Child sex abuse cases need more research.

Health professionals regard child sex abuse as worse than physical abuse even though the mortality in physical abuse is far higher, according Harry Zeitlin, consultant psychiatrist at Westminster Children's Hospital.

Child abuse, cardiopulmonary resuscitation, and rib fractures.

Rib fractures have occasionally been described in children receiving cardiopulmonary resuscitation (CPR). Because child abuse is sometimes suspected in these cases, it is both medically and legally important to establish whether the rib fractures are secondary to abuse or CPR. One hundred thirteen children, including 41 victims of child abuse, 50 patients who had CPR, and 22 patients who had rib fractures, were studied. Twenty-nine patients had rib fractures; 14/29 (48%) were abusive. Other causes of fracture were: motor vehicle accidents (four), rickets/osteoporosis (five), surgery (five), an…

Understanding the Narratives of Child Sexual Abuse.

This qualitative study consisted of in-depth, retrospective interviews with participants over the age of 18 who experienced child sexual abuse. Through narrative analysis, study findings shed light on three overall findings: the perception of memories, the perception of grooming, and the perception of the lived lives of child sexual abuse survivors. Findings suggest that the narratives elicited from the participants included current views but also past experiences and anticipation about the future, including individual and societal levels of meaning. These narratives are furthermore entangled

Child Sexual Abuse and the Moralization of Purity.

Recent studies were able to associate disgust with the moral domain of purity, as well as a heightened sensitivity to disgust with sexual victimization. However, no empirical evidence has yet to document the exact relation between sexual victimization and its impact on the moralization of purity. Therefore, the present study aimed to examine the relationship between child sexual abuse (CSA) and the moral domain of purity, by means of judgments toward three different types of disgust: pathogen, sexual and moral. To test this, The Three Domains of Disgust Scale (TDDS) was given to both CSA parti

Mantle of forensics in child sexual abuse.

Child sexual abuse (CSA) is defined as inappropriate adolescent or adult sexual behavior or contact with a child. Sexual abuse may be committed by any person including those under the age of 18 years when that person is either significantly older than the victim or is in a position of power or control over the child. Detecting CSA requires a high incidence of suspicion and familiarity with physical, behavioral, and verbal indications of abuse. Shame and guilt often may have discussion difficult. Studies have shown that approximately 60% of abused children have injuries to head, face, and mouth

The Eye Examination in the Evaluation of Child Abuse.

Child abuse can cause injury to any part of the eye. The most common manifestations are retinal hemorrhages (RHs) in infants and young children with abusive head trauma (AHT). Although RHs are an important indicator of possible AHT, they are also found in other conditions. Distinguishing the number, type, location, and pattern of RHs is important in evaluating a differential diagnosis. Eye trauma can be seen in cases of physical abuse or AHT and may prompt referral for ophthalmologic assessment. Physicians have a responsibility to consider abuse in the differential diagnosis of pediatric eye t…

Child abuse and physical health in adulthood.

BACKGROUND: A large literature exists on the association between child abuse and mental health, but less is known about associations with physical health. The study objective was to determine if several types of child abuse were related to an increased likelihood of negative physical health outcomes in a nationally representative sample of Canadian adults. DATA AND METHODS: Data are from the 2012 Canadian Community Health Survey-Mental Health (n = 23,395). The study sample was representative of the Canadian population aged 18 or older. Child physical abuse, sexual abuse, and exposure to intim…

Child abuse in the irritable bowel syndrome.

Stress is considered a risk factor for the irritable bowel syndrome (IBS). One of the main stress sources is represented by the negative life events and trauma suffered in childhood. Several papers have endorsed the hypothesis that youth submitted to stress are prone to develop IBS. We have undertaken a review of the literature, searching all Pubmed papers pertinent to child abuse and IBS. The data suggest that indeed, children submitted to physical, psychological and sexual abuse are at risk to develop IBS as adults. However, cultural diffferences exist.

An epidemiological overview of child sexual abuse.

Child sexual abuse (CSA) is a universal problem with grave life-long outcomes. The estimates vary widely depending on the country under study, the definitions used, the type of CSA studied, the extent of coverage, and quality of data. This study intended to assess the magnitude and the issues related to CSA. We searched databases such as PubMed, Google scholar, web (newspaper reports), and government websites. The relevant data was extracted from these sources for gathering evidence on CSA and secondary data analysis was done. The prevalence of CSA was found to be high in India as well as thro

[Perforation of the eardrum caused by child abuse].

Munchausen syndrome by proxy (MSBP) is a complex, multifaceted form of child abuse that presents a challenge to all health-care providers. Diagnosis is difficult in cases of MSBP, and it is imperative to recognise the clinical features of MSBP in both the child and the perpetrator. All members of the health-care team must stay objective in the interactions with families where MSBP is suspected and make referrals to the proper agencies. A case is presented to explain the medical and social complexities associated with nursing and diagnosing MSBP. This article also provides a brief review of the…

The neurobiological toll of child abuse and neglect.

Exposure to interpersonal violence or abuse affects the physical and emotional well-being of affected individuals. In particular, exposure to trauma during development increases the risk of psychiatric and other medical disorders beyond the risks associated with adult violence exposure. Alterations in the hypothalamic-pituitary-adrenal (HPA) axis, a major mediating pathway of the stress response, contribute to the long-standing effects of early life trauma. Although early life trauma elevates the risk of psychiatric and medical disease, not all exposed individuals demonstrate altered HPA axis

Bruises in children: normal or child abuse?

Bruises in active children are common and often are considered "normal" childhood injuries. However, bruises also may be the result of physical abuse or other conditions. The evaluation of a child with bruising should include a thorough history and physical examination to determine the cause. When abuse is suspected, prompt reporting is necessary to protect the child from further injury. This article provides information intended to assist nurse practitioners and other pediatric health care providers in their decision making when assessing a child with bruises.

Child abuse in Kuwait: problems in management.

OBJECTIVES: To explore the extent of child abuse in Kuwait and examine the difficulties associated with its management. SUBJECTS AND METHODS: A retrospective analysis of the 60,640 medical records of children admitted to Al-Amiri and Mubarak Al-Kabeer Hospitals, Kuwait, between 1991 and 1998 was done. RESULTS: Of the 60,640 records, 16 children showed evidence of abuse. Of these, the perpetrator was a parent in 75% of the cases, which involved the following abuses: physical, 13; sexual, 2; and Munchausen syndrome by proxy in 1. Children with physical abuse had more than one type that include…

[Child abuse as a problem of the family physician].

There are four major forms of child abuse: physical abuse, emotional abuse, sexual abuse and neglect. Various combination of these four aspects often exists in the individual child and clinical manifestation can not be so clear-cut. In this paper the authors present some problems of child abuse syndrome (taking into consideration risk factors, clinical symptoms, recognition and methods of treatment) whose knowledge may be helpful for the family doctor who comes into contact with such children.

Current and emerging concepts in child abuse.

Within the three decades since Henry Kempe first popularized the concept of "the battered child syndrome," much has been learned not only about physical abuse but also neglect and sexual abuse. More recently, physicians with an interest in child abuse have expanded their focus to include child death review and other prevention strategies. Despite the increasing specialization of child abuse evaluation and management, all physicians working with children are likely to encounter child abuse in their practice. In 1994, more than 3.1 million reports of child abuse were made, of which more than 1 m…

Fractures of the first rib in child abuse.

PURPOSE: To evaluate the incidence and specificity of fracture of the first rib as an indicator of child abuse and to determine a mechanism of fracture. MATERIALS AND METHODS: All infants and young children with rib fractures who were seen at the radiology department of an academic medical center over a 2-year period were identified from the radiology data base. RESULTS: Rib fractures were attributed to child abuse in 12 of 35 children identified. Apart from a neonate with congenital osteogenesis imperfecta, only three children, all abused, had first-rib fracture, one bilaterally. Two additi…

Orofacial manifestations of child abuse and neglect.

About one-half of all child abuse cases involve some form of orofacial injury. Common signs of physical abuse include fractures of the teeth or the maxilla, mandible and other facial bones, facial burns, lacerations of the lips and lingual frenum, and bite marks on the face and neck. Sexual abuse should be suspected if erythematous, ulcerative, vesiculopustular, pseudomembranous and condylomatous lesions are present on the lips, tongue, palate, face or pharynx. Dental neglect, a common form of child maltreatment, should be suspected if rampant caries and oral infection, bleeding and trauma per…

Child abuse intervention in the emergency room.

The pediatrician and other emergency room caregivers are often on the frontlines in the identification and treatment of child abuse and neglect. The ability to recognize the possibility of intentional injury is crucial because immediate intervention may be necessary to prevent further harm to the child. The evaluation requires a careful taking of history, thorough physical examination, and detailed documentation. Reporting of suspected abuse or neglect is mandated and must be made to appropriate authorities. Follow-up of medical treatment, services, and available therapy should be emphasized t…
