Brachial plexus injury as a complication after colorectal surgery.

Peripheral neuropathy associated with anesthesia is a significant source of morbidity and the second most frequent cause of professional liability in anesthetic practice. Nerve injuries are a well-recognised complication of anesthesia. Brachial plexus neuropathy is a rare syndrome with an incidence of 1.6 cases per 100,000. Unfortunately, we have a limited understanding of the relations between conventional perioperative care and the genesis of peripheral neuropathy. We describe 3 cases of postoperative brachial plexus neuropathy that occurred after abdominoperineal anterior resection for rect

[Surgery for non-colorectal and non-neuroendocrine liver metastases].

INTRODUCTION: Hepatic resection has been shown to prolong survival in selected patients with colorectal liver metastases. Due to slow tumor growth patients with neuroendocrine liver metastases tend to have a good prognosis and benefit from chemo-embolisation and symptomatic treatment. The role of surgery in treating non-neuroendocrine and non-colorectal liver metastases is discussed controversially, due to the limited knowledge on this subject. The aim of our study was, therefore, to evaluate our own experiences with hepatic surgery for non-neuroendocrine, non-colorectal liver metastases. MET

Minimally invasive surgery for colorectal cancer. Initial follow-up.

OBJECTIVE: An analysis was performed to evaluate early patterns of recurrence and survival in patients undergoing laparoscopic-assisted colectomies for primary colorectal cancer. Thirty-nine patients are available with a minimum of 24 months postoperative follow-up. SUMMARY BACKGROUND DATA: The techniques and expected surgical outcomes for patients undergoing laparoscopically assisted colectomies are slowly being defined as these procedures become more common and more widely available. One of the areas of greatest concern is the use of laparoscopic-assisted colectomy for the surgical treatmen

[Surgery for complications resulting from para-clinical colorectal examinations].

Colo-rectal perforations during barium enema are exceptional, but very serious. They give rise to a retro- or intra-peritoneal septic barioma. Despite its emergency treatment, the peritonitis remains very severe (50% mortality) and peritoneal toilet is difficult. Peri-visceral complications are also possible. Perforations during diagnostic colonoscopy are less serious, as they occur in a prepared colon. When operated immediately, the mortality is less than 20%. The greatest risk consists of overlooking the initial accident and only reviewing the patient at a late stage. In this case, the morta

[Evaluation of systemic antibiotic preventive treatment in colorectal surgery].

A comparison was made of two methods of prophylactic antibiotic therapy against the septic complications of colonic and rectal surgery: --preoperative oral antibiotics associated with peroperative systemic antibiotics; --peroperative systemic antibiotics only, continued for 24 hours after surgery. Both types of antibiotic therapy were of short duration and were designed to cover aerobic and anaerobic organisms. Two groups of 30 patients were selected at random. They were homogeneous. The septic complication rate was 10% in the oral plus systemic and 24% in the systemic group. It is felt that t

Prophylactic administration of low-dose heparin in colorectal surgery.

A retrospective study of 192 patients operated on by three members of the Edward Wilson Colon and Rectum Unit, Sydney Hospital, was carried out. All operations performed involved dissection within the pelvis. Prophylactic administration of low-dose heparin was used for 71 of these patients. The incidences of clinical thromboembolic disease were 7 per cent in both the group receiving heparin and the other group. In slightly more than half of the patients, pulmonary emboli occurred in the absence of peripheral deep venous thrombosis. It is suggested that the source of these emboli was thrombosis

Hazard function analysis for development of second primary colorectal cancer after surgery for primary colorectal cancer.

BACKGROUND AND AIM: Although history of colorectal cancer (CRC) is a known risk factor for developing second CRC, the optimal surveillance protocol has not been established. Using hazard function analysis to evaluate changes in the hazard rate for the development of second primary CRCs or high-grade adenomas (HGAs), we aimed to clarify when and on whom to focus in order to effectively identify second primary colorectal neoplasms after initial surgery for CRC. METHODS: We retrospectively enrolled 1823 consecutive patients with stage 0-III CRCs who underwent radical surgery between 2004 and 201

Laparoscopic Colorectal Surgery in Resident Training: Is It Safe and Feasible?

Background/Objectives: The evolution of minimally invasive techniques has established advanced laparoscopic skills as an increasingly essential component of surgical curricula. However, the feasibility of senior residents performing technically demanding procedures such as Laparoscopic Colorectal Surgery (LCS) remains debated. This study evaluated the perioperative safety, short-term oncologic outcomes, and operative-time implications of supervised Laparoscopic Colorectal Surgery (LCS) performed by senior residents (post-graduate years 3 to 5 of a 5-year program). Methods: A retrospective obse…

Disparities in robot utilization in colorectal surgery: the widening gap.

BACKGROUND: Robot-assisted surgery has expanded in adoption and indication among multiple surgical specialties, including colorectal surgery. It is unclear if access to robotic surgery is increasing equally among all patient populations. OBJECTIVE: (1) Identify the factors that may influence type of surgical approach and (2) evaluate disparities in access and subsequent outcomes of colectomy and proctectomy from years 2013-2020 using a national database. STUDY DESIGN: We performed a retrospective study using the National Surgical Quality Improvement Program to identify adult patients who had…

ChatGPT in Colorectal Surgery: A Promising Tool or a Passing Fad?

Colorectal surgery is a specialized branch of surgery that involves the diagnosis and treatment of conditions affecting the colon, rectum, and anus. In the recent years, the use of artificial intelligence (AI) has gained considerable interest in various medical specialties, including surgery. Chatbot Generative Pre-Trained Transformer (ChatGPT), an AI-based chatbot developed by OpenAI, has shown great potential in improving the quality of healthcare delivery by providing accurate and timely information to both patients and healthcare professionals. In this paper, we investigate the potential a…

[Evaluation of efficacy of mechanical bowel preparation in colorectal surgery].

Mechanical bowel preparation (MBP) is commonly used for preparing patients before elective colorectal surgery. MBP has long been considered indispensable for the prevention of complications, especially of the infectious ones. There is little scientific evidence demonstrating the efficiency of this practice in reducing the rate of infectious complications. The purpose of this study was to evaluate the usefulness of MBP in colorectal surgery. A prospective non randomized study analyzed 99 patients divided into two groups (60 with and 39 without MBP) which underwent colonic surgery finished with …

The impact of obesity on outcome after major colorectal surgery.

PURPOSE: There is an epidemic of obesity in the Western world and its associated substantial morbidity and mortality. This review examines the data on the impact of obesity on perioperative morbidity and mortality specifically in the context of colorectal surgery. METHODS: MEDLINE, PUBMED, and the Cochrane library were searched for relevant articles. A manual search for other pertinent papers also was performed. RESULTS: There is good evidence that obesity is a risk factor for wound infection after colorectal surgery. Obesity may increase the risk of wound dehiscence, incisional site herniat…

Major colorectal surgery in a patient with cold agglutinin disease.

We present the case of a 62-year-old man with severe cold agglutinin disease who underwent major colorectal surgery. Cold agglutinin disease is a condition in which auto-antibodies, usually immunoglobulin M, cause red blood cell agglutination at decreased body temperature. Haemolysis may result. Agglutination results in impaired perfusion, resulting in symptomatic Raynaud's phenomenon and acrocyanosis. Haemolysis can result in anaemia and thrombotic events caused by microvascular occlusion, in addition to haemoglobinuria and renal failure. Peri-operative hypothermia is common in all patients a…

Prophylaxis in elective colorectal surgery: the cost of ignoring the evidence.

INTRODUCTION: Three strategies are used to prevent complications in colorectal surgery: heparin and antibiotics given perioperatively and abdominal drains placed intraoperatively. To investigate the appropriate and inappropriate use of these prophylactic techniques and to assess the costs associated with their inappropriate use, we studied patients who underwent elective colorectal procedures. METHODS: We reviewed the charts of 103 patients operated on between April and December 1999 at a 519-bed tertiary care, teaching hospital in Edmonton, Alta. The procedures carried out were elective sigm…

Results of extensive surgery for liver metastases in colorectal carcinoma.

Hepatic resections were performed during the past 13 years on 31 patients with hepatic metastases from colorectal carcinoma. Of the 31 patients, 22 underwent lymph node dissection of the hepatic hilus. Ten patients underwent removal of recurrent lesions in the liver, lung, adrenal gland and brain after initial hepatic resection. The overall 5-year survival rate was 45 per cent. The outcome for six patients who underwent repeat hepatectomy after an initial hepatectomy was significantly better than for nine patients with unresectable recurrence (P less than 0.01). Six of the 22 patients who unde
