[Compression/flexion fractures of the radius base (Smith fractures)].

Out of 416 Colles-fractures we observed 18 of the Smith-type. The Smith fracture is a rare injury, it will generally occur in consequence of a fall on the supinated hand which is forced in pronation. The dislocation of the carpus and peripheral fragment is in the volar and proximal direction. It should be treated operatively, especially in intraarticular fractures. Surgical treatment of patients will result in better movability of he wrist and intact distal joint surfaces.

The importance of the posteromedial fragment and its specific pinning in fractures of the distal radius.

In fractures of the lower end of the radius, the presence of a posteromedial fragment should be recognized if present. This fragment with two articular surfaces, if not reduced, will damage the radiocarpal and inferior radioulnar articulations. The diagnosis needs to be made on 3/4 X-ray views, as the fracture is often mistaken for a Colles' fracture. The author advises reduction and fixation by a method of double pinning. By comparing two sets of statistics, the overall results are shown to be improved without more complications.

Need the thumb be immobilised in scaphoid fractures? A randomised prospective trial.

Immobilisation of the thumb is widely believed to be important in the management of fractures of the carpal scaphoid. To assess the need for this, we randomly allocated 392 fresh fractures for treatment by either a forearm gauntlet (Colles') cast, leaving the thumb free, or by a conventional 'scaphoid' plaster incorporating the thumb as far as its interphalangeal joint. In the 292 fractures which were followed for six months, the incidence of nonunion was independent of the type of cast used.

Perioperative management of distal radius fractures.

Fractures of the distal radius are the most common upper limb fracture and account for over a sixth of all fractures seen in emergency departments. Although most of these fractures are managed non-operatively, a significant number of complex injuries undergo operative management. This educational review of up to date guidelines discusses the perioperative management of distal radius fractures and provides readers with continuing professional development activities.

Treatment of distal radial fractures with antegrade intra-medullary Kirschner wires.

The treatment of unstable Colles-type distal radial fractures remains a challenge. A prospective study was conducted to evaluate the outcomes of the treatment of unstable distal radial fractures using antegrade intra-medullary K-wires. Twenty-eight Colles-type distal radial fractures were selected excluding comminuted intra-articular and Barton's fractures. The blunt tips of intra-medullary K-wires were introduced in an antegrade direction to support the subchondral bone of the distal fragment. The scoring system of Green and O'Brien modified by Cooney et al. was used for the final clinical ev

[Intra-focal pinning of fractures of the distal end of the radius 10 years later].

Ten years after its introduction, the technique of intra-focal pinning of fractures of the lower extremity of the radius is now widely used. In view of the large number of operators, it is important to define a number of technical points: insertion of the pins without damaging the nerves or tendons, the number and type of pins, conduct of the operation, clinical and radiological criteria of success, post-operative course and removal of the pins. The author also reviews his experience concerning the frequency and the type of possible complications together with methods designed to avoid them an

Simultaneous scaphoid and distal radial fractures.

Simultaneous fractures of the distal radius and scaphoid are uncommon. In a prospective 3-year study we registered 2,330 distal radial fractures and 390 scaphoid fractures, and 12 were combined. Ten of these had high energy trauma; six were styloid fractures, four Colles' fractures, one was a greenstick fracture and one Salter-Harris Type 2 epiphyseal fracture. All but one of the 12 scaphoid fractures were stable and healed without problems, and one was a trans-scaphoid, trans-styloid peri-lunate fracture-dislocation. The study supports the opinion that the distal radial fracture constitutes t…

Disk lesion of the wrist joint.

Thirty-three patients with lesions of the articular disk of the wrist joint were treated. Predictability of results is best when there is an isolated lesion without osteoarthritis or severe dislocation of the distal radioulnar joint. However, good results can be obtained in combined injuries (for example, after a Colles' fracture). Thirty of the thirty-three patients were satisfied with the results of their operation.

Late rupture of the extensor pollicis longus tendon: the case against attrition.

The authors report the case of a 23-year-old man who sustained ipsilateral volar Barton's and scaphoid fractures which were complicated by a late rupture of the extensor pollicis longus tendon. The anatomy of the injury precluded attrition as the cause of the tendon rupture and the alternative theory of ischaemic rupture is examined. Clinicians should be aware that injuries other than the Colles' fracture may be complicated by rupture of extensor pollicis longus.

[Carpal instabilities].

The ligaments of the wrist and the bony configuration--beside muscles and tendons--are responsible for stability in the wrist joint, which is impaired in certain pathological conditions: unstable fractures of the scaphoid, scapholunate dissociations, perilunate fracture-dislocations, malunited Colles' fractures, Kienböcks disease, rheumatoid arthritis and posttraumatic laxity of ligaments. The differentiation between dorsal and palmar instability is made by radiological examination and measuring of the scapholunate, capitolunate and radiolunate angles. The understanding of normal anatomy and (…

Rupture of the extensor pollicis longus tendon after wrist trauma.

In the period of 1976 to 1997 our clinic treated 33 patients after Colles fracture with ruptured extensor pollicis longus tendon. The occurrence of functional loss was observed after the trauma in 3 to 9 weeks. In 30 cases the surgical treatment of extensor indicis proprius tendon, in 2 cases a direct suture of the ruptured tendon was performed as a primary repair and in one patient a palmaris longus interposition was utilised.

Beware the sprained wrist. The incidence and diagnosis of scapholunate instability.

A consecutive series of 100 cases of wrist injury, other than those referred with a radial fracture, have been reviewed to determine the incidence of acute scapholunate instability; a "clenched fist" radiograph was used in addition to the routine scaphoid views. Of 19 patients with an increase in the scapholunate gap, five were eventually considered to have significant scapholunate instability, two in association with Colles' fractures. Injuries producing significant ligamentous damage and carpal instability may be as common as scaphoid fractures. They require special consideration in diagnosi…

Do We need to include osteoporosis in today's classification of distal radius fractures?

Although the correlation between decreased bone mineral density and increased incidence of distal radius fractures is widely acknowledged, the effect of osteoporosis on specific fracture types is seldom addressed. No classification system has been established for clinical use that has been validated for osteoporotic fractures or that includes parameters of the bone density or bone quality. So far, only one experimental study could report a clear correlation between bone properties and fracture patterns, and an additional clinical study assesses bone density in relation to displacement in Colle

[Significance of ulnar ligament injuries in typical radius fractures].

In 20 patients with COLLES' fractures, stress-films of the wrist demonstrated that without fracture of the ulnar styloid ruptures of the ulnar ligaments are possible, and lead to increased radial abduction of the carpus. This instability together with interposition of the ruptured soft tissue produces a tendency to radial subluxation of the hand, in spite of good alignment of the fragments. In conclusion, we suggest that in younger patients closed reduction should be stabilized by means of percutaneous Kirschner-wires and that the interposed soft tissue should be excised.

Determinants of incidence of osteoporotic fractures in the female Spanish population older than 50.

It is well known that the adoption of preventive measures for osteoporosis may contribute to minimizing its impact as a result of bone fractures. However, there are well-recognized risk factors involved in the onset of osteoporosis that are not possible to modify. Better knowledge of these non-modifiable factors could aid prevention in subjects at high risk of fractures. The aim of this study was to evaluate the likely association between gynecological, reproductive and family history of hip fracture with the incidence of vertebral and nonvertebral osteoporotic fractures in women older than 50

External fixation of complex hand and wrist fractures.

From 1977 to 1980 fixation with an external fixateur was performed in 33 injuries of the upper extremity: ten fractures of the hand, 22 Colles' fractures, and one osteotomy for Madelung's deformity. Application was primary in 19 patients and after another type of fixation (cast or Kirschner wire) had failed to hold reduction in 14 patients. All ten hand fractures healed in 4 to 12 weeks, and the distal radius fractures united in 6 to 10 weeks. The external fixateur for unstable fractures of the hand and wrist with bone or soft-tissue loss provides rigid skeletal stabilization, accessibility fo…

[Palmar Displacement in Distal Radius Fractures following Extension Injuries of the Wrist].

BACKGROUND: Fractures of the distal radius have been traditionally classified based on the suspected mechanism of injury. Among clinicians, the terms "Colles" and "Smith" fractures are commonly used to refer to a distal radius fracture that is either displaced to the dorsal (Colles) or palmar (Smith) side of the radius. When analysing x-rays, it is not uncommon for a Smith fracture to be detected in cases where patients describe a fall on the wrist in an extended position. Thus, the question arises how a flexion-type fracture can occur after an extension injury. So far, only little research ha

Late rupture of extensor pollicis longus after wrist arthroscopy.

The first cases of impending rupture of the extensor pollicis longus after wrist arthroscopy are reported and the etiology is compared with extensor pollicis longus ruptures after nondisplaced or minimally displaced Colles fractures. Both cases were treated with extensor indices proprius to extensor pollicis longus transfer with good clinical results.
