Should an emergency physician be a "surgeon" in a rural area? A case of blunt cardiac rupture successfully treated by an emergency physician.

OBJECTIVE: Blunt cardiac rupture is a life-threatening injury that requires surgical repair by cardiovascular or trauma surgeons. We report a case of blunt cardiac rupture in a rural area in which emergency physicians performed emergency department thoracotomy and surgical repair to save the patient's life. PATIENT AND METHODS: This case involved an 18-year-old female who was injured in a traffic accident and underwent emergency thoracotomy and surgical repair. RESULTS: The patient's left thorax was deformed, and sonographic assessment revealed pericardial effusion. She experienced cardiopul

Expert Opinions on the Debate of Pericardial Reconstruction in Cardiovascular Surgery: To Close or Not to Close?

BACKGROUND: As of 2019, pericardial closure was performed in only a small portion of the over 320,000 cardiac surgeries performed annually. However, evidence regarding the benefits of pericardial closure or reconstruction has been accruing, particularly with the publication of the RECON study in 2019.  Methods: This group of authors convened to try to arrive at consensus expert opinion regarding pericardial reconstruction. Structured topic questions initially were used to stimulate discussion. Subsequently, a survey of proposed expert opinion statements was conducted among the authors. Based o

Incidence and Predictors of Cardiac Tamponade in Contemporary Thermal Catheter Ablation of Atrial Fibrillation.

BACKGROUND: Contributing factors to cardiac tamponade after contemporary thermal ablation remain poorly defined, especially in relation to ablation strategies. We aimed to clarify the incidence and identify the risk factors associated with cardiac tamponade following thermal atrial fibrillation (AF) ablation. METHODS: Data from consecutive AF ablation procedures at 20 cardiovascular centers between 2022 and 2024 were analyzed. RESULTS: Among 12 820 AF ablation procedures (mean age 68 ± 11 years; 8877 men; 6,963 paroxysmal AF; 9824 index procedures), radiofrequency and cryoballoon ablation we

Systemic sclerosis and the risk of perioperative major adverse cardiovascular events for inpatient non-cardiac surgery.

AIM: We investigated the association between systemic sclerosis (SSc) and perioperative cardiovascular risk for inpatient non-cardiac surgical procedures. METHODS: We used data from the National Inpatient Sample (NIS) for the year 2014 to identify patients undergoing inpatient non-cardiac surgery. SSc and major adverse cardiovascular events (MACE) were defined by International Classification of Diseases 9th Revision diagnosis codes. Univariate and multivariate analyses were performed. We adjusted for demographic information, socioeconomic status, cardiac comorbidities, cardiovascular risk fac…

Vasopressin therapy in cardiac surgery.

BACKGROUND: Arginine vasopressin (AVP) is a naturally occurring peptide with diverse effects mediated through selective V1 and V2 receptors. About 10% of patients undergoing cardiopulmonary bypass develop postoperative vasodilatory shock requiring high-dose catecholamines. We sought to examine the role of AVP therapy in cardiac surgery. METHODS: A search of Medline was conducted through September 2018 using key words and medical subject headings (MeSH) relating to AVP, copeptin, and cardiac surgery. A systematic review was performed on articles as they pertained to AVP for use as a vasopresso
