Atrial coronary angiography, tachyarrhythmias and the Ta segment.

Electrocardiograms and angiograms were reviewed to determine if atrial Ta segment displacements and atrial flutter or fibrillation indicate atrial coronary disease. Atrial circulation was assessed by angiography in 28 patients with chest pain and normal coronary arteries, 29 patients with significant stenosis of at least one major coronary vessel, and 16 with coronary artery disease and atrial flutter or fibrillation. The prevalence of Ta segment displacement was 71% without coronary disease and 79% with coronary disease. There was no relationship between Ta displacement and segmental atrial c

[Coronary CT angiography: Diagnosis, prognosis and PCI planning].

Coronary CT angiography (CCTA), long confined to a diagnostic triage role, now occupies a central place at every stage of coronary artery disease management. Diagnostically, it overcomes the limitations of the calcium score - blind to non-calcified plaque - by directly visualizing the arterial wall, and now integrates functional assessment through CT-derived fractional flow reserve. Prognostically, overall atherosclerotic burden and plaque morphology (high-risk plaque, pericoronary fat) prove more predictive of cardiovascular events than stenosis severity alone, a finding that led to the revis

Assessing Invasive Coronary Angiography Interpretation Education and Resources.

BACKGROUND: There is a surprising lack of research surrounding effective teaching and learning methodologies in cardiology, especially in invasive coronary angiography (ICA). Therefore, a survey study was designed to assess perceived gaps in education in trainees' interpretation of ICA and available resources. METHODS: A 20-question survey was distributed via an online survey platform disseminated through a CardioNerds email newsletter and social media. The intended audience included medical trainees and medical students through to subspecialty fellows. RESULTS: The survey received 144 respo…

Complications Following Ulnar Artery Catheterization for Coronary Angiography.

BACKGROUND: Use of radial and ulnar access has increased due to its perceived benefits over femoral access. Ulnar artery catheterization can … plication rates were compared with previously published studies on ulnar artery catheterization for coronary angiography (percutaneous coronary intervention). RESULTS: A total of 41 patients were available for review with a mean age of 59 years. Of these, 17 patients (41%) sustained complications in the immediate postprocedural period. These complications included he …

Evaluation of Routine Coronary Angiography Before Pulmonary Thromboendarterectomy.

BACKGROUND: At the University of California, San Diego, routine coronary angiography has generally been performed in men 40 years of age and older and women 45 years of age and older before pulmonary thromboendarterectomy for chronic thromboembolic pulmonary hypertension (CTEPH). The prevalence of significant coronary artery disease (CAD) in this population has not been evaluated, however, and the optimal screening strategy has not been established. This study sought to evaluate whether the current approach may be better optimized on the basis of cardiac risk factors. METHODS: This study incl…

Reconciling discordant myocardial perfusion imaging and coronary angiography.

A common clinical conundrum presents itself in the discordance between nuclear stress testing and invasive coronary angiography (ICA) in the patient presenting with angina. A patient with an abnormal perfusion scan and "normal coronary angiography" may result in the patient's symptoms being dismissed as "non-cardiac." Alternatively, a patient with a "normal perfusion study," who nonetheless undergoes ICA and is found to have significant coronary artery disease may confound efforts to risk stratify and potentially treat patients with angina. This paper will review the current evidence to explai…

Changes in Medical Management after Coronary CT Angiography.

INTRODUCTION: Coronary computed tomography angiography (CCTA) allows for non-invasive coronary artery disease (CAD) phenotyping. There are still some uncertainties regarding the impact this knowledge has on the clinical care of patients. OBJECTIVE: To determine whether CAD phenotyping by CCTA influences clinical decision making by the prescription of cardiovascular drugs and their impact on non-LDL cholesterol (NLDLC) levels. METHODS: We analysed consecutive patients from 2008 to 2011 submitted to CCTA without previous diagnosis of CAD that had two serial measures of NLDLC, one up to 3 month

Coronary CT angiography and high-risk plaque morphology.

Computed tomography angiography (CTA) is commonly employed for exclusion of coronary artery disease and demonstration of the extent of coronary vascular involvement. It has been recently proposed that coronary artery plaques could be visualized noninvasively. This review article focused on the high risk plaque detected by CTA. Plaque characteristics of acute coronary syndrome (ACS) was compared to sable angina pectoris (SAP). The presence of positive remodeling (ACS 87 %, SAP 12 %, p < 0.0001), low attenuation plaque (LAP) (ACS 79 %, SAP 9 %, p < 0.0001), and spotty calcification (ACS 63 %, SA

Screening CT-coronary angiography: ready for prime time?

Computed tomographic coronary angiography (CTCA) can assist with the diagnosis of a variety of cardiovascular disorders. The rationale for performing screening CTCA is to define the presence, absence, and severity of coronary artery disease, particularly in those patients who are categorized to be at intermediate risk by conventional risk factor assessment for a cardiovascular event. In addition to coronary artery disease, the interventional cardiologist can also use CTCA to evaluate the presence of an anomalous origin of the coronary arteries, the size of the coronary arteries for potential s…

The Transulnar Approach to Coronary Angiography and Intervention: Assessing the Anatomy of the Ulnar Artery Using Angiography.

BACKGROUND: The transulnar approach (TUA) has been proposed as a safe alternative to the more established transradial approach (TRA) for cardiac catheterization. However, no study has assessed the anatomy and variability of the ulnar artery using angiography. METHODS: A retrospective analysis of patients who underwent transradial cardiac catheterization during routine clinical care was conducted. Both quantitative and qualitative measurements of artery diameter were collected. RESULTS: Among 700 consecutive patients, mean distal ulnar artery diameter (UAD) was larger in men (3.2 ± 0.9 mm) co

Analysis of anomalous origin of coronary arteries by coronary angiography in Chinese patients with coronary artery disease.

With the development of coronary angiography for the diagnosis of coronary artery disease, its clinical significance in detecting coronary artery anomalies and evaluating the seriousness is attracting more attention. In the study we aimed to assess the prevalence of anomalous origin of coronary arteries in a Chinese population who underwent coronary angiography for coronary artery disease, and explore any patterns in the common variants and typical anomalies, especially the potentially serious ones. Patients who underwent coronary angiography from January 2013 to December 2016 in Fuwai Hospita…

Assessment of coronary veins in patients with isolated coronary artery ectasia by antegrade coronary venous angiography.

BACKGROUND: Coronary artery ectasia (CAE) is characterized by an abnormal dilatation of the coronary arteries. The most common cause of CAE … e coronary venous system was imaged in a left anterior oblique view at an angle of 45° by antegrade coronary angiography. RESULTS: Patients with isolated CAE had significantly larger coronary veins compared with control individuals with angiographically normal coronary arteries (coronary sinus ostium: 10.1 ± 1.0 vs. 8.5 ± 2.2 mm, respectively, P=0.003; coronary sinus …

Thermal coronary angiography: a method for assessing graft patency and coronary anatomy in coronary bypass surgery.

Thermal coronary angiography was evaluated in 50 patients undergoing 137 saphenous vein and 48 internal mammary artery bypass grafts. A total of 177 thermal coronary angiograms were performed after completion of the distal anastomoses by injection of cold cardioplegia into the vein or by reperfusion with warmer blood in the internal mammary artery grafts. These angiograms provided details of graft and anastomosis patency, flow directions, and presence of native coronary stenoses. Temperature differences between the injectant and the epimyocardium of greater than 4 degrees C resulted in high-co…
