Calcium at the Helm: Mechanisms and Therapeutic Targets in the Retinal Neurovascular Unit.

Retinal neurovascular unit (RNVU) dysfunction underlies major blinding and neurodegenerative conditions including glaucoma, diabetic retinopathy (DR), age-related macular degeneration (AMD), retinal ischemia-reperfusion (RIR) injury, and Alzheimer's disease (AD)-associated retinopathy. Within the RNVU, calcium ions coordinate neurotransmission, glial activation, vascular tone, and blood-retinal barrier maintenance, and calcium dysregulation is emerging as a unifying pathogenic hub across these conditions. Although upstream triggers differ, including mechanical stress in glaucoma, hyperglycemia…

Reversibility of retinal microvascular changes in severe falciparum malaria.

Malarial retinopathy allows detailed study of central nervous system vascular pathology in living patients with severe malaria. An adult with cerebral malaria is described who had prominent retinal whitening with corresponding retinal microvascular obstruction, vessel dilatation, increased vascular tortuosity, and blood retinal barrier leakage with decreased visual acuity, all of which resolved on recovery. Additional study of these features and their potential role in elucidating the pathogenesis of cerebral malaria is warranted.

Breach and restoration of retinal immune privilege: barrier failure, innate dysregulation, and adaptive autoimmunity.

The retina preserves vision by tightly regulating inflammation ("immune privilege") via blood-retinal barriers, neuroglial checkpoints, and tolerogenic cues. This actively maintained-and potentially restorable-state is breached in major retinal diseases through three recurrent archetypes. We synthesize 2015-2025 advances into a framework of barrier failure, innate dysregulation, and adaptive autoimmunity. Across age-related macular degeneration (AMD), diabetic retinopathy (DR), retinitis pigmentosa (RP), and non-infectious uveitis (NIU): AMD exhibits complement-microglia para-inflammation with…

New hypotheses on the pathogenesis and treatment of serous retinal detachment.

Recent experimental work has shown that, under normal conditions, most subretinal fluid is absorbed rapidly by active transport across the retinal pigment epithelium (RPE). However, in the presence of damage to the RPE blood-retinal barrier, subretinal fluid is rapidly cleared by passive forces. Thus, it is apparent that RPE defects do not by themselves cause serous retinal detachment. A hypothesis is presented that some serous detachments occur because normal metabolic transport systems of the RPE have been damaged, while the blood-retinal barrier remains intact to prevent passive drainage of…

Gut-eye axis and transporter-mediated strategies: enhancing retinal uptake of dietary nutrients.

The blood-retinal barrier (BRB) stringently restricts the delivery of dietary nutrients to retinal tissues, presenting a major bottleneck for targeted nutritional intervention. This review systematically delineates three core pathways through which food-derived bioactive compounds overcome the limited delivery efficiency of the BRB: transporter-mediated active transport across the barrier, biotransformation by gut microbiota and the liver into bioactive metabolites with superior bioavailability, and gut-retina axis-mediated remote regulation of retinal homeostasis. An innovative focus is place…

Vitreous fluorophotometry recordings in posterior segment disease.

The results of vitreous fluorophotometry in a variety of retinal and systemic diseases are described, particularly in photoreceptor and retinal pigment epithelium dystrophies, in Best dystrophy, in choriocapillaris atrophy, in choroidemia and in inflammatory conditions, and in cystoid aphakic and non-aphakic macular edema. The patterns involved in breakdown of the blood-retinal barrier are defined for various diseases, and an attempt is made to characterize each disease type by a fluorophotometric "profile."

Using Adeno-associated Virus as a Tool to Study Retinal Barriers in Disease.

Müller cells are the principal glial cells of the retina. Their end-feet form the limits of the retina at the outer and inner limiting membranes (ILM), and in conjunction with astrocytes, pericytes and endothelial cells they establish the blood-retinal barrier (BRB). BRB limits material transport between the bloodstream and the retina while the ILM acts as a basement membrane that defines histologically the border between the retina and the vitreous cavity. Labeling Müller cells is particularly relevant to study the physical state of the retinal barriers, as these cells are an integral part of…

Tyrosine kinase inhibitors enhance ciprofloxacin-induced phototoxicity by inhibiting ABCG2.

The tyrosine kinase inhibitor (TKI) class of anticancer agents inhibits ABCG2-mediated drug efflux. ABCG2 is an important component of the blood-retinal barrier, where it limits retinal exposure to phototoxic compounds such as fluoroquinolone antibiotics. Patients treated with TKIs would be expected to be at greater risk for retinal phototoxicity. Using an in vitro system, our results indicate that the TKIs gefitinib and imatinib abrogate the ability of ABCG2 to protect cells against ciprofloxacin-induced phototoxicity. We conclude that the concurrent administration of ABCG2 inhibitors with ph…

Diabetes associated pericyte metabolic signatures and pathogenesis of diabetic retinopathy.

Pericytes are metabolically active perivascular supporting cells. They are essential for establishing and maintaining the inner blood retinal barrier and retinal neurovascular homeostasis. In diabetic retinopathy, the chronic hyperglycemic environment adversely affects pericyte functions including their glucose flux and metabolism. The early alterations in pericyte metabolic signatures during diabetes make them susceptible as an early damage target. Altered pericyte glucose flux through the polyol pathway, pentose phosphate pathway, hexosamine biosynthesis pathway, protein kinase C pathway, an

Wnt Pathway Agonism as a Therapeutic Strategy for Retinal Vascular Diseases: A Narrative Review of Evidence to Date.

Breakdown of the blood-retinal barrier (BRB) is implicated in many blinding retinal diseases, including diabetic macular edema, neovascular age-related macular degeneration, and retinal vein occlusion. Anti-vascular endothelial growth factor (anti-VEGF) therapies or laser photocoagulation are currently the standard of care for retinal diseases that are characterized by breakdown of the vascular barrier function and vascular hyperpermeability. However, inadequate response to anti-VEGF therapy is seen in a substantial proportion of patients and laser photocoagulation does not restore vision loss…

Partial rescue of retinal function and sterol steady-state in a rat model of Smith-Lemli-Opitz syndrome.

The Smith-Lemli-Opitz syndrome (SLOS) is the first-described in a growing family of hereditary defects in cholesterol biosynthesis, and presents with a spectrum of serious abnormalities, including multiple dysmorphologies, failure to thrive, cognitive and behavioral impairments, and retinopathy. Using a pharmacologically induced rat model of SLOS that exhibits key hallmarks of the disease, including progressive retinal degeneration and dysfunction, we show that a high-cholesterol diet can substantially correct abnormalities in retinal sterol composition, with concomitant improvement of visual

GPR30-driven fatty acid oxidation targeted by ginsenoside Rd maintains mitochondrial redox homeostasis to restore vascular barrier in diabetic retinopathy.

Blood-retinal barrier (BRB) breakdown, a pivotal contributor to multiple retinal vascular diseases, manifests as a progressive increase in vascular permeability induced by various pathological stimuli. The functional plasticity of retinal endothelial cells can be intricately shaped by metabolic alteration. However, little is known about the mechanisms through which endothelial metabolic disorders trigger the dissolution of inter-vascular junctions and the selective approaches to targeting metabolic homeostasis. Herein, we identify AMPK-associated fatty acid oxidation (FAO) inhibition as a crit…

[Development of cGMP Analogues for the Pharmacological Treatment of Neurodegenerative Diseases of the Retina].

Hereditary retinal degenerative diseases are mostly diseases of the photoreceptors and/or the retinal pigment epithelium, which lead to loss of vision or even complete blindness. To this day, these diseases are mostly untreatable and represent a considerable burden for patients and their relatives. This review article highlights some of the challenges that arise in the development of new therapies for inherited retinal degeneration, in particular the problem of the enormous genetic heterogeneity of these diseases and the question of how new forms of treatment can be made to cross the blood ret
