His-bundle pacing in a patient with persistent left superior vena cava.

The presence of a persistent left superior vena cava was encountered in a 53-year-old woman undergoing a dual-chamber pacemaker implant for sick sinus syndrome. Active fixation leads were used and positioned in the right atrium and right ventricle via the left superior vena cava draining into the coronary sinus. The natural course of the right ventricular lead riding up the lateral atrial wall and curving backward toward the septum as it exits from the coronary sinus os was found to facilitate the positioning of the lead tip close to the His bundle without the use of a specially designed guidi

[Nodo-ventricular accessory pathway unmasked by a block in the trunk of the bundle of His].

A case is reported in which, contrary to the unusual finding, conduction was slower in an accessory pathway than in the node-His pathway. Only when this latter became blocked (in the trunk of the bundle of His) following on the longest refractory period, was a pre-excitation syndrome unmasked related to the right nodo-ventricular fibres (the fibres of Mahaim).

Impact of electroanatomical mapping-guided lead implantation on procedural outcome of His bundle pacing.

AIMS: Conventional His bundle pacing (HBP) can be technically challenging and fluoroscopy-intense, particularly in patients with His-Purkinje conduction disease (HPCD). Three-dimensional electroanatomical mapping (EAM) facilitates non-fluoroscopic lead navigation and HB electrogram mapping. We sought to assess the procedural outcome of routine EAM-guided HBP compared with conventional HBP in a real-world population and evaluate the feasibility and safety of EAM-guided HBP in patients with HPCD. METHODS AND RESULTS: We included 58 consecutive patients (72 ± 13 years; 71% male) who underwent an

Utilization of Permanent His-Bundle Pacing for Management of Proarrhythmia Related to Biventricular Pacing.

Biventricular pacing has occasionally been associated with ventricular proarrhythmia, which at times can be refractory to conventional antiarrhythmic drug therapy and ablative intervention. Recently, permanent His-bundle pacing has emerged as a reasonable alternative to right ventricular and biventricular pacing. In addition, by maintaining the physiologic pattern of ventricular electrical activation, it is less likely to be arrhythmogenic. We report the use of permanent His-bundle pacing to manage ventricular proarrhythmia that developed after initiation of biventricular pacing and was unresp

Extraction of the permanent His bundle pacing lead: Safety outcomes and feasibility of reimplantation.

BACKGROUND: Permanent His bundle pacing (HBP) is a physiological alternative to right ventricular pacing. However, concerns remain about the feasibility and safety of lead extraction from the His bundle region. OBJECTIVE: The aim of our study was to assess the safety and feasibility of extraction of chronically implanted permanent HBP leads in addition to report on the feasibility of reimplanting in the His bundle region. METHODS: Patients undergoing extraction of leads from the His bundle location for standard indications were studied. The primary outcomes were removal success rates, need f

Intra-His bundle block in second-degree Mobitz I atrioventricular block with right bundle branch block.

Because second-degree Mobitz I atrioventricular (AV) block is usually associated with a disturbance at the AV nodal level, and rarely at the infra- or intra-His bundle level, it has been considered as mostly benign in character. We report on a patient with intra-His bundle block in second-degree Mobitz I AV block with right bundle branch block.

HIS bundle electrogram in rheumatic mitral valve disease with special reference to Bachmann's bundle block in P mitrale.

HBEs were recorded from either the right or left ventricle or simultaneously from both in 26 patients with chronic rheumatic mitral stenosis alone or in association with other mild valvular lesions during the diagnostic cardiac catheterization. Eleven of the patients had auricular fibrillation. Of the remaining 15 patients with sinus mechanism and P mitrale in the surface electrocardiogram, 12 were noted to have H potential preceded the termination of P wave and gave P2H interval of negative value- so-called "Bachmann's bundle block". Among these, double atrial activities (A and A' waves) coul

Permanent His Bundle Pacing for Cardiac Resynchronization Therapy in Patients With Heart Failure and Right Bundle Branch Block.

Background Cardiac resynchronization therapy utilizing biventricular pacing is an effective therapy for patients with left ventricular (LV) systolic dysfunction, left bundle branch block, and heart failure. Benefits of biventricular pacing may be limited in patients with right bundle branch block (RBBB). Permanent His bundle pacing (HBP) has recently been reported as an option for cardiac resynchronization therapy. The aim of the study was to assess the feasibility and outcomes of HBP in patients with RBBB and heart failure. Methods HBP was attempted as a primary or rescue (failed LV lead impl

[His bundle block 2:1 during paroxysmal supraventricular tachycardia involving an occult accessory atrio-His pathway].

A high degree AV block during paroxysmal supraventricular tachycardia is an uncommon and short-lasting finding in patients with intranodal re-entry circuits. The AV block is the result of temporary inadequacy of the infranodal conduction system--which is not part of the circuit--to cope with the sudden increase in heart rate in terms of refractory periods and conduction velocity. In a patient with paroxysmal reciprocating tachycardia at a very high rate (230-250 bpm), 2:1 intra-hisian AV block was constantly observed after arrhythmia initiation by atrial extrastimuli. This persisted for 1-4 mi

Mid-term comparison of new-onset AHRE between His bundle and left bundle branch area pacing in patients with AV block.

BACKGROUND: Atrial high-rate episodes (AHRE) detected by cardiac implanted electronic devices are known markers for adverse cardiac events. Previous studies have shown that the incidence of new-onset AHREs in patients with right ventricular pacing reaches 50%. At the same time, His bundle pacing (HBP) and left bundle branch area pacing (LBBAP) were associated with significantly fewer AHRE. This study aimed to compare the incidence of new-onset AHRE between HBP and LBBAP in patients with atrioventricular block and no history of atrial fibrillation. METHODS: One hundred and forty-two patients,

Safety and performance of the Medtronic 3830 lead in His-bundle and Left bundle branch area pacing: A single-center experience.

BACKGROUND: Conduction system pacing (CSP) using His Bundle Pacing (HBP) and Left Bundle Branch Area Pacing (LBAP) is an evolving alternative to traditional right ventricular pacing (RVP), promising better physiological outcomes. This study evaluates the safety, feasibility, and performance of HBP and LBAP with Medtronic SelectSecure 3830 leads. METHODS: We conducted a single-center retrospective analysis of 490 patients undergoing HBP or LBAP. The study assessed implant success rates, pacing thresholds, device longevity, and complication rates over an average follow-up of 28 months for HBP a
