Multifocal granulocytic sarcoma of the spine mimicking neurofibromatosis Type 2: case report.

In this report the authors report on a patient with a very indolent course of granulocytic sarcoma, characterized by steroid-induced remission of spinal and cranial tumors and recurrence over a period of several years. This 24-year-old man with history of leukemia presented with rapid-onset quadriparesis secondary to multiple extraaxial masses of the cervicothoracic spine, from C-5 to T-3, and lumbosacral spine, from L-5 to the coccyx. Although the imaging features were highly suggestive of neurofibromatosis Type 2, the patient's history and clinical course were consistent with granulocytic sa…

[A case of adenocarcinoma of the anal canal in which palliation was achieved by chemoradiotherapy].

An 82-year-old man who complained of constipation, anal pain, and bleeding was diagnosed with internal hemorrhoid in another hospital 6 months previously. At the time of our consultation, a tumor was observed in the anal canal. Computed tomography (CT) imaging revealed that the tumor extended to the front of the coccyx with abscess formation. Histological examination of biopsy specimens confirmed a diagnosis of adenocarcinoma. Since the patient refused to undergo surgery, we recommended chemoradiotherapy for relieving pain. A total of 60 Gy of irradiation was administered to the lesser pelvis,…

Morphology of pilonidal sinus disease: some evidence of its being a unilocalized type of hidradenitis suppurativa.

BACKGROUND: The term 'pilonidal sinus' (PS) reflects an acute exacerbating, purulent, fistulating chronic inflammation located in the coccyx region. Systematic histological investigations are scarce, and the etiology has remained controversial. METHODS: Histological and immunohistochemical characterization of totally excised material of 27 patients (68 specimens) with PS (no antecedent and no current clinical signs of hidradenitis suppurativa, HS) and its correlation with data on HS which we published earlier. RESULTS: Follicular hyperplasia/hyperkeratosis and interfollicular epidermal hyper…

Incoherent imaging using continuous wave ultrasound. A preliminary study using bovine intervertebral discs.

As an object rotates with respect to a stationary planar ultrasonic beam, the scattering centres within the object return echoes that are Doppler-shifted in frequency by amounts depending on the velocities of the individual scatterers. The backscattered echo amplitude at any particular frequency is the line integral of the scattered radiation at the cross-range corresponding to that frequency. The amplitude as a function of frequency can be interpreted as a tomographic projection. A tomographic reconstruction algorithm can produce an image of the distribution of scattering centres in the inson

Congenital anophthalmia with caudal vertebral anomalies in Japanese Brown cattle.

Anophthalmia and caudal vertebral anomalies such as taillessness or wry tail were morphologically examined in ten Japanese Brown calves obtained in Kumamoto Prefecture. The anomalous calves lacked eyeball bilaterally or unilaterally but had small-sized eyelids and narrow palpebral fissures. A small cystic, solid or spot-like remnant of eyeball (REB) was buried in the mixture of vestigial extraocular muscles, lacrimal gland and adipose tissue of the orbit. The REB was composed of irregularly arranged elements of ocular wall such as sclera, choroid and retina. The retina was often dysplastic and

Diagnosis and management of sacrococcygeal chordoma.

Chordoma is a rare, slow-growing but locally aggressive malignant tumor derived from the primitive notochord and located along the axial skeleton. Between 1973 and 1991, of 15 patients with chordomas treated at the Medical College of Virginia, eight originated in the sacrococcygeal area. There was a median one year interval between the onset of symptoms and diagnosis (range of four months to six years) for this latter group. Two patients had undergone coccygectomies and one patient a lumbar discectomy prior to establishing the correct diagnosis of sacral chordoma. Seven patients underwent rese

Sacrococcigeal and vertebral chordomas. Report of three cases and review of the literature.

Three cases of sacrococcygeal and vertebral chordoma are described. Histogenetic and anatomopathological aspects with particular reference to differential diagnosis from similarly distributed neoplasias are discussed. Anti-cytoskeleton monoclonal antibodies were used to this purpose. The clinical profile of sacrococcygeal and vertebral chordomas is characterized by a pronounced metastatic potential; radiation treatment can only partially counter their biological behaviour, while chemotherapy has proved little or no effectiveness. Where possible, radical surgery is currently the only treatment

Magnetic resonance imaging features of sacrotuberous ligament partial injury with normal hamstring tendons.

The sacrotuberous ligament has an oblique course, from a broad fan-like origin in the posterior superior iliac spine, transverse tubercles of the sacrum and the upper part of the coccyx, descending laterally to attach to the medial surface of the ischial tuberosity. It provides stability to the pelvic girdle, including the sacroiliac joint, counterbalancing its rotation and limiting its movement. Magnetic resonance imaging (MRI) can identify this ligament, depicting its origin, course and continuity with the conjoined biceps femoris and semitendinosus tendon. However, there is no previous repo…

Chronic Primary Pelvic Pain Syndromes in Women: A Comprehensive Review.

Chronic pelvic pain (CPP) in women is a multifactorial and complex condition. It often remains undiagnosed or inadequately treated. Despite its high prevalence, CPP continues to be a taboo subject, leading to delays in seeking medical care. Chronic primary pelvic pain syndromes (CPPPS) are pain conditions without an obvious underlying diagnosis, including painful bladder syndrome, vulvodynia, genito-pelvic pain/penetration disorder, levator ani syndrome, proctalgia fugax, myofascial syndrome, pudendal neuralgia, and coccyx pain syndrome. A comprehensive review of the literature was conducted t…

Late occurrence of malignancy following resection of a histologically mature sacrococcygeal teratoma. Report of a case and literature review.

A sacrococcygeal adenocarcinoma is reported in a male patient nearly 40 years old following resection of a histologically mature sacrococcygeal teratoma. The adenocarcinoma arose within soft tissue of the presacral area and residual coccyx and extensively invaded the coccygeal stump that had not been removed in toto with the teratoma during initial surgery at 2 months of age. The patient died nearly 2 years later with local and regional recurrence of tumor and metastases to lymph nodes, liver, lung, bone, and brain. At autopsy there was no evidence of origin from deep internal organs such as t…

[Posterior approach to the rectum. Technic, indications, complications. Apropos of 41 cases].

Certain lesions in the middle part of the rectal ampulla can only be approached with safety through the posterior surface of the rectum after resection of the coccyx and the last part of the sacrum. This approach was employed in 41 patients, with conservation of the sphincter in all cases. Operation was performed 39 times for excision of a large villous tumor, in 1 case for a polycystic lesion, and in 1 case for a localized epithelioma. Extensive tumors in 3 cases required rectal resection with end-to-end anastomosis, employing a circular mechanical suturing apparatus in 1 patient. Two patient…

[Evaluation of the anal position in newborns and children with chronic constipation. Incidence of anterior ectopic anus].

INTRODUCTION: Anterior ectopic anus (AEA), continues to be a controversial issue. The objective is to determine the normal position of the anus or anal ano-genital index (AGI) in newborns (NB) and in a group of infants with chronic constipation, know the incidence of AEA in both groups, and its relationship with constipation. METHODS: A prospective aleatory study is performed in where the AGI is determined in 529 NB and 64 older patients with chronic constipation. The AGI is obtained by dividing the distances (cm): vulvar fornix-anus/fornix-coccyx in females and scrotum-anus/scrotum-coccyx in…

Treatment of chronic refractory coccydynia with peripheral nerve field stimulation: a novel case.

Coccydynia or coccyx pain is a rare form of low back pain located below the sacrum and above the anus. The causes of this condition vary, including previous trauma, underlying conditions (such as infection or neoplasm), and idiopathic causes. Our purpose was to describe the successful treatment of chronic refractory coccydynia with the application of sacral area neuromodulation. A 54-year-old female patient had a 9-year history of chronic drug-resistant coccydynia associated with low back pain and right-sided sciatica. Her clinical examination revealed bilateral pain on palpation of the sacroi…
