[The skin and mucosa of the head and neck area...What was the innervation again?].

The skin innervation of the head and neck area is largely provided by the trigeminal nerve and the cervical plexus. The separation between the two areas is approximately on the line crown-jaw-chin. The face is provided by the trigeminal nerve (nervus V), while the area below it up to the shoulders is provided by the cervical plexus. The innervation from the oral mucosa to the esophagus is provided by the trigeminal nerve, glossopharyngeus and vagus. This segmental distribution allows for a clear organization of the sensory input, which is clinically important in diagnosing disorders.

The role of magnetic resonance imaging in the management of traction injuries to the adult brachial plexus.

Magnetic resonance imaging (MRI) of the cervical spine and brachial plexus was performed on 26 consecutive patients presenting with traction injuries of the brachial plexus during 1996 and 1997. These included T1 and T2 weighted coronal, sagittal and axial images of the cervical spine and coronal images of the brachial plexus. The results were compared with surgical findings, intraoperative neurophysiology, and subsequent clinical progress. Operations for exploration and repair have been performed in 23 and 26 patients scanned. Evidence of root avulsion was seen in 11 patients in the form of d

Clavipectoral fascial plane block for clavicle fracture surgery: A case report.

BACKGROUND: Regional anaesthesia can be an attractive alternative anaesthetic approach in clavicle surgery, but it requires the peripheral block of multiple cervical and brachial nerves that transmit nociceptive information. Deep cervical plexus blocks, as paravertebral nerve block, can lead to severe side effects, such as unilateral diaphragmatic paralysis. CASE REPORT: A 66-year-old male patient, American Society of Anesthesiologists physical status III, was scheduled for open reduction and internal fixation of the right clavicle with plates and screws after a high-energy trauma. Pre-anaest…

Face and neck neurogenic neoplasms.

Surgeons should be aware that any mass in the region of the cranial nerves, brachial plexus, cervical sympathetic plexus, or a major peripheral nerve can be of neurogenic origin. Solitary neurogenic tumors of the head and neck can simulate metastatic masses or congenital lesions. If they are resected unrecognized and/or without regard to their nerve origin, major and permanent nerve defects can unnecessarily occur. Thirty-two patients, 16 males and 16 females, ranging in age from 5 to 69 years, had 33 extracranial solitary neurogenic neoplasms resected. The nerves involved were the cervical sy

Diagnostic value of combined magnetic resonance imaging examination of brachial plexus and electrophysiological studies in multifocal motor neuropathy.

BACKGROUND/AIM: Multifocal motor neuropathy (MMN) is an immune-mediated disorder characterised by slowly progressive asymetrical weakness of limbs without sensory loss. The objective of this study was to investigate the involvement of brachial plexus using combined cervical magnetic stimulation and magnetic resonance imaging (MRI) of plexus brachialis in patients with MMN. We payed special attention to the nerve roots forming nerves inervating weak muscles, but without detectable conduction block (CB) using conventional nerve conduction studies. METHODS: Nine patients with proven MMN were inc

Ultrasound anatomy of the neck: The infrahyoid region.

The infrahyoid region of the neck includes the visceral, anterior cervical, posterior cervical, carotid, retropharyngeal, and perivertebral spaces. The visceral space contains the thyroid, parathyroid glands, larynx, hypopharynx, the cervical trachea, and esophagus, the recurrent laryngeal nerve. The carotid space contains two parts, which extend from the skull base to the aortic arch and are delimited by the three layers of the deep cervical fascia (superficial, middle, and deep). It contains the internal carotid artery, the internal jugular vein, cranial nerves (IX-XII), the sympathetic plex

Use of ultrasound-guided percutaneous perineural corticosteroid injections for refractory cervical pain secondary to vertebral venous system abnormality in a dog.

A 2-year-old neutered male Greyhound was presented for evaluation of chronic, progressive right forelimb lameness and cervical pain. On presentation, the dog was ambulatory with mild to moderate right thoracic limb lameness and pain on cervical palpation with a right thoracic limb nerve root signature. Cervical magnetic resonance imaging (MRI) identified moderate right-sided C6 internal venous plexus enlargement. Medical management and physiotherapy did not result in clinical improvement. Repeat cervical MRI identified right-sided C5-T1 internal vertebral venous plexus enlargement with associa

[Permanent local nerve injuries after carotid endarterectomy].

UNLABELLED: Functional assessment of nerves, especially motor rami of cranial nerves, in patients at postoperative period after carotid endarterectomy (CEA), is particularly important in case of necessity of contralateral carotid artery surgery. Bilateral damage to recurrent laryngeal or hypoglossal nerve is a potentially life-threatening complication. Sensory disturbances due to intraoperative injuries of cervical plexus branches may cause residual discomfort in numerous patients. The aim of this study was the assessment and comparison of frequency of persistent (for more than 12 months posto…

Referral diagnosis versus electroneurophysiological finding. Two years electroneuromyographic consultation in a rehabilitation clinic.

This study describes the referral practice to an electrodiagnostic unit in a rehabilitation clinic and compares the provisional diagnoses of different referring sources with the outcome of the neurophysiological tests. The study comprises data of 495 patients which were examined over a two years period. General practitioners, the author, physiatrists and surgeons, followed by occupational health physicians and neurologists used the electrodiagnostic service. Two thirds of the cases were referred to rule out upper limb mononeuropathies or cervical root and plexus neuropathies. Carpal tunnel syn

Peripheral nerve blocks in the cervical region: from anatomy to ultrasound-guided techniques.

Cervical plexus nerve blocks have been employed in various head and neck operations. Both adequate anaesthesia and analgesia are attained in clinical practice. Nowadays, ultrasound imaging in regional anaesthesia is driven towards a certain objective that dictates high accuracy and safety during the implementation of peripheral nerve blocks. In the cervical region, ultrasound-guided nerve blocks have routinely been conducted only for the past few years and thus only a small number of publications pervade the current literature. Moreover, the sonoanatomy of the neck, the foundation stone of int…

Thoracic outlet syndrome with right subclavian artery dilatation in a child - transaxillary resection of the pediatric cervical rib.

Cervical rib is a well-documented congenital anomaly, and thoracic outlet syndrome (TOS) with vascular compression is extremely rare in childhood. The subclavian artery, subclavian vein, brachial plexus, or a combination of these can be affected in this syndrome. We present a case of TOS in an eight-year-old boy and describe the rare finding of subclavian artery compression with post-stenotic dilatation and severe brachial plexus compression symptoms. The patient underwent right transaxillary resection of the first thoracic and cervical rib. The presentation is unique and may be elusive.

Cervical myelopathy due to chronic overshunting in a pediatric patient: case report and review of the literature.

We present a rare cause of cervical myelopathy produced by an engorged suboccipital epidural venous plexus due to chronic cerebrospinal fluid (CSF) overdrainage. A 17-year-old boy with obstructive hydrocephalus due to a retrocerebellar cyst and secondary implantation of a ventricloperitoneal shunt (VP-shunt) presented with progressive spastic tetraparesis. MRI imaging revealed myelopathy due to significant compression of the cervical spinal cord by engorged epidural veins. Further assessment at a low-pressure setting revealed a broken shunt valve. The VP-shunt valve was changed with an additio

Neuralgic amyotrophy. An update.

A century after the first description of neuralgic amyotrophy (NA), its pathophysiology remains unknown. An inflammatory (auto)immune pathophysiology is presumed, with mechanical or infectious precipitating conditions, which triggers attacks. Clinically, NA is an acute and painful unique or multiple mononeuropathy that causes palsy, amyotrophy and sensory loss in an asymmetric and patchy distribution. It involves the upper brachial plexus rather than the other parts but also may involve the cervical plexus, lumbosacral plexus and cranial nerves. The impairment can be restricted to one fascicul…

Is Pseudomeningocele an Absolute Sign of Root Avulsion Brachial Plexus Injury?

BACKGROUND: The finding of pseudomeningocele from cervical myelogram is widely accepted as a pathognomonic sign for diagnosing root avulsion in brachial plexus injury. In general, motor power in this setting should be absent. However, in clinical practice, we observed that motor power in some patients was still preserved. The objective of this study is to evaluate the accuracy of pseudomeningocele from cervical myelogram for predicting root avulsion in brachial plexus injury. METHODS: We retrospectively reviewed 201 patients with brachial plexus injury from 2007-2011. Four patients were exclu

Interscalenic approach to the cervico-brachial plexus.

The concept of a closed peri-neurovascular space surrounding the cervicobrachial plexus, introduced by A. Winnie, allows the blockade of the cervical and brachial plexuses by means of a single puncture technique. The single puncture has positive advantages: 1. The rapidity of the blockade; 2. The simplicity of the blockade; 3. Comfort for the patient. The landmarks are easy to make. As with epidural blockade, the injection level and the volume of local anesthetic determine the quality and extent of the block. The traditional indication is surgery of the shoulder and of the supraclavicular area

A rare anomaly of the cervicobrachial plexus: penetration of the anterior scalene muscle tendon by the T1 nerve root.

The cervicobrachial plexus is known to exhibit anatomical variation, particularly the brachial plexus, which maintains a close anatomical relationship with the subclavian artery and the scalene muscles. Knowledge of these structural variations is critical for clinicians involved in the diagnosis and management of brachial plexus injuries. This case report describes a rare case involving multiple anomalies in the root formation of the brachial plexus, observed in a Japanese male cadaver. In this case, both brachial plexuses were formed from C4 to T1, and the both subclavian arteries passed betw

Does Interventional Pain Management Play a Role in the Treatment of Cervical Post-Surgical Neuropathic Pain?

Post-surgical neuropathic pain is still an underdiagnosed medical condition with persistent pain occurring in 10-50% of patients submitted to surgery. We present a case of a 52-year-old patient with persistent paracervical, supraclavicular, and upper chest neuropathic pain after the excision of a massive deep right cervical tumor, concluded to be an accessory spinal nerve schwannoma. A thorough physical and ultrasound examination helped conclude injury of the superficial cervical plexus. Therefore, an ultrasound-guided hydrodissection of several neuromas was performed at the level of the super…

[Pancoast's syndrome secondary to neck metastasis with normal radiography of the thorax].

Pancoast's syndrome is characterized by pain in the shoulders and upper extremities, Horner's syndrome, bone loss and hand muscle atrophy. Bronchogenic carcinoma is the most common cause, although other neoplasms or lung infection are occasionally responsible. An apical mass on the chest film can be seen in over 90% of cases, although apical pleural thickening is sometimes the only radiographic finding. We describe a patient whose clinical picture was highly suggestive of Pancoast's syndrome but whose chest film was normal. Magnetic resonance imaging disclosed a cervical mass adjacent to the b

High resolution neurography of the brachial plexus by 3 Tesla magnetic resonance imaging.

The study of the structures that make up the brachial plexus has benefited particularly from the high resolution images provided by 3T magnetic resonance scanners. The brachial plexus can have mononeuropathies or polyneuropathies. The mononeuropathies include traumatic injuries and trapping, such as occurs in thoracic outlet syndrome due to cervical ribs, prominent transverse apophyses, or tumors. The polyneuropathies include inflammatory processes, in particular chronic inflammatory demyelinating polyneuropathy, Parsonage-Turner syndrome, granulomatous diseases, and radiation neuropathy.
