Spinal Epidural Hematoma Due To Tyre-Blast Injury: A Case Report.

STUDY DESIGN: A retrospective case report. OBJECTIVE: The objective of this article is to report a spinal epidural hematoma (SEH) due to shock wave. SUMMARY OF BACKGROUND DATA: SEH is an infrequent condition. Most of SEH's are spontaneous. We have reported an SEH traumatic case without bone lesions due to exploding truck tire. A different category of blast injuries is the one related with exploding tyres. Shock waves are the main mechanism that is responsible for blast injuries. We are presenting the first report of acute SEH due to shock wave. METHODS: A 33-year-old man was brought to the …

A case of Tannerite(®) target mixture causing severe blast injury.

Tannerite(®) is a proprietary blend of an oxidizer, ammonium nitrate, and aluminum powder catalyst used to make homemade exploding targets. While it is currently approved for unrestricted sale in the United States, it can be used to form devices capable of inflicting major blast injury. We present here a case of close proximity exposure to detonation of the mixed Tannerite(®) blend. In our patient, the exposure lead to injuries typical of blast injury, such as tympanic membrane rupture, globe injury, and severe burns. We review here the sequelae of blast injuries that one must consider when tr…

Respiratory responses following blast-induced traumatic brain injury in rats.

Blast overpressure (OB) injury in rodents has been employed for modeling the traumatic brain injury (TBI) induced by an improvised explosive device (IED) in military service personnel. IED's can cause respiratory arrest if directed at the thorax due to the fluid-tissue interface of the lungs but it is unclear what respiratory changes occur in a head-directed OB injury. The diaphragm is the primary muscle of inspiration and electromyographic (EMG) recordings from this muscle are used for recording breathing in anesthetized and conscious rats. The breathing pattern of the rodents will be recorde

Mechanisms and pathophysiology of the low-level blast brain injury in animal models.

The symptoms of primary blast-induced mTBI, posttraumatic stress disorder and depression overlap. Evidence of an organic basis for these entities has been scarce and controversial. We present a review of animal studies demonstrating that low-level blast causes pathophysiological and functional changes in the brain. We monitor a time period from minutes to approximately 1 week after blast exposure from multiple modes (air, underwater, localized and whole body). The most salient findings observed were (1) the peak pressures (P(max)) in the brain, elicited from the blast from the firing of milita

Bomb blast, mild traumatic brain injury and psychiatric morbidity: a review.

Traumatic brain injury (TBI) arising from blast exposure during war is common, and frequently complicated by psychiatric morbidity. There is controversy as to whether mild TBI from blast is different from other causes of mild TBI. Anxiety and affective disorders such as Post-traumatic Stress Disorder (PTSD) and depression are common accompaniments of blast injury with a significant overlap in the diagnostic features of PTSD with post-concussive syndrome (PCS). This review focuses on this overlap and the effects of mild TBI due to bomb blast. Mild TBI may have been over diagnosed by late retros

Auditory and vestibular dysfunction associated with blast-related traumatic brain injury.

The dramatic escalation of blast exposure in military deployments has created an unprecedented amount of traumatic brain injury (TBI) and associated auditory impairment. Auditory dysfunction has become the most prevalent individual service-connected disability, with compensation totaling more than 1 billion dollars annually. Impairment due to blast can include peripheral hearing loss, central auditory processing deficits, vestibular impairment, and tinnitus. These deficits are particularly challenging in the TBI population, as symptoms can be mistaken for posttraumatic stress disorder, mental-

Blast injury of the ear: clinical update from the global war on terror.

The purpose of this study was to describe the effects of blast exposure on hearing status. This study retrospectively analyzed hearing thresholds and otologic complaints for >250 patients with blast-related injuries from the global war on terror. Of patients who received full diagnostic evaluations, 32% reported a history of tympanic membrane perforation, 49% experienced tinnitus, 26% reported otalgia (ear pain), and 15% reported dizziness. Expected hearing thresholds were computed by applying age-correction factors to hearing tests performed earlier in the service members' careers and before

Systolic pressure variation in hemodynamic monitoring after severe blast injury.

Fluid management in patients following blast injury is a major challenge. Fluid overload can exacerbate pulmonary dysfunction, whereas suboptimal resuscitation may exacerbate tissue damage. In three patients, we compared three methods of assessing volume status: central venous (CVP) and pulmonary artery occlusion (PAOP) pressures, left ventricular end-diastolic area (LVEDA) as measured by transesophageal echocardiography, and systolic pressure variation (SPV) of arterial blood pressure. All three patients were mechanically ventilated with high airway pressures (positive end-expiratory pressure

Traumatic air blast injury with intracranial, bilateral orbital, and mediastinal air.

Air at 75 lbs/sq in from an air compressor hose struck a 19-year-old man in his right eye. Injected through a superior conjunctival laceration, the air entered subcutaneous, mediastinal, and bilateral orbital and intracranial spaces. High-resolution computed tomography scanning of the head and orbits revealed air adjacent to the right optic nerve throughout the orbit. We hypothesize that air was injected intracranially, but extradurally, through the superior orbital fissure. The patient suffered no visual loss.

Development of a Severe Blast Lung Injury Model in a Goat for Ultrasonographic Evaluation.

Blast lung injury (BLI) causes severe lung damage and high morbidity, especially in military and industrial settings, leading to hemorrhage and potential respiratory failure, highlighting the need for improved diagnostics and treatments. This study aimed to develop a standardized severe BLI model in goats to evaluate the accuracy and feasibility of point-of-care ultrasound for dynamic assessment. Goats were subjected to controlled blast overpressure using the biologic shock tube (BST-Ⅰ) at driving pressures of 4.0 MPa (n = 4), 4.5 MPa (n = 12), and 5.0 MPa (n = 4), respectively. Key parameters

Blast Overpressure-Induced Neuroinflammation and Axonal Injury in the Spinal Cord of Ferrets.

BACKGROUND: Blast-induced spinal cord injuries (bSCI) account for 75% of all combat-related spinal trauma and are associated with long-term functional impairments. However, limited studies have evaluated the neuropathological outcomes in the spinal cord following blast exposure. Objectives In this study, we aimed to determine the acute and sub-acute neuropathological changes in the spinal cord of ferrets after blast exposure. METHODS: An advanced blast simulator was used to expose ferrets to tightly coupled repeated blasts. The Catwalk XT system was used to detect gait performances in ferrets

The Neurovascular Unit as a Locus of Injury in Low-Level Blast-Induced Neurotrauma.

Blast-induced neurotrauma has received much attention over the past decade. Vascular injury occurs early following blast exposure. Indeed, in animal models that approximate human mild traumatic brain injury or subclinical blast exposure, vascular pathology can occur in the presence of a normal neuropil, suggesting that the vasculature is particularly vulnerable. Brain endothelial cells and their supporting glial and neuronal elements constitute a neurovascular unit (NVU). Blast injury disrupts gliovascular and neurovascular connections in addition to damaging endothelial cells, basal laminae,
