Submucous cleft palate is a structural abnormality of the palate. The clinical features vary, as does the functional impairment, which ranges from none to severe. Although there is a poor correlation between the presenting clinical signs and the severity of velopharyngeal insufficiency during speech, a clinical grading system could assist surgical management and allow more meaningful comparisons to be made between outcome studies. The grading system described is based on clinical examination alone and reflects the likely degree of structural abnormality of the musculature of the soft palate.

The reconstructions of the asymmetrical soft palate cleft is a surgical challenge when it comes to achieving symmetry and optimal soft palate muscular function. Three different versions of the intravelar veloplasty have been used: the intravelar veloplasty (1969) (type I), the modification according to anatomical defects (1991) (type II), and the modification using part of Sommerlad's technique and part of Ivanov's technique (2008) (type III). The perioperative outcomes of the type II and type III intravelar veloplasty were assessed and compared in asymmetrical cleft cases. Two hundred and sev

This study aimed to investigate the age at diagnosis, palatal characteristics, and symptoms of submucous cleft palate (SMCP) and to compare the speech outcomes between 3 operative techniques for primary repair of SMCP.It was a retrospective review of 92 patients diagnosed with SMCP between 1994 and 2008, where patients were treated with 1 of 3 surgical procedures: double opposing z-palatoplasty, radical intravelar veloplasty, or pharyngeal flap. The need for a second procedure was considered a primary outcome measure, with operative failure defined by the need for a secondary operation. Postope…

Muencke syndrome results from mutations in the FGFR3 gene, and although it is well recognized that the clinical presentation is variable, the important key finding includes coronal synostosis. We present a family where a mother with proven FGFR3 Pro250Arg mutation gave birth to identical twins both of whom had craniosynostosis but had coexisting bilateral cleft lip and palate. We believe that this is the first description of clefting occurring in conjunction with Muencke syndrome and so further extends the range of phenotypic variation that can occur in this syndrome.

We report two cases of massive tongue edema in routine palatoplasty. All patients had uneventful recovery. We postulated that the macroglossia was secondary to ischemia and venous congestion after prolonged use of Killner Dott mouth gag with slotted tongue blade exaggerated by hyperextension of neck and Trendelenberg position.

Surgical cleft repair aims to restore function of the oro-nasal sphincter and oro-nasal soft tissues and re-establish the complex relationship between perioral and perinasal muscle rings without compromising subsequent mid-facial growth and development. Here we review the surgical anatomy of this region, optimal timing for surgical repair and current thinking on the use of surgical adjuncts. In addition, an overview of current surgical techniques available for the repair of cleft lip, cleft palate and velopharyngeal insufficiency is presented. Finally, we briefly discuss nasal revision surgery…

OBJECTIVES: To investigate age of diagnosis, typical symptoms, finding of the palate, therapy options and accompanying diseases.
METHODS: A retrospective analysis of 439 patients with symptomatic submucous cleft palate (SMCP), who received a veloplasty operation (butterfly suture technique developed by Haase) was made.
RESULTS: SMCP was initially diagnosed at the mean age of 4.9 years. Main symptoms were hyper nasal speech (51%) and conductive hearing loss (45%), which resolved after veloplasty (often in combination with adenotomy and insertion of ventilation tubes). Typical findings of the …

LEARNING OBJECTIVES: After reading this article, the participant should be able to: 1. Understand the embryology of normal palate development, and the genetic and environmental causes of clefting. 2. Delineate the anatomy of the normal palate and the cleft palate. 3. Understand the Veau classification for clefts of the palate and techniques used to repair various clefts of the palate. 4. Understand the factors that contribute to complications following palate repair, including fistula formation, velopharyngeal insufficiency, maxillary growth inhibition, and sleep apnea.

Gastroschisis is a common congenital abdominal wall defect. Rarely, it is associated with extraabdominal midline congenital anomalies. Oral clefts are the commonest craniofacial congenital midline defects. They can be associated with other midline defects like omphalocele. We believe these are the first 3 case reports of gastroschisis and cleft palate occurring in the same patient.

10 cleft lip and palate temporary facilities were set up 1994-2003 in a Christian Hospital in Central India. 725 patients were operated upon. An analysis of the operations in regard to lip closure, closure of the soft and the hard palate, secondary cleft operations and cleft-related surgery is given. Differing therapy concepts in developed countries are discussed and compared to experiences of other third world volunteer medical groups.

OBJECTIVE: To assess the efficacy of decellularized dermal grafting used as an adjunct to the performance of primary repair of wide cleft palates.
DESIGN: Retrospective review.
SETTING: Tertiary referral center for large managed care organization.
METHODS: Seven consecutive patients with clefts of the hard and soft palates wider than 15 mm as measured at the posterior edge of the hard palate. Palates were repaired in the standard 2-flap approach with intravelar veloplasty. The decellularized dermal graft (AlloDerm) was applied immediately deep to the oral mucosal closure. Patients were foll…

OBJECTIVE: To gain a clear idea of the function of middle ears and hearing loss in cleft palate.
METHODS: 59 cleft palate cases, including 117 ears are investigated by tympanogram and hearing threshold.
RESULTS: In 80.4% of patients the function of middle ears are abnormal. 59.4% ears are conductive hearing loss in various degrees.
CONCLUSION: Function of middle ears in cleft palate should be regularly examined as early as possible. If it is secretory otitis media, the tympanostomy pressure equalization tubes should be inserted to compensate inadequate function of the eustachian tube so as …

Children with cleft palate often suffer from hearing and speech and language problems. Detailed knowledge about the long-term outcome of various therapeutic strategies including velopharyngoplasty or speech therapy seems mandatory to design an efficient rehabilitation plan for affected children. In this follow-up study 417 children with cleft palate (excluding isolated cleft lip) were examined in an interdisciplinary approach by maxillofacial surgeons, orthodontists, otolaryngologists, audiologists and speech and language pathologists. We found that 92% of the children had speech or language d…

This article is an overview of the treatment possibilities and the role of the orthodontist in the management of patients with cleft palate. Orthodontic intervention and treatment may be indicated soon after birth (infant orthopedics), in the deciduous, mixed, and permanent dentitions and also in the adult patient. This article focuses primarily on decision-making and procedural steps performed by the orthodontist during the permanent dentition stage. The need for coordinated interaction with other team members is emphasized.
