Policy analysis of access to and reimbursement for nonpharmacologic therapies for cancer-related fatigue.

Cancer-related fatigue (CRF) is an important public health issue that involves millions of community-dwelling cancer survivors. CRF is the most debilitating patient reported symptom related to cancer therapies and exacts a significant economic and social toll. It adversely impacts patients' work, social relationships, and overall quality of life. CRF prevalence ranges from 30% to 90% during therapy and often persists months and years afterwards. This policy analysis examines the problem of lack of patient access to evidence-based nonpharmacologic CRF therapies. The authors use a five-step proc

A Pragmatic Trial of a Multilevel Community-Centered Intervention to Reduce Pregnancy Related and Associated Morbidity and Mortality and Disparities: A Study Protocol.

INTRODUCTION: Maternal morbidity and mortality in the US disproportionately affect some women, including the Medicaid-insured, rural, and Black, Hispanic, and Native American populations. The drivers of disparities are complex and there is an urgent need for multilevel interventions focused on pregnancy-related and associated morbidity and mortality (PRAMM). This study will be the first large-scale test of Community Health Worker (CHW) inclusive home visiting and provider/practice/system level improvement strategies that support empowered patients, pro-active providers, and integrates communit

Irritable bowel syndrome and the incidence of colorectal neoplasia: a prospective cohort study with community-based screened population in Taiwan.

BACKGROUND: We aim to report the prevalence of irritable bowel syndrome (IBS) and elucidate the influence of IBS on the incidence of colorectal neoplasm through a community-screening-based, longitudinal follow-up study. METHODS: We enroled 39,384 community residents aged 40 years or older who had participated in a community-based colorectal cancer-screening programme with an immunochemical faecal occult test since 1999. We followed a cohort that was free of colorectal neoplasm (excluding colorectal neoplasm at baseline) to ascertain the incident colorectal neoplasm through each round of scree

Disability Prevalence and the Awareness-Utilization Gap for Disability Services in an Urban Community of Delhi, India: A Cross-Sectional Study.

INTRODUCTION: Disability-related welfare schemes form a critical component of India's social protection architecture, yet community-level evidence on the gap between awareness and utilization in urban settings remains limited. This study aimed to estimate the prevalence of disability and its socio-demographic correlates and to assess awareness and utilization of disability-related services among adults in an urban community of Delhi, generating baseline data on the awareness-utilization gap for this setting. METHOD: A community-based cross-sectional study was conducted in Palam, an urban rese

Health promotion in the city: a review of current practice and future prospects in the United States.

To achieve its health goals, the United States must reduce the disproportionate burden of illness and poor health borne by urban populations. In the 20th century, patterns of immigration and migration, changes in the global economy, increases in income inequality, and more federal support for suburbanization have made it increasingly difficult for cities to protect the health of all residents. In the last 25 years, epidemics of human immunodeficiency virus infections and substance abuse and increases in homelessness, lack of health insurance, rates of violence, and concentrations of certain po

The impact of digital interventions on health insurance coverage for reproductive, maternal, newborn and child health services utilization in Kakamega, Kenya: a cluster randomized controlled trial.

The National Hospital Insurance Fund (NHIF) of Kenya was upgraded to improve access to healthcare for impoverished households, expand universal health coverage, and boost the uptake of essential reproductive, maternal, newborn and child health (RMNCH) services. However, premiums may be unaffordable for the poorest households. The Innovative Partnership for Universal Sustainable Healthcare (i-PUSH) programme targets low-income women and their households to improve their access to and utilization of quality healthcare, including RMNCH services, by providing subsidized, mobile phone-based NHIF co

Association between social determinants of health and systemic lupus erythematosus: a nationally representative analysis of 2017-2021 data.

Limited US recent data is available on the prevalence of systemic lupus erythematosus (SLE) by patient's social determinants of health (SDOH). Careful assessment of individual SDOH that affects SLE is crucial, as such evidence could help improve care and hence reduce health disparities for patients with SLE, especially for those who are most vulnerable and at the highest risk of poor outcomes. We estimated the prevalence of systemic lupus erythematosus (SLE) overall and by patient's social determinants of health (SDOH), and also explored the associations between SDOH and SLE. We conducted a po

OBJECTIVES: Young adults aged 18 to 24 face increased risk for acquiring sexually transmitted infections (STIs) and reduced access to healthcare as they transition into adulthood and assume responsibility for their health. Furthermore, racial, ethnic, gender, and sexual minority young adults are at an increased risk for STIs and are less likely to have health insurance. This study evaluates the demographics of young adults undergoing STI and human immunodeficiency virus (HIV) testing at a sexual health clinic, and the impact of healthcare access and insurance status on the likelihood of STI di

Improving Access to Eye Care: A Systematic Review of the Literature.

PURPOSE: The goals were to develop a working and inclusive definition of access to eye care, identify gaps in the current system that preclude access, and highlight recommendations that have been identified in prior studies. This manuscript serves as a narrative summary of the literature. CLINICAL RELEVANCE: Health care disparities continue to plague the nation's well-being, and eye care is no exception. Inequities in eye care negatively affect disease processes (i.e., glaucoma, cataracts, diabetic retinopathy), interventions (surgical treatment, prescription of glasses, referrals), and popul

Mindfulness Training Enhances Self-Regulation and Facilitates Health Behavior Change for Primary Care Patients: a Randomized Controlled Trial.

BACKGROUND: Self-management of health is important for improving health outcomes among primary care patients with chronic disease. Anxiety and depressive disorders are common and interfere with self-regulation, which is required for disease self-management. An insurance-reimbursable mindfulness intervention integrated within primary care may be effective for enhancing chronic disease self-management behaviors among primary care patients with anxiety, depression, trauma, and stress-related and adjustment disorders compared with the increasingly standard practice of referring patients to outside

The impact of home- and community-based services on life satisfaction among older adults: evidence from marginal treatment effect framework.

BACKGROUND: In the context of rapid population aging, home- and community-based services (HCBS) have become an important intervention for improving the quality of life among older adults. However, the causal impact and the heterogeneous effects of HCBS utilization on life satisfaction remain insufficiently explored. METHODS: Data from the 2023 China Longitudinal Aging Social Survey (CLASS) were analyzed. An instrumental variable (IV) approach and the marginal treatment effect (MTE) framework were employed to empirically examine the main effects, mechanisms, and heterogeneous effects of HCBS u
