BACKGROUND: The most common psychiatric illnesses in the background of suicide are schizophrenia spectrum disorders (SSD) and affective disorders (AD). While depression and hopelessness are important factors for suicide in psychiatric patients, the role of psychotic symptoms is unclear. Few empirical studies have examined the attitudes and ethical beliefs of psychiatrists. Arsonists are often treated in forensic settings. However, high recidivism rates indicate that treatment is not yet optimal for these offenders. The Danish law concerning commitment and compulsory procedures in psychiatry was reformed in October 1989, and for the first time laid down regulations for compulsory treatment of somatic disease in psychotic patients. The aim of the study was to evaluate whether the law is useful in clinical practice. The course of 28 psychotic patients who were committed and/or compulsorily treated due to severe somatic disease is described. We conclude that the law is a useful tool in the acute treatment of life-endangering somatic disease in psychotic patients.

Seventy-five patients were admitted to the ward of the Lund Suicide Research Center following a suicide attempt. After 5 years, the patients were followed up by a personal semistructured interview covering sociodemographic, psychosocial and psychiatric areas. BACKGROUND: In Belgium, the law of "social defense" stipulates that an accused "which is either in a state of dementia or in a serious state of mental disturbance or mental deficiency, incapable of controlling his actions" can be interned. The establishment of social defense (ESD) in Tournai (Belgium) hosts 350 inmates.

BACKGROUND: Anorexia nervosa is a mental disorder with a high long-term mortality. Patients are ambivalent about treatment and often avoid it. Of necessity, compulsory treatment sometimes must be considered. A report from the Mental Health Act Commission has helped to reduce previous confusion.
AIMS: To identify the premorbid and clinical features that predisposed to compulsory admissions, the short-term benefits of the treatment and the long-term mortality rates (derived from the National Register).
