[A comparison of cerebral angiography and computer tomography in patients with strokes (author's transl].

The computer tomographic findings in 230 patients with a clinical diagnosis of strokes or cerebral infarcts are described. In 185 of these patients cerebral angiography was performed in addition. The size and localisation of infarcts as shown on the computer tomogram were compared with the changes found in the carotid angiogram and were also related to the localisation of occlusion or stenosis of the corresponding extra- and intracranial vessels. In the presence of a normal carotid angiogram, CT may show infarcts of varying localisation and size; with stenoses or occlusion of the extracranial …

Cerebral arterial revascularization: the value of repeated angiography in selection of patients for operation.

Three patients with occlusive cerebrovascular disease were evaluated for possible extracranial-intracranial arterial anastomosis. Cerebral angiography repeated at a later date revealed disappearance of the initial vascular stenosis or occlusion and development of extensive collateral circulation. It was concluded in these cases that surgical revascularization would not improve upon natural revascularization. Repeated angiography provides information of value to the physician who must make a decision about such surgical treatment.

Symptomatic middle cerebral artery stenosis and occlusion: comparison of three-dimensional time-of-flight magnetic resonance angiography with conventional angiography.

The usefulness of magnetic resonance (MR) angiography using the three-dimensional time-of-flight method for the characterization of symptomatic middle cerebral artery (MCA) occlusive lesions was evaluated in 10 patients with MCA occlusion and 10 with MCA stenosis. All lesions were symptomatic and documented by conventional angiography. There was no false-negative MR angiogram that failed to demonstrate the MCA occlusive lesion. MR angiography correctly evaluated the location of lesions and the difference between stenosis and occlusion. Stenosis appeared as a focal signal loss (< 1.0 cm) of the

Fifty-percent diluted contrast medium provides acceptable image quality in cerebral angiography.

PURPOSE: Cerebral angiography is essential for neuro-endovascular treatment but carries a risk of contrast-induced nephropathy (CIN). Although dilution of iodinated contrast medium may reduce this risk, its impact on diagnostic image quality has not been fully clarified. This study evaluated whether 50% diluted contrast medium provides sufficient diagnostic confidence during preoperative angiography for unruptured cerebral aneurysms. METHODS: Twenty-two patients with unruptured anterior circulation aneurysms were prospectively enrolled. Each patient underwent paired acquisitions with standard…

Contrast-Induced Encephalopathy Following Cerebral Angiography: A Case Report and Literature Review.

Contrast-induced encephalopathy (CIE) is a rare neurotoxic complication that occurs following administration of contrast agents. Diagnosis is often challenging due to the wide variety of clinical presentation, which can include transient neurological deficits such as vision changes, sensory deficits, paralysis, and altered mental status. It is self-limited, and symptoms usually resolve within 48-72 hours. It is important to consider contrast-induced encephalopathy in a patient that experiences neurological defects following procedures using contrast agents. This report describes a 58-year-old

Comparative analysis of cerebral angiography via right versus left radial artery approaches.

PURPOSE: The use of right transradial approach (TRA) in cerebral angiography has been mature, but it is still limited by anatomical variation of the right radial artery or left-sided vertebral artery lesions. This study aimed to compare outcomes between right and left TRA for cerebral angiography and evaluate the safety of left TRA. METHODS: The study cohort comprised 80 patients who underwent cerebral angiography via TRA. The patients' clinical data were analyzed retrospectively. Bilateral proximal and distal radial artery diameters were measured preoperatively using ultrasonography. Punctur…

Pressurized Contrast-Related Posterior Reversible Encephalopathy Syndrome After Cerebral Digital Subtraction Angiography.

Posterior reversible encephalopathy syndrome is a multifactorial neurological disorder characterized by clinical and neuroimaging findings. The "vasogenic theory" suggests that, in this condition, disrupted cerebrovascular autoregulation leads to blood-brain barrier failure and vasogenic edema. Posterior reversible encephalopathy syndrome is commonly associated with hypertension, chemotherapy, and transplantation, but neurosurgical patients, including those undergoing digital subtraction angiography, are also at elevated risk. A patient with a history of traumatic brain injury underwent survei

Outcomes of Combined Revascularization Surgery for Moyamoya Disease without Preoperative Cerebral Angiography.

OBJECTIVE: Cerebral angiography is the gold standard for diagnosing moyamoya disease (MMD), whereas magnetic resonance (MR) imaging/angiography is becoming more popular in the field of cerebrovascular disease due to its low invasiveness. Although there are concerns about using only MR imaging/angiography for preoperative analysis of MMD, considering the underestimation of pre-existing transdural collateral circulations and dangerous collaterals related to the risk of hemorrhage, we retrospectively reviewed our 10-year experience of MR imaging-first diagnosis and analyzed the perioperative outc…

Ophthalmic artery arising from the anterior cerebral artery diagnosed by MR angiography.

Extremely rarely, the ophthalmic artery (OphA) arises from the A1 segment of the anterior cerebral artery (ACA). Discovery of this anomalous OphA during angiography or surgery has been reported in several patients. We report a case in which an OphA of ACA origin was diagnosed by magnetic resonance (MR) angiography and confirmed by selective cerebral angiography. To our knowledge, this is the first report of this variation with MR angiography. Careful observation of MR angiographic images is important for detecting rare arterial variations. Partial maximum-intensity-projection images are useful…

Computerized Tomographic Angiography for Detection of Cerebral Vasospasm after Ruptured Intracranial Aneurysm.

OBJECTIVE: Digital subtraction angiography (DSA) is the gold standard to diagnose cerebral vasospasm but it is usually not available due to lack of expertise and proper equipment. The present study aimed to compare the diagnostic accuracy of brain computerized tomographic angiography (CTA) in detecting cerebral vasospasm after intracranial aneurysmal rupture. MATERIAL AND METHOD: Between January 2011 and October 2014, 20 patients who were suspected of cerebral vasospasm after aneurysmal subarachnoid hemorrhage (SAH) were prospectively enrolled. All patients underwent brain CTA andDSA within 2

Intraoperative computed tomographic angiography in cerebral aneurysm surgery: a pilot feasibility study.

OBJECTIVE: To assess the feasibility and potential utility of intraoperative computed tomographic angiography (iCTA) in identifying unexpected residual aneurysms and major cerebral artery occlusion after cerebral aneurysm surgery. METHODS: We prospectively studied 6 consecutive patients with ruptured anterior circulation aneurysms who underwent craniotomy and clipping. iCTA was performed in all cases after the surgeon was satisfied that the aneurysm was completely obliterated and the clip did not occlude a major artery. We analyzed the iCTA images with regard to residual aneurysm and major ar

Spatio-temporal data fusion for 3D+T image reconstruction in cerebral angiography.

This paper provides a framework for generating high resolution time sequences of 3D images that show the dynamics of cerebral blood flow. These sequences have the potential to allow image feedback during medical procedures that facilitate the detection and observation of pathological abnormalities such as stenoses, aneurysms, and blood clots. The 3D time series is constructed by fusing a single static 3D model with two time sequences of 2D projections of the same imaged region. The fusion process utilizes a variational approach that constrains the volumes to have both smoothly varying regions

Incidence and predictors of silent embolic cerebral infarction following diagnostic coronary angiography.

BACKGROUND: Coronary angiography (CAG) is an invasive diagnostic procedure, which could lead to procedure related complications. One of the well known post-procedural complications is cerebral embolic infarction with or without symptoms. Silent embolic cerebral infarction (SECI) has clinical significance because it can progress to a decline in cognitive function and increase the risk of dementia in the long term. The aim of this study was to detect the incidence and predictors of SECI after diagnostic CAG using diffusion-weighted magnetic resonance imaging (DW-MRI). METHODS: A total of 197 pa

Dynamic 3-D contrast-enhanced angiography of cerebral tumours and vascular malformations.

We describe the applicability and clinical use of dynamic 3-D contrast-enhanced MR subtraction angiography performed at 3 T with parallel imaging and intelligent k-space readout for imaging both treated and untreated cerebral arteriovenous malformations (AVMs), AV fistulae (AVFs) and brain tumours. An in-plane submillimetre spatial resolution with temporal resolution of one image per 1.3 s was obtained. The spatial resolution was comparable to that of other MRA techniques (i.e. TOF or PC MRA) while the scanning time was markedly reduced and the evaluation of both the arterial and venous vessel

Three-dimensional computerized tomography angiography-guided surgery of acutely ruptured cerebral aneurysms.

OBJECT: The aim of this study was to assess whether aneurysm surgery can be performed in patients with ruptured cerebral aneurysms by using three-dimensional computerized tomography (3D-CT) angiography alone, without conventional catheter angiography. METHODS: In a previous study, 60 patients with subarachnoid hemorrhage (SAH) from ruptured aneurysms were prospectively evaluated using both 3D-CT and conventional angiography, which resulted in a 100% accuracy for 3D-CT angiography in the diagnosis of ruptured aneurysms, and a 96% accuracy in the identification of associated unruptured aneurysm

Posterior cerebral artery laterality on magnetic resonance angiography predicts long-term functional outcome in middle cerebral artery occlusion.

BACKGROUND AND PURPOSE: Prominent posterior cerebral artery (PCA) laterality upon 3-dimensional time-of-flight magnetic resonance angiography is often encountered in patients with middle cerebral artery occlusion. We hypothesized that this sign is correlated with improved functional outcome in patients with middle cerebral artery occlusion treated with intravenous recombinant tissue plasminogen activator. METHODS: Fifty acute ischemic stroke patients with middle cerebral artery occlusion were treated with intravenous recombinant tissue plasminogen activator from April 2007 to October 2009. Al

Swarm Learning for the Detection of Middle Cerebral Artery Occlusion On CT Angiography.

PURPOSE: Swarm Learning has been introduced as a decentralized alternative to Central Learning. We aimed to evaluate whether Swarm Learning (SL) is equivalent to Central Learning (CL) for training AI models to detect middle cerebral artery (MCA) occlusions on CT angiography (CTA). METHODS: This retrospective study included CTAs of patients suspected of ischemic stroke and negative controls from two institutions and three scanners between October 2010 and March 2024. After manual segmentation of occluded vessels by two raters, two AI algorithms (MedNeXt‑S, Swin UNETR) were trained using CL and

Simulation of MR angiography imaging for validation of cerebral arteries segmentation algorithms.

BACKGROUND AND OBJECTIVE: Accurate vessel segmentation of magnetic resonance angiography (MRA) images is essential for computer-aided diagnosis of cerebrovascular diseases such as stenosis or aneurysm. The ability of a segmentation algorithm to correctly reproduce the geometry of the arterial system should be expressed quantitatively and observer-independently to ensure objectivism of the evaluation. METHODS: This paper introduces a methodology for validating vessel segmentation algorithms using a custom-designed MRA simulation framework. For this purpose, a realistic reference model of an in
