INTRODUCTION: The hand-arm vibration syndrome of the foundry workers using chipping hammers and grinders were analysed in Hungary last in 1950th-s years. Therefore it seemed necessary to consider the present situation of the disease.
AIM: To study of the signs of the hand-arm vibration syndrome of the foundry workers using chipping hammers and grinders.
METHOD: Retrospective analysis of the clinical findings of 154 foundry workers using chipping hammers and grinders. The examination included the vascular, neurological and musculoskeletal systems of the upper extremities in 95 patients.

BACKGROUND: When the surgeon's approach is from behind the patient's shoulder via the infratentorial supracerebellar approach in the prone position (Concorde position), the patient's shoulder nearest the surgeon occasionally interferes with the visual route and surgical manipulation. To avoid this difficulty the author developed a modified Concorde position.

The analysis of 35 cases of delayed healing of fractures of the upper arm treated from 1981 to 1990 shows that fractures resulting from road accidents (16/35) and comminuted fractures (16/35) are particularly frequent in this group. Delayed fracture healing after conservative treatment (10 patients) was caused mainly by inadequate reduction (4/10) or by the fact that the conservative treatment performed was in fact inappropriate (4/10). A retrospective study was carried out of the morbidity in the donor limb following use of the radial forearm flap in 27 patients. Of the 13 patients whose flaps contained bone, 4 sustained fractures of the radius. This was associated with a 50% loss of power of grip and pinch, and limitation of other movements, particularly pronation and supination. The anatomy of the lateral arm flap is further elucidated, and the surgical techniques involved in raising the flap are described in detail. The data are derived from studies in 25 cadavers and five clinical cases. The relative merits of this versatile free flap are discussed. Three clinical cases are cited for illustration.

A patient is described whose initial symptoms, though suggestive of polymyalgia rheumatica, were attributed to psoriatic arthritis, and whose subsequent development of claudication of the left arm led to the biopsy proven diagnosis of giant cell arteritis. Attention is drawn to widespread arterial involvement by giant cell arteritis which, though uncommon, must be considered in all patients with ischemic limb pain. In addition, the consequences of overlooking polymyalgia rheumatica as an early manifestation of giant cell arteritis are outlined.

A 13-year-old girl presented with microcephaly, short and broad neck, low posterior hairline, congenital heart disease, limitation of joint movement, and mild mental retardation. Chromosomal analysis showed interstitial deletion of band p14 of the short arm of chromosome 7.
