The influence of bowel preparation on postoperative complications in surgical treatment of colorectal cancer.

Introduction Colorectal cancer is the most common gastrointestinal cancer treated by departments and surgical clinics in Poland. Currently, the biggest challenge of surgery is to reduce the number of leaks in the bowel anastomoses and postoperative wound infections to a minimum. Objective Whether bowel preparation before surgery affects the early results of treatment? Methods The study was retrospective and included patients operated electively on one surgical center due to colorectal and rectal cancer in years 2013-2018. Patients who underwent surgery were divided into two groups in the study

A bélelőkészítés lehetőségei és hazai gyakorlata a tervezett colorectalis műtétek előtt.

INTRODUCTION: The bowel preparation before elective colorectal surgery is a controversial topic in the surgical practice. During the last 15 years numerous publications concerning the necessity and options of the bowel preparation were published. The aim of our team was to perform a survey about the current, domestic practice as well as to give a short overview of the relevant literature and recommendations. METHODS: 50 surgical departments performing colorectal surgeries routinely were asked to fill out an online, anonymous survey. The data of the survey filled out by 40 surgical departments

Early (microinvasive) colorectal carcinoma. Pathology, diagnosis, surgical treatment.

The term early colorectal carcinoma is used for an infiltrating carcinoma with submucosal spread, but no involvement of the muscle coat (muscularis propria). Our experience with 249 such tumours is reported. Lymph node metastases were detected in only 3% of 130 patients subjected to classical surgery. Early colorectal carcinoma represents a cancer stage with an excellent prognosis (age-corrected 10-year survival rate 100%). The survival rates after limited therapeutic procedures (endoscopic polypectomy, local surgical excision, segmental/tubular resection) or after classical radical surgery do

Incidence, microbiological spectrum, and antimicrobial resistance patterns of surgical site infections at a tertiary care hospital in Pakistan: a retrospective observational study.

BACKGROUND: Surgical site infections (SSIs) are among the most common healthcare-associated infections and contribute substantially to postoperative morbidity, prolonged hospitalization, and increased healthcare costs. Data on SSI epidemiology, microbiological profiles, and antimicrobial resistance patterns remain limited in many low- and middle-income countries, including Pakistan. This study aimed to determine the incidence of surgical site infections and characterize the associated microbiological spectrum and antimicrobial resistance patterns using surveillance data from a tertiary care ho

Major liver resections synchronous with colorectal surgery.

BACKGROUND: Surgical strategy in liver metastases synchronous to colorectal cancer remains controversial. The aim of this study was to evaluate feasibility and short-term outcomes of major hepatectomies synchronous to colorectal surgery. METHODS: Between January 1985 and December 2004, 79 patients underwent major hepatectomy for metastases synchronous to colorectal cancer; 31 underwent synchronous hepatectomy and colorectal surgery, and 48 underwent delayed liver resection. RESULTS: The synchronous group had a higher rate of right colectomy (38.7% vs. 18.8%, P = .0499) and larger metastases

Prophylaxis of infection for elective colorectal surgery.

BACKGROUND: Influenced by the key results of the clinical trials conducted in the early 1970s by Condon, Nichols, and Gorbach, surgeons have adopted the routine use of mechanical bowel prep and antimicrobial prophylaxis prior to elective colorectal procedures as a widely established practice. Recent clinical trial data, however, led us to reexamine the benefits of mechanical bowel preparation, methods of antimicrobial prophylaxis and to assess the role of new, specific risk factors for surgical site infection after colorectal operations. METHODS: Pertinent studies on antimicrobial prophylaxis

Conventional laparoscopic segmental bowel resection with mini-laparotomy specimen extraction compared with Natural Orifice Specimen Extraction (NOSE) procedures in patients with colorectal endometriosis.

BACKGROUND: In patients with colorectal endometriosis the optimal surgical approach for excising disease remains unknown. OBJECTIVES: To compare the safety and efficacy of classic colorectal resection with mini-laparotomy and extracorporeal specimen removal and anastomosis formation (CLS) and colorectal resection with natural orifice specimen (bowel segment) extraction (NOSE), which is performed entirely in the abdominal cavity. METHODS: This is a single centre, retrospective observational study of 161 cases of colorectal endometriosis. Women underwent laparoscopic excision of deep endometri

[Familial adenomatous polyposis: what is new for the clinician?].

Familial adenomatous polyposis (FAP) is a rare autosomal-dominant disease characterized by the development of more than 100 colorectal adenomatous polyps in young adults. In the absence of surgical intervention, colorectal cancer ineluctably develops in all affected patients. Recent progress in the isolation of the gene responsible for the disease allows to detect gene carriers before they present with symptoms attributable to polyps. Moreover, the presence of four or more lesions of congenital hypertrophy of the retinal pigment epithelium is an extracolonic manifestation of FAP allowing presy

Choosing the right technique for deep endometriosis.

The surgical management of bowel endometriosis is a real challenge. In addition to the fact that only symptomatic patients should undergo surgery, no consensus has been approved in the literature. Among the surgical techniques, the surgeon has to choose between rectal shaving, disc excision, or segmental colorectal resection. All those procedures are associated with complications, but the risk of rectovaginal fistula is higher if a disc excision or segmental colorectal resection is performed. It is therefore of utmost importance to evaluate preoperatively the bowel infiltration by several imag

Scintigraphic pattern of planar gallium scan in wound infection after colorectal surgery.

BACKGROUND: The incidence of postoperative wound infection is higher after colorectal surgery than after other surgical procedures. No studies have evaluated the usefulness of gallium 67 scintigraphy for detecting wound infection after colorectal surgery. We investigated the clinical potential of gallium imaging for detecting wound infection after colorectal surgery. METHODS: Fifty patients who had fever after colorectal surgery were enrolled in this study. All patients had open surgical incisions at the midline of the abdomen. Planar gallium scintigraphy was performed 24 h, and then as neede

Indirect Evidence for the Volume-Outcome Relationship in Corrective Surgery for Hirschsprung Disease: Insights from Adult Colorectal Surgery.

Hirschsprung disease (HD) is a rare malformation that requires complex corrective surgery. Low caseload, heterogeneous patient cohorts, and difficulties in linking long-term outcomes to the initial corrective surgery limit the available direct evidence for a volume-outcome relationship. Indirect evidence from adult colorectal cancer (CRC) surgery may be transferable, given the partial similarity of the procedures.Using the Institute for Quality and Efficiency in Health Care (IQWiG) V24-07 rapid report's methodology, this study evaluated the transferability of the volume-outcome relationship fr

Robotic versus Laparoscopic Colectomy for Cancer.

Minimally invasive colon and rectal surgery provides oncologic outcomes at least equivalent to open surgery with a multitude of benefits. While laparoscopic colectomy remains effective and widely practiced, its adoption rate has stalled and robotic surgery offers an opportunity to expand minimally invasive oncologic colorectal surgery. Robotic colectomy overcomes many of the laparoscopic limitations and is increasing in popularity in colorectal practice. Broader dissemination remains constrained by cost, operative setup demands, and the need for structured training. Emerging technologies, incl

Ergonomic port placement in robotic colorectal surgery.

AIM: The aim of the study was to determine how spacing between ports and alignment of ports (oblique or vertical) influences manipulation angles in robotic colorectal surgery. METHOD: Abdominal CT scans of 10 consecutive robotic right hemicolectomy and 10 consecutive robotic high anterior resection patients were analysed. The manipulation angles were calculated using fixed points on the preoperative abdominal coronal CT scan. Port placements were marked on the CT scan. The fixed points used to measure the manipulation angles were from the most lateral part of the caecum, hepatic flexure, sple

Prophylactic surgery for patients with longstanding ulcerative colitis. Which option? Histopathological and clinical implications.

Patients with longstanding chronic ulcerative proctocolitis are at risk to develop colorectal cancer Conflicting views as regards surveillance, the indications for surgery and type of preventive procedure exist. For permanent prevention of cancer development complete removal of all potential malignant colorectal mucosa has to be done. Panprocto-colectomy with a conventional ileostomy or continent ileostomy removing all colorectal mucosa should therefore eliminate further risks of colorectal cancer. Colectomy and ileorectal anastomosis is a controversial issue. While many surgeons today are rel

Robotics on the rise, but not everywhere: National trends in robotic approach for colorectal cancer surgery.

BACKGROUND: Minimally invasive surgical approaches, including laparoscopic and robotic techniques, have been shown to improve the outcomes in colorectal cancer resection by decreasing pain, hospital length of stay, and perioperative mortality. However, broader adoption of robotics faces several barriers. This study aimed to evaluate national trends in robotic colorectal surgery using the National Cancer Database (NCDB). METHODS: Patients with stage I to III colon and rectal adenocarcinomas who underwent resection (2010-2021) were identified. Trends in surgical approaches (open, laparoscopic,

Clinical spotlight review: best practices for the management of colorectal cancer in the emergency and acute care setting.

BACKGROUND: Colorectal cancer (CRC) is a leading cause of cancer mortality, and up to one-third of patients present with obstruction, perforation, or bleeding. Emergency presentation is associated with advanced stage, higher rates of open surgery and stoma formation, and worse survival. This review synthesizes current evidence and provides practical guidance for emergency and acute care management of CRC. METHODS: We conducted a comprehensive search of MEDLINE and EMBASE for English-language studies from 2000 through July 2025, including randomized trials, meta-analyses, and cohort studies. D
