Vitamin D is a lipid-soluble steroid hormone whose role has been well-known in calcium and bone metabolism for a long time. In the last decades, vitamin D deficiency, which affects approximately 24% to 40% of the population, has been linked to the pathogenesis of cardiovascular disorders including cerebrovascular diseases, one of the leading causes of death worldwide. According to epidemiological studies, vitamin D deficiency is associated with an increased risk and more severe outcome of ischemic stroke, vascular cognitive impairment, and cerebral cavernous malformations. Although several con

BACKGROUND: Acute ischemic stroke (AIS) remains the leading cause of mortality and disability worldwide. While revascularization therapies-such as intravenous thrombolysis (IVT) and endovascular thrombectomy (EVT)-have significantly improved outcomes, their success is strongly influenced by the status of cerebral collateral circulation. Collateral vessels sustain cerebral perfusion during vascular occlusion, limiting infarct growth and extending therapeutic windows. Despite this recognized importance, standardized methods for assessing collateral status and integrating it into treatment strate

Leptomeningeal collaterals (LMCs) are crucial in mitigating the impact of acute ischemic stroke (AIS) by providing alternate blood flow routes when primary arteries are obstructed. This article explores the evolutionary pathophysiology of LMCs, highlighting their critical function in stroke and the genetic and molecular mechanisms governing their development and remodelling. We address the translational challenges of applying animal model findings to human clinical scenarios, emphasizing the need for further research to validate emerging therapies-such as pharmacological agents, gene therapy a

Mass transport can be generally defined as movement of material matter. The circulatory system then is a biological example given its role in the movement in transporting gases, nutrients, wastes, and chemical signals. Comparative physiology has a long history of providing new insights and advancing our understanding of circulatory mass transport across a wide array of circulatory systems. Here we focus on circulatory function of nonmodel species. Invertebrates possess diverse convection systems; that at the most complex generate pressures and perform at a level comparable to vertebrates. Many

A study of cigarette smoking was undertaken in a pair of craniopagus twins to determine how a transfer of products of smoking is occurring between the twins. Alternately and independently, one twin smoked a nicotine-free cigarette, then the second twin smoked a nicotine-containing cigarette. The procedure enabled the investigators to study the migration of nicotine and carbon monoxide from one twin to the other. Salivary determination provided a noninvasive method of measuring cross circulation in conjoined twins. Measurements of salivary nicotine, however, indicated that, although the nicotin

Moyamoya disease (MMD) is a chronic cerebrovascular disorder marked by the narrowing of the internal carotid artery and the formation of atypical collateral blood vessels. To reduce the risk of ischemic or hemorrhagic events, surgical revascularization is considered the standard treatment. Also, a major postoperative concern is cerebral hyperperfusion syndrome (CHS), which results serious complications such as seizures and intracranial hemorrhage. This systematic review and meta-analysis aimed to evaluate the incidence of CHS in patients with MMD undergoing cerebral revascularization surgery.

BACKGROUND: Spreading depolarization describes a near-complete electrical discharge with altered local cerebral blood flow. It is described in association with acute and chronic diseases like hemorrhagic stroke or migraine. Moyamoya vasculopathy is a chronic, progressive cerebrovascular disorder leading to cerebral hypoperfusion, hemodynamically insufficient basal collateralization, and increased cortical microvascularization.
METHODS: In a prospective case series, we monitored for spontaneous spreading depolarization activity by using intraoperative laser speckle imaging for real-time visual

Although cerebral hyperperfusion syndrome is rarely encountered, this condition can be serious after procedures that increase cerebral blood flow; most reports are related to carotid revascularization. Sharp cerebral hemodynamic changes, along with impaired cerebrovascular autoregulation, are the main mechanisms contributing to this syndrome. We report a patient who underwent surgical correction for congenital supravalvular aortic stenosis and presented with postoperative hypertension, headache, impairment of consciousness, and cerebral edema 2 days after operation. He recovered well when the

Inflammation and microvascular dysfunction have been implicated in a variety of pathologic conditions affecting the brain. Features of the inflammatory response that are common to many of these pathological conditions and that are manifested in the neurovasculature include oxidative stress, diminished endothelial barrier function (increased vascular permeability), increased expression of endothelial cell adhesion molecules, and the recruitment of rolling and adherent leukocytes and platelets. The evidence implicating leukocyte-endothelial cell adhesion in cerebral microvessels as a rate-determ

Continuous brain blood supply in healthy individuals is warranted by brain autoregulation, the borders of which are not completely strict and can be changed as a part of dynamic physiological control. In acute neurological diseases like hypertensive encephalopathy, brain infarction, intracerebral hemorrhage and subarachnoid hemorrhage, a great attention should be paid to cerebral perfusion pressure preservation (which represents a difference between mean arterial pressure and intracranial pressure) when blood pressure is regulated. To preserve neurological function it is necessary to know anat

The paper describes the results of examining intracerebral blood flow in patient with different stages of dyscirculatory encephalopathies. Transcranial Doppler was applied. Ultrasound examination of the great arteries of the head was made by determining the linear and volumetric velocities of blood flow. Progressive cerebral atherosclerosis was found to result in an earlier decrease in cerebrovascular blood flow.

To investigate the possible existence of chronic selective hemodynamic impairment in the arterial borderzone regions of the brain, we used positron emission tomography (PET) to measure regional mean vascular transit time (rt, equal to the ratio of regional cerebral blood volume to cerebral blood flow) and regional oxygen extraction fraction (rOEF) in 32 patients with either severe internal carotid artery stenosis or occlusion and 11 normal controls. Twenty-four of the patients had had TIAs or amaurosis fugax from 1 to 60 days before PET; all had normal brain CT. We used a stereotactic localiza

The effect of altered perfusion pressure (cephalic arterial pressure minus sagittal venous pressure) on cerebral blood flow was examined in chronically catheterized fetal sheep under ganglionic blockade. Perfusion pressure was decreased by partial inflation of a brachiocephalic artery occluder and increased by an intravenous angiotensin II infusion. Cerebral flood flow was measured with radiolabeled microspheres. On average, 45% +/- 3% (SEM) reductions in perfusion pressure were accompanied by 49% +/- 4% decreases in cerebral blood flow. Increases in perfusion pressure of 42% +/- 3% were assoc

Intraoperative Doppler exploration during neurosurgery was used during treatment of intracranial aneurysms, arteriovenous malformations (AVM) and cerebral revascularization by extra-intra cranial anastomosis (EICA). It was also employed to study behavior of cortical arteries under different conditions. During treatment of aneurysms, Doppler can show, prior to clipping, whether blood flow through. The flow is regular--spasm of supply artery provokes acceleration of blood rate. After clipping it is possible to evaluate whether artery is still permeable without stenosis or torsion. In AVM, Dopple

UNLABELLED: Cerebrovascular inflammation is prevalent in a majority of Alzheimer's patients. Inflammatory cytokines, such as tumor necrosis factor-alpha (TNF-alpha), circulating in the plasma have been shown to cause the inflammation of blood-brain barrier (BBB) endothelium lining the cerebral microvasculature. The BBB inflammation has been implicated in the increase of toxic Aβ accumulation within Alzheimer's disease (AD) brain. TNF-alpha in the peripheral circulation can aggravate the accumulation of amyloid-beta (Aβ) peptides in Alzheimer's disease brain. In the current study, we have shown

BACKGROUND: Normal pressure hydrocephalus (NPH) is one of the few remediable causes of decline in gait and cognitive function in the ageing population. The roles of the cerebral circulation including haemodynamic reserve and cardiovascular co-morbidity in the pathogenesis, management and prognostication of NPH remain ill-defined. In this systematic review, we have updated Owler & Pickard’s review of 2001 to examine whether: global/regional Cerebral Blood Flow (CBF) changes are specific to NPH, appropriately coupled to cerebral metabolism and/or reflect cerebral ischaemia; changes in global/reg
