[Speech therapy for cognitive disorders in elderly patients].

The aim of providing speech therapy to elderly patients, in the framework of a personalised approach, is to help them maintain their autonomy and delay their move to a specialised hospital. The family and caregivers play an essential role in ensuring the success of this therapy.

Cerebral fat embolism diagnosed by cognitive disorder.

Fat embolism syndrome is a rarely seen complication of skeletal trauma, and it is seen at a rate of 2-5% after fractures of the long bones of the lower extremities. Its classic triad-+consists of hypoxemia, petechial bleedings on the skin and neurological findings. These neurological findings are highly variable and non-specific, and they can present with lethargy, irritability, delirium, stupor, convulsion or coma. In this report, a male case is presented who was diagnosed with cerebral embolism due to acute cognitive disorder after a segmental tibial fracture.

A neurobiological approach to the cognitive deficits of psychiatric disorders.

Deficits in brain networks that support cognitive regulatory functions are prevalent in many psychiatric disorders. Findings across neuropsychology and neuroimaging point to broad-based impairments that cross traditional diagnostic boundaries. These dysfunctions are largely separate from the classical symptoms of the disorders, and manifest in regulatory problems in both traditional cognitive and emotional domains. As such, they relate to the capacity of patients to engage effectively in their daily lives and activity, often persist even in the face of symptomatically effective treatment, and

Cognitive emotion regulation in euthymic bipolar disorder.

BACKGROUND: Based on findings indicating increased stress reactivity and prolonged stress recovery in individuals with bipolar disorder (BD), it has been proposed that emotion regulation (ER) deficits lie at the core of this disorder. Recent studies show an increased use of maladaptive ER strategies and a decreased use of adaptive ER strategies in BD. Whether this pattern is merely a correlate of affective episodes or might be a stable characteristic of BD, however, remains to be explored. In addition, it is unclear whether these deficits in ER are specific to people with a history of BD. MET

Insight in eating disorders: clinical and cognitive correlates.

BACKGROUND: The aim of this study was to explore the extent of lack of insight and its components in eating disorders (EDs) and to investigate the relationship between insight and clinical and cognitive characteristics in this group. METHOD: Seventy-five participants were enrolled in the study: 25 with anorexia nervosa (AN), 15 with bulimia nervosa (BN) and 35 healthy controls (HC). Insight was assessed with a modified version of the Schedule for the Assessment of Insight for EDs (SAI-ED) and multi-dimensional scaling (MDS) analysis was used to clarify the internal structure of the scale. Cognitive flexibility and clinical severity in eating disorders.

OBJECTIVES: The aim of this study was to explore cognitive flexibility in a large dataset of people with Eating Disorders and Healthy Controls (HC) and to see how patient characteristics (body mass index [BMI] and length of illness) are related to this thinking style. METHODS: A dataset was constructed from our previous studies using a conceptual shift test--the Brixton Spatial Anticipation Test. 601 participants were included, 215 patients with Anorexia Nervosa (AN) (96 inpatients; 119 outpatients), 69 patients with Bulimia Nervosa (BN), 29 Eating Disorder Not Otherwise Specified (EDNOS), 72

Cognitive-behavioral treatment of panic disorder in adolescence.

This investigation represents the first randomized controlled trial to evaluate the feasibility and efficacy of Panic Control Treatment for Adolescents (PCT-A). Thirteen adolescents, ages 14 to 17, were randomized to 11 weekly sessions of PCT-A treatment, whereas 13 were randomized to a self-monitoring control group. Results indicate that adolescents receiving immediate PCT-A showed a significant reduction in clinician-rated severity of panic disorder and in self-reported anxiety, anxiety sensitivity, and depression, in comparison to control group participants. These treatment gains were maint

[Cognitive function in eating disorders and therapeutic approach].

We examined brain activation in the presence of eating disorders while processing unpleasant words concerning body images using functional magnetic resonance imaging (fMRI). Anorexia nervosa restricting type (AN-R) and anorexia nervosa binge-eating/ purging type (AN-BP) patients showed significant amygdala activation. Farther, AN-BP and bulimia nervosa (BN) patients showed significant medial prefrontal cortex activation. Next, we conducted short-term integrated group therapy for eating disorders. After treatment, depressive scores of POMS and emotional-oriented coping scores of CISS were signi

Vascular cognitive disorder. A biological and clinical overview.

Although vascular dementia (VaD) represents the second most common cause of dementia after Alzheimer's disease (AD) in the elderly, and is referred as the "silent epidemic of the twenty-first century", there is still a controversy on terminology, classification and diagnostic criteria of VaD. The diagnosis of VaD resides in clinical criteria determining a cognitive impairment, the presence of cerebrovascular disease and, only in the case of post-stroke dementia or multi-infarct dementia, a temporal relationship between these. The search for a reliable biochemical tests helping in the diagnosis

[Interpreting cognitive impairment in mood disorders in the elderly].

In the elderly, cognitive impairment is often associated with depression. Through two case reports, we would like to underline that cognitive impairment should not be reduced immediately to an organic disorder but should be interpreted considering its potential meaning in the psychic life of the patient. The case of Mme R. show that a psychic trauma can lead to cognitive dysfunction by regression mechanisms, and the story of Mme S. remind us that cognitive capacity can fluctuate according to intrapsychic conflicts even in the course of a neurodegenerative disease. Cognitive impairment in depre

Cognitive behavior therapy in the treatment of panic disorder.

BACKGROUND: Comprehensive cognitive behavior therapies have been proved to be more effective than behavioral interventions. However, the efficacy of CBT is not studied in the Indian context and also, the amount of change brought about by CBT is not known. AIMS: This study aims to examine the efficacy of cognitive behavioral intervention (CBI) in the treatment of panic disorder. Our specific objectives were to assess the effectiveness of CBI in reducing symptom severity as well as cognitions related to panic and panic-related behaviors. DESIGN: The study adopted a two-group comparison with pr

[Cognitive 'Theory of Mind' processing in bipolar disorder].

OBJECTIVE: Bipolar disorder is associated to persistent functional impairments. Although many researches have shown that bipolar individuals might have cognitive deficits, a small number of studies evaluated the role of problems of social cognitive Theory of Mind processing (regarding the capacity to infer mental states) in the emergence of bipolar disorder's symptoms and its possible social poor outcomes. The objective of the present manuscript is to review systematically and critically the literature on Theory of Mind processing in bipolar disorder. METHOD: A search in the electronic databa

Alexithymia and cognitive dysfunctions in patients with panic disorder.

BACKGROUND: In patients with panic disorder (PD), the difficulty to identify and manage emotional experience might contribute to the enduring vulnerability to panic attacks. Such a difficulty might reflect a dysfunction of fronto-temporo-limbic circuits. The present study was designed to test the hypothesis that drug-free patients with PD, as compared with healthy subjects (HS), show a higher prevalence of alexithymia, greater difficulty in emotional stimuli processing and poorer performance on neuropsychological tests exploring the activity of fronto-temporo-limbic circuits. METHODS: Alexith

[Cognitive deficits in schizophrenia and other psychotic disorders].

Cognitive deficits are vulnerability indicators for psychotic disorders, particularly schizophrenia. The deficits remain stable throughout the illness in schizophrenia, but fluctuate with clinical state in other psychotic disorders. They are limiting factors for patients' prognoses and there is considerable incentive to develop treatments that can improve these deficits. The current brief review summarizes the relevance of cognitive deficits for the pathogenesis and prognosis of psychotic disorders, and identifies pertinent issues within the research field.

[Cognitive-behavioral treatment for impulse control disorders].

OBJECTIVES: This paper reviews the cognitive-behavioral treatment of kleptomania, compulsive buying, and pathological gambling. METHOD: A review of the published literature was conducted. RESULTS: Treatment research in all of these areas is limited. The cognitive-behavioral techniques used in the treatment of kleptomania encompass covert sensitization, imaginal desensitization, systematic desensitization, aversion therapy, relaxation training, and alternative sources of satisfaction. Regarding compulsive buying, no empirical support for treatment exists but common techniques examined were co

[Questioning the ability to drive in patients with cognitive disorders].

The relatives of an 81-year-old man questioned whether he was fit to drive, but he refused to discuss the matter. His son sent a written notice to the Dutch Driving License Centre (CBR) and the physicians were about to do so when the man fell and spent one night lying on the floor in his house. The man was hospitalised and his son took his car keys away. The approach to assessing the ability to drive in patients with cognitive disorders and dementia is a difficult matter, especially when a patient is unaware of his or her illness. At this time, licensed drivers with cognitive disorders are not

The enigma of vascular cognitive disorder and vascular dementia.

The prevalence, morphology and pathogenesis of vascular dementia (VaD), recently termed vascular cognitive impairment, are a matter of discussion, and currently used clinical diagnostic criteria show moderate sensitivity (average 50%) and variable specificity (range 64-98%). In Western clinic-based series, VaD is suggested in 8-10% of cognitively impaired aged subjects. Its prevalence in autopsy series varies from 0.03 to 58%, with reasonable values of 8-15%, while in Japan it is seen in 22-35%. Neuropathologic changes associated with cognitive impairment include multifocal and/or diffuse dise

[Cognitive disorders in Parkinson's disease without dementia].

The cognitive disorders observed in non-demented Parkinsonian patients are frequent but subtle. They mostly result from difficulties to control attentional resources. These deficits particularly disturb the strategies involved in planning as well as in encoding and retrieval processing of memory, whereas consolidation of the mnemonic traces and instrumental functions are relatively spared. These deficits can be related to the striatal dopamine depletion (in particular in the caudate nuclei) which seems sufficient to account for the cognitive disorders that appear early in the course of the dis

[Cognitive-behavioral psychotherapy for adolescents with eating disorders].

Anorexia nervosa (AN), bulimia nervosa (BN), and binge eating disorder (BED) are grouped together under the term eating disorders. Due to its typical onset in adolescence, AN in particular represents a frequent disorder with often an unfavourable course in this age range (Steinhausen 2002). The mental, social and physical consequences are serious. Research has shown that cognitive-behavioral treatment (CBT) has good effectiveness in adult patients with AN, BN and BED and that it is superior to other treatments. However, there have been few studies on children and adolescents. The effectiveness

Cognitive-behavioral treatment for childhood sleep disorders.

Sleep problems are very prevalent during childhood and may have adverse developmental impact. The efficacy of a number of cognitive-behavioral interventions for the most prevalent problems such as difficulty falling asleep and night-wakings has been repeatedly demonstrated with relatively rapid outcomes and high success rates. Preventive interventions in infancy have shown some promise in lowering the rates of sleep problems in infants of trained parents. Cognitive-behavioral interventions have also been proposed for childhood parasomnias (sleepwalking, night terrors, nightmares, and rhythmic
