Recently, two new rationale for surgical treatment of atrial fibrillation have been developed which have been used, initially, primarily in patients with lone atrial fibrillation. We performed the "corridor" operation in eleven patients with paroxysmal or chronic atrial fibrillation. Management of atrial fibrillation includes assessing the need for rate control, identifying underlying conditions, and performing cardioversion or instituting long-term medical therapy. Elective cardioversion should be strongly considered for every patient, chiefly to decrease the incidence of embolic stroke. Patients who remain in chronic atrial fibrillation require attention to rate control; digoxin (Lanoxin) alone may be a poor choice if they are vigorous and active. Many subgroups of patients benefit from long-term anticoagulation.

I review the present understanding of thromboembolic complications and their prevention in patients with nonrheumatic atrial fibrillation. The paper describes five cases of atrial fibrillation detected after racing. In four of them, the arrhythmia disappeared spontaneously within 24 h and they were regarded as paroxysmal in type. In the fifth case, which won its race, the arrhythmia persisted for at least 45 h after racing. It was therefore regarded as an example of persistent atrial fibrillation. It was then treated with quinidine sulphate which restored sinus rhythm. It would seem that paroxysmal atrial fibrillation may be a cause of sudden decrease in racing performance.

Of 210 patients with thyrotoxicosis, 21 (10%) had atrial fibrillation. Five of these patients (24%) had systemic emboli (which were multiple in three patients, one of whom died) and two other patients had pulmonary embolism. Embolic disease is common in thyrotoxic atrial fibrillation, and anticoagulant therapy should be considered for all patients.
