OBJECTIVE: We described treatment approaches after 20 doses of denosumab, including continuation or transition to zoledronic acid or romosozumab, and examined subsequent BMD trajectories.
METHODS: This retrospective single-centre cohort included patients who received ≥20 consecutive doses of denosumab at the Osteoporosis Centre between June 2012 and December 2024. Characteristics of patients who continued denosumab were compared with those who transitioned to zoledronic acid or romosozumab. BMD was obtained from DXA, and trajectory analyses were restricted to patients without delayed dosing a

Osteoporosis international : a journal established as result of cooperation between the European Foundation for Osteoporosis and the National Osteoporosis Foundation of the USA2025eng
UNLABELLED: Denosumab is an effective osteoporosis therapy, but discontinuing treatment can result in rebound-associated fractures. Guidelines recommend starting follow-on osteoporosis therapy within 6 months of the last dose, but real-world practice is under-described. We found only 6.0% of patients initiated follow-on therapy within 1 year after denosumab discontinuation, suggesting many are at increased fracture risk.
PURPOSE: We aimed to describe the incidence and predictors of follow-on osteoporosis therapy use for US Medicare beneficiaries who discontinued denosumab.
METHODS: In this c

Glucocorticoid-induced osteoporosis (GIOP) is a major cause of secondary bone loss characterized by rapid trabecular deterioration and increased fracture risk, and while bisphosphonates are widely used as first-line therapy, the comparative effectiveness of denosumab remains clinically relevant. This systematic review synthesized evidence from randomized trials, imaging studies, and observational cohorts evaluating denosumab versus bisphosphonates in adults receiving glucocorticoids, focusing on bone mineral density, bone turnover, microarchitecture, fractures, and safety. Denosumab consistent

Atypical femur fractures (AFFs) are rare complications of antiresorptive therapies like denosumab, resulting from suppressed bone remodelling. These fractures occur with minimal or no trauma and are often preceded by prodromal thigh or groin pain. Radiographically, they present as transverse or short oblique fractures with lateral cortical thickening and minimal comminution. We report a case of an elderly bisphosphonate-naïve patient on denosumab who developed a subtrochanteric AFF. The patient underwent intramedullary nailing, denosumab was discontinued, and a transition to bisphosphonates was

Zoledronic acid and denosumab are bone-modifying agents that are clinically important in multiple aspects of bone management for breast cancer patients. These aspects include the prevention of osteoporosis induced by cancer-treatment bone loss, treatment and prevention of bone metastasis, and improvement of survival directly or indirectly by maintaining bone health. Interestingly, zoledronic acid and denosumab have different anticancer activities, and they may be associated with the improvement of survival of breast cancer patients under different mechanisms. Zoledronic acid is the most potent

A rebound of osteoclast activity during the 2 years after a treatment or prevention of osteoporosis with denosumab (Dmab) leads to an increased risk of vertebral fractures (VFs). We attempted to identify the risk factors for these VF and to examine the protective role of bisphosphonates. For that, 22 specialists in Switzerland provided data of unselected patients, treated with denosumab for osteoporosis or breast cancer without metastases under aromatase inhibitors, who have received at least two injections of Dmab, with at least 1 year of follow-up after discontinuation. The questionnaire cov

A 40-year-old man, with a history of metastatic parathyroid carcinoma, status post primary tumour resection and lung metastasectomy, was hospitalised for persistent severe hypercalcaemia and elevated parathyroid hormone levels despite conventional management and escalating doses of cinacalcet. A single dose (120 mg) of denosumab was given and his calcium level plummeted from 14.8 mg/dL to 5.5 mg/dL. After second lung metastasectomy, he developed prolonged hypocalcaemia that required calcium and vitamin D supplements for more than 3 years. In patients with severe hypercalcaemia refractory to co

Denosumab leads to sustained suppression of bone turnover if given every 6 months, with escape occurring approximately 2 to 3 months after the last dose. Whilst escape from denosumab has been reported in malignancy, there has only been one reported case in the setting of osteoporosis. We present the case of a 62-year old woman with systemic sclerosis who did not respond to denosumab secondary to premature escape from suppression of bone resorption. Our patient was diagnosed with osteoporosis 15 years previously based on bone densitometry, with no prior fragility fractures. Initial treatment wi

Osteoporosis international : a journal established as result of cooperation between the European Foundation for Osteoporosis and the National Osteoporosis Foundation of the USA2015eng
Osteoporosis treatments are usually given for a limited period of time in order to balance benefits and risks. We report three cases of postmenopausal women without any previous fragility fracture who presented severe spontaneous vertebral fractures after denosumab discontinuation. We think that the occurrence of these fractures could be explained by the severe rebound effect observed after denosumab discontinuation and that a consensus regarding the end of treatment with denosumab has to be defined.

CONTEXT: Hypercalcemia associated with myelofibrosis is rare, and its pathogenesis and treatment are not known.
OBJECTIVE: We report a unique case of hypercalcemia associated with post-essential thrombocythemia myelofibrosis and review the clinical and laboratory features, pathogenesis, and responsiveness to treatment with the bone antiresorptive agent, denosumab.
RESULTS: A 62-yr-old woman with essential thrombocythemia presented with progression to myelofibrosis with lytic skull lesions and symptomatic hypercalcemia. Other causes of hypercalcemia were excluded. Her disturbance in calcium h

BACKGROUND: Giant cell tumors of bone (GCTB) are RANK/RANK-ligand positive, progressive osteolytic tumors. There was no medical treatment for GCTB based on efficacy and safety data from Chinese patients. A single-arm, phase II study demonstrated the promising efficacy of JMT103 in unresectable or surgically challenging GCTB. Patient-level outcomes from the single-arm trial were compared with real-world data from denosumab or non-denosumab treated patients to estimate comparative efficacy in unresectable or surgically challenging GCTB.
METHODS: The real-world data were retrospectively collecte

Osteoporosis international : a journal established as result of cooperation between the European Foundation for Osteoporosis and the National Osteoporosis Foundation of the USA2026eng
UNLABELLED: A patient with multisystem Langerhans cell histiocytosis and extensive jaw osteolysis experienced remarkable mandibular bone regeneration following short-term denosumab therapy. This unique observation challenges current concepts of denosumab action in the jaw and provides novel in vivo evidence of structural repair through RANKL inhibition.
BACKGROUND: Denosumab is an established inhibitor of receptor activator of nuclear factor-κB ligand (RANKL) that effectively suppresses osteoclastic bone resorption but has also been associated with medication-related osteonecrosis of the jaw

Central giant cell granuloma (CGCG) is a rare, benign but potentially aggressive jaw lesion. While surgery is a standard treatment, it carries high recurrence rates and morbidity, particularly in young patients. Denosumab, a RANK-L inhibitor, has emerged as a non-surgical alternative, though its long-term efficacy remains uncertain. We present a 16-year-old female with aggressive mandibular CGCG, treated with multiple resections and bone grafting. To avoid radical reconstruction, denosumab was administered for 12 months, leading to complete ossification and over 4 years of disease stability. H

PURPOSE: Optimal dosing of denosumab in osteogenesis imperfecta (OI) remains undefined. This prospective cohort study evaluated the 12-month efficacy and safety of denosumab in OI patients, with a historical control study with alendronate.
MATERIALS AND METHODS: Eight pediatric patients (1 mg/kg every 3 months; ≤60 mg/dose) and ten adults (60 mg every 6 months) received denosumab. Outcomes included lumbar spine (LS) and femoral neck (FN) bone mineral density (BMD), bone turnover markers (BTMs), vertebral compression fractures (assessed via AI-assisted Genant grading [AI_OVF_SH system]), fract

Prolongation of the corrected QT (QTc) interval is a known risk factor for life-threatening ventricular arrhythmias and can result from medications, electrolyte imbalances, or both. We report the case of a 70-year-old female with atrial fibrillation on amiodarone and osteoporosis treated with denosumab who presented with dizziness, but without syncope, palpitations, or chest pain. She was hemodynamically stable with negative orthostatic vitals, and neurologic examination revealed a positive Romberg sign. Brain imaging was unremarkable. ECG demonstrated marked QTc prolongation >550 ms (referenc

Medication-related osteonecrosis of the jaw (MRONJ) is a rare but well-recognized complication of treatment with antiresorptive agents. Medication-related osteonecrosis of the external auditory canal (MROEAC), on the other hand, is even rarer and mostly reported during bisphosphonate exposure. Its pathophysiology is thought to involve complex multifactorial processes, including inhibition of bone remodeling, altered angiogenesis, infection, and inflammation. Due to the poor recognition of MROEAC, ear canal health is not routinely discussed with patients, and there is no requirement for auditor

BACKGROUND: Denosumab has been reported to improve bone mineral density (BMD), but the clinical impact of denosumab on osteoporosis in kidney transplant recipients (KTRs) remains controversial.
METHODS: We analyzed 98 KTRs who used denosumab from 2018 to 2023. We investigated the change in BMD, laboratory findings, complications of denosumab, fracture risk assessment tool (FRAX) score, acute rejection within 1 year, and graft failure.
RESULTS: Mean T-scores at 1 year after denosumab were significantly increased compared to mean T-scores pre-denosumab at the femur neck and spine area, respect
