study. The group included 10 boys and 8 girls, with a mean age of 2.5 years. All patients had hydrocephalus. Surgical treatment included ve…

Intracranial arachnoid cysts in a chimpanzee (Pan troglodytes).

An intracranial arachnoid cyst was detected in a 32-year-old, 44.6-kg, female chimpanzee at the Primate Research Institute, Kyoto University. Magnetic resonance imaging (MRI) and computed tomography (CT) were performed and the cognitive studies in which she participated were reviewed. MRI revealed that the cyst was present in the chimpanzee's right occipital convexity, and was located in close proximity to the posterior horn of the right lateral ventricle without ventriculomegaly. CT confirmed the presence of the cyst and no apparent signs indicating previous skull fractures were found. The th…

Institutional experience of endoscopic suprasellar arachnoid cyst fenestration.

INTRODUCTION: Suprasellar arachnoid cysts can differ from other arachnoid cysts in several ways, making a separate analysis of these cysts worthwhile. Herein, we present the outcome and perform volumetric analysis of six children with suprasellar arachnoid cysts treated with endoscopic ventriculocystocisternostomy in order to evaluate the long-term outcomes. PATIENTS AND METHODS: Operative and postoperative data were retrospectively reviewed for six patients harboring suprasellar arachnoid cysts. Imaging was then used to follow success of surgical intervention. RESULTS: Six patients with sup…

Intracranial arachnoid cyst; an unusual cause of epiphora.

PURPOSE: To describe a case of unilateral intracranial arachnoid cyst in association with enophthalmos and epiphora on the same side. METHODS: Case report. RESULTS: A young man with symptoms of unilateral epiphora is described. He had a large intracranial arachnoid cyst with an unusually large orbit leading to enophthalmos and symptomatic epiphora on that side. Radiological features of the orbit and associated pneumosinus dilatants are described. We also offer hypotheses to describe the paradoxical occurrence of an expanding intracranial mass and inward growth of the orbit and paranasal sinu…

Symptomatic increased intracranial pressure due to arachnoid cysts.

Intracranial arachnoid cysts are typically benign lesions, but rarely, they may cause signs and symptoms relating to increased intracranial pressure. We report 4 pediatric patients with arachnoid cysts who received successful surgical treatment after failing to respond to conservative medical therapies. After undergoing a shunting procedure, each patient experienced improvement in symptoms, resolution of optic disc swelling, and reduction or elimination of medications necessary to manage the condition. These cases illustrate the potential for arachnoid cysts to outstrip compensatory mechanisms…

[Effect comparison of arachnoid cysts in sacral canal].

OBJECTIVE: To evaluate the clinical outcomes of two different surgical treatments for arachnoid cysts in sacral canal. METHODS: From January 2004 to March 2009, 55 cases of arachnoid cysts in the sacral canal were treated by traditional simple sacral laminectomies with resection of the cysts (group A, 25 cases) and novel CT-guided percutaneous fibrin glue therapy of arachnoid cysts (group B, 30 cases). Of them, there were 23 males and 32 females, aging 15-66 years with an average of 42.6 years; the duration of symptoms was 6 months to 15 years with an average of 3.5 years. L5-S1 was involved …

Coexistent intraventricular abnormalities in periventricular giant arachnoid cysts.

OBJECT: Arachnoid cysts are congenital lesions that arise during development by splitting of the arachnoid membrane. Large cysts can be adjacent to CSF pathways causing a marked midline shift and hydrocephalus. The association between a large arachnoid cyst and hydrocephalus has been commonly described as being due to a mass effect, but these previous reports have not focused closely on any associated intraventricular abnormalities. METHODS: Seven patients who were previously treated with a cystoperitoneal shunt presented with shunt failure, hydrocephalus, and/or cyst expansion. All of these …

Supratentorial arachnoid cyst mimicking a Ménière's disease attack.

Arachnoid cysts (AC) often present with symptoms depending on their anatomical location within the skull; while supratentorial cysts grow causing relatively few symptoms, infratentorial ones may impair liquor circulation at the level of the fourth ventricle, giving rise to intracranial hypertension, or may stretch the complex nerve network in the cerebello-pontine angle. We report the singular clinical history of a 54-year-old male, who suddenly began to feel dizzy while sitting at his workplace, complaining of left tinnitus and aural fullness, in a classic clinical picture of Ménière's diseas…

Radiologic differentiation of intracranial epidermoids from arachnoid cysts.

OBJECTIVE: Intracranial epidermoids (cholesteatomas) mimic arachnoid cysts in their radiologic characteristics, especially in the cerebellopontine angle. It is essential to differentiate the two conditions because they warrant different therapeutic interventions. The objective of this study is to elucidate the different radiologic characteristics of the conditions. STUDY DESIGN AND SETTING: This was a retrospective study of 4 patients referred for a differential diagnosis and management of intracranial cystic lesions to the Departments of Neurotology/Neurosurgery and Neuroradiology in a terti…

[Arachnoid cyst with orbital meningocele: a case report].

Arachnoïd cysts are rare. 1% of cranial neoformation. We report the case of a six years old boy who presented an arachnoid cyst impressive by its compressive phenomena. Present complaint are macrocrania and right orbitary tumor. Tomodensitometry reveal a huge liquid collection which starts from the right cerebellopontine angle to the frontal region and invading the orbital cavity. In addition we have a thalamic fusion Its congenital origin is obvious. The associated malformations seem to be related to an anomaly of the centre line. This case point out the primum movens of malformations which a…

[Suprasellar arachnoid cyst associated with syringomyelia. Case report].

We present a case of suprasellar arachnoid cyst which was revealed by visual impairement and hypopituitarism. Neuroradiological imaging showed the peculiar association of the suprasellar cyst with cerebellar tonsillar herniation and a large asymptomatic cervical syringomyelic cavity. Surgical treatment of the suprasellar cyst allowed the reduction of both the cyst and the syrinx. A common pathophysiological mechanism of these lesions is discussed. We suggest the possibility that an initial obstruction of the basal cisterns caused the suprasellar cyst formation which led to medullar cavity form…
