PURPOSE: Hamate hook fractures are uncommon injuries for which treatment is controversial. Excision of the hamate hook is considered to be the preferred method of treatment but the effects of hamate excision are not clearly delineated. The purpose of this study was to determine what effect, if any, excision of the hamate has on flexor tendon function.
METHOD: The biomechanical effects of hamate hook excision on flexor tendon function were studied in fresh cadaveric forearm specimens with wrists fixed in 3 positions (neutral, 30 degrees extension, 30 degrees extension with 30 degrees ulnar dev

The treatment aim is restoration of adequate length of the radius, adjustment of the distal radioulnar joint, alignment of the distal radius joint surface, and prevention of reflex dystrophy syndrome. Stable fractures are treated by close reduction and a cast for 3-4 weeks, depending on bone stock quality. Instable extra-articular fractures are treated by K-wiring using various methods. In case of a comminuted dorsal or palmar area, K-wiring is unstable, so additional fixation is necessary (autogeneous bone graft + external fixator + plate or cast). The external fixator alone or in combination

INTRODUCTION: Previous reports indicated the effectiveness of surgical resection for locally recurrent rectal cancer (LRRC). Most cases with posterior invasion patterns require concomitant sacrectomy to secure negative histologic margins, although this is a highly invasive procedure. Here, we present a new minimally invasive laparoscopic surgical technique for LRRC with concomitant sacrectomy.
MATERIALS AND SURGICAL TECHNIQUE: A 64-year-old man presented with LRRC on the surface of the sacral bone. He underwent laparoscopic abdominoperineal resection with concomitant sacrectomy below the S4 v

Previous experiments have suggested that mechanical activity of electrode implants may contribute to the observed bone formation that has been attributed to the electrical currents. This was tested by implanting movable and stationary wires in the medullary canal of the rabbit femora or tibiae. The moving implants uniformly showed significant medullary bone formation at the wire, whereas the stationary ones did not. This bone persisted for as long as 8 weeks and was mostly resorbed by 12 weeks as the implants became fixed. The new trabecular bone closely resembled that typically seen at electr

A 17-month-old domestic short-hair cat was referred due to a non-union in the left tibia. The initial repair, conducted 3 months prior at another animal hospital, involved an intramedullary (IM) pin and wire to address a comminuted fracture. Unfortunately, the wire knot caused a skin tract, resulting in osteomyelitis. Although the wire knot was removed at that hospital, the draining tract persisted, continuously discharging exudate. Upon evaluation, the first surgery was reassessed and revised, involving the removal of the IM pin and the application of external skeletal fixation alongside an a

The spontaneous migration, in a 62-year-old patient, of a broken Kirschner's pin from the right sternoclavicular joint to the anterior mediastinum, with perforation of the pericardium and puncture of the main trunk of the pulmonary artery resulting in cardiac tamponade, is described. A review of the literature stresses the high risk of migration when pins or wires are used for internal fixation of the shoulder girdle, resulting sometimes in life-threatening situations and the need for major thoracic interventions. It seems mandatory to remove all foreign material after consolidation, certainly

The authors introduce the concept of a "frame structure graft" in which a harvested rib bone was adjusted to fit facial asymmetry. On the costochondral junction of the sixth or seventh rib, a 5 cm incision was made. Through a subperiosteal dissection, the rib bone was harvested. Using a reciprocating saw, the harvested rib was scored on its anterior surface as well as its posterior surface with a partial depth at different intervals. The harvested rib bone was placed on the skin surface of the unaffected side of the face and a curvature was created exactly matching that of the unaffected side

CASE: Fractures in patients with osteopetrosis pose unique technical challenges to the orthopaedic surgeon. We present a case of a 43-year-old man with osteopetrosis who underwent tension band wiring of a patella fracture. The intraoperative appearance of osteopetrotic bone, technical difficulties in passing wires through dense bone, and how this was overcome are presented.
CONCLUSIONS: A simple transverse fracture of the patella in a patient with osteopetrosis can be fixed successfully using a standard tension band construct.

INTRODUCTION AND IMPORTANCE: Kirschner wires (K-wires) are commonly used implants in the treatment of bony fracture. The migration of K-wire has been reported in the literature, but such migration into the urinary bladder is extremely rare.
PRESENTATION OF CASE: We report a case of an asymptomatic patient with a migrating K-wire in the urinary bladder, who came to our follow-up clinic after treatment of a hip fracture. The patient was absolutely fine but follow-up image showed K-wire in the urinary bladder. In view to prevent further migration and injuries, the laparotomy was planned and wire

Kirschner wires (K-wires) are often used for osteosynthesis particularly in the upper limb. Postoperative K-wire migration through the tissues is a well-recognised and significant complication of surgery of the clavicle, the wire ending up in the lungs, the oesophagus, the aorta, or the subclavian artery. Localisation of a K-wire migration into the spinal cord is very rare. We report the case of a 34-year-old man with K-wire migration into the spinal cord through the intervertebral foramen of T2, two months after surgery for nonunion of a fracture of the lateral clavicle. Apart from acute resp

INTRODUCTION: Comminuted fracture of patella is most commonly treated by tension band wiring with cerclage wiring. Broken cerclage wiring after such surgery is not uncommon but intra-articular migration of broken cerclage wire is rarely known and described complication.
CASE REPORT: A 51-year-old male patient of Indian origin which was treated, 15 months back for fracture patella with tension band wiring with cerclage wiring, presented with painful restriction of knee movements for the last 4 weeks. X-ray showed cerclage wire had broken at multiple sites with one broken piece migrated intra-a

Elastic titanium nails are widely used for the established procedure of Elastic Stable Intramedullary Nailing (ESIN), but are costly. Thirty-five diaphyseal fractures in various long bones in children were treated by flexible nailing using stainless steel Kirschner (K) wires. The results were comparable with reports by other authors using titanium nails and stainless steel K wires. Cost-effective K wires and instrumentation are easily available. A stainless steel K wire is 92% cheaper than a Nancy nail and 84% cheaper than a Synthes titanium elastic nail. K wires can be used for flexible naili

Ten cases of unstable scaphoid nonunions treated with Herbert screw fixation were compared to nine cases treated with Herbert screw fixation supplemented with a single Kirschner wire (K-wire) across the fracture site. Corticocancellous bone graft from the distal radius was used in all cases to reconstruct the scaphoid. The age of the patients and duration of the nonunions were similar in both groups. Postoperative radiographs demonstrated that only 5 of 10 patients with Herbert screw fixation alone healed, while 8 of 9 patients with Herbert screw and K-wire fixation healed. The time to union a

BACKGROUND: To show the benefits of using a novel approach to closure of the median sternotomy through a mechanical model and mechanical testing. Simple cannulated screws are placed on either side of the sternotomy. Conventional stainless steel wire is passed through the cannula of each screw and the sternotomy is closed in the usual manner.
METHODS: Hertzian contact analysis was used to estimate the stress between the wire and the sternum. Mechanical testing was used to compare using wire on its own with a sternum screw plus wire. Ten samples of balsa wood (sternum substitute) had wire place

Any traumatic event that produces maxillofacial and/or mandibular fractures generally results in gross and usually severe patient disfigurement and often results in the patient's inability to eat and drink. These fractures are exceptionally rewarding cases as simple techniques may be performed resulting in a successful functional outcome (ability to eat and drink) within a very short period of time (24 hours) after fracture stabilization. A markedly improved cosmetic appearance follows shortly thereafter once inflammation and edema resolve. The primary principle of fracture treatment, ie, prov

In the management of recent fractures of the zygomatic bone and arch we prefer transcutaneous repositioning with a sharp hook. If additional fixation is necessary, wire sutures are used. The advantages of osteosynthesis with wire suturing and the treatment of simultaneous fracture of the orbital floor are presented and discussed.

BACKGROUND: Rigid bone fixation is the standard of care for all bone reconstructions except that after sternotomy. Sternal reconstruction after median sternotomy using rigid fixation with plates may improve bone healing and reduce pain when compared with wire cerclage.
METHODS: One-hundred forty patients at six centers who were determined preoperatively to be at high risk for sternal wound complications were randomly assigned to sternal closure with rigid plate fixation (n=70) or wire cerclage (n=70). Sternal healing was evaluated at 3 or 6 months by a core laboratory using computed tomograph

Distal humerus fractures commonly occur in adults with low bone mineral density causing major technical challenges for orthopedic surgeons. Persian fixation method was introduced as a novel technique to stabilize small fragments in comminuted distal humerus fractures using a set of K-wires and a reconstruction plate. The present study aims to measure this technique's stiffness and stability of this technique and analyze the effect of influential parameters with numerical simulation and biomechanical testing on a cadaveric specimen. Validation of the finite element (FE) model was conducted base

This study examines the risk of injury to medial soft-tissue structures during the fixation of calcaneal fractures performed through a lateral approach. An L-shaped lateral incision was performed on 10 fresh cadaver feet. The calcaneus was divided into three zones for study. Ten K-wires were inserted in a standard configuration from a lateral starting point extending through the medial cortex. The feet were dissected medially to measure the distance from each exiting K-wire to the medial and lateral plantar vessels, the calcaneal branch of the posterior tibial nerve, and the tendons of the fle

BACKGROUND: Reconstructing both coracoclavicular ligaments following acromioclavicular dislocation has recently been reported to restore the function of the acromioclavicular joint better than traditional procedures. Knowing the appropriate position and orientation of the bone tunnels and the potential risks of neurovascular injuries leads to safe reconstruction. We aimed to answer the following questions: what is the difference in the accurate clavicular bone tunnel positions (BTPs) during coracoclavicular ligament reconstruction between sex, and what are the potential risks for neurovascular
