Thirty premenopausal women with recurrent, pronounced cyclical mastodynia associated with mammographically confirmed fibrocystic disease were studied. In order to establish the current treatment of cyclical mastalgia, a postal questionnaire was sent to 276 consultant general surgeons (over 25% of the UK total), randomly selected from the 12 UK regional health authorities. Surgeons were questioned about their choices of treatment for cyclical mastalgia, after initial resassurance, and for persistent pain. Ovarian endocrine function in endometriosis was studied in 19 women before, during, and after treatment with danazol. Striking abnormalities were seen during treatment. Serum estradiol (E2) and testosterone (T) levels increased markedly, while follicle-stimulating hormone (FSH), luteinizing hormone (LH), and prolactin were unaffected. A case of endometriosis causing ureteral obstruction in a young woman was observed for 3.5 years. The conclusion is that early diagnosis is important and that intravenous pyelography must be carried out at the slightest suspicion. In some cases initial treatment with Danazol alone can be tried. Otherwise the treatment must be surgical, possibly combined with Danazol.

A phyllodes tumor appeared at the site of a resected fibrocystic disease focus. Administration of danazol resulted in temporary regression, but the tumor resumed growth. Relative hyperestrogenism also continued to increase in spite of continued long-term administration of danazol. After 12 months a simple mastectomy was performed. Most of the resected mass consisted of the phyllodes tumor, but it also contained fibrocystic disease.

BACKGROUND: Hereditary angioedema with C1 inhibitor deficiency (C1-INH-HAE) is a rare, potentially life-threatening disorder characterized by recurrent edematous attacks. The edema formation is the consequence of interaction of bradykinin and various vasoactive peptides with endothelium. An analysis of abdominal wall endometrioma management was performed. History of cesarean section and/or uterine surgery and ultrasound are important in the diagnosis. Danazol test may confirm the diagnosis as well as reduce tumor size before operation. HF Electrosurgery, including spray coagulation technique are recommended in all cases. Hormonal therapy, including Danazol, Medroxyprogesterone or Lynestrenol gives poor effects in cases of endometrioma.
