Immunology of allergic contact dermatitis.

Allergic contact dermatitis (ACD) is a T-cell mediated skin inflammation caused by repeated skin exposure to contact allergens. This review summarizes current knowledge on the immunology of ACD. Different phases in ACD are distinguished, i.e. sensitization, elicitation and resolution phases. We discuss contact allergen presentation and the central role of antigen presenting cells during sensitization phase. There is an extremely complex interaction of different kinds of immune cells, such as antigen presenting cells, T, B, NK lymphocytes, keratinocytes (KCs), endothelium, mast cells (MCs) and

[Allergic contact dermatitis and atopy].

Retrospective studies demonstrate that the prevalence of skin sensitization does not significantly differ between atopic and non-atopic patients. In children and adolescents the risk for sensitization seems to occur independently from AD. According to the results of a recent study, AD patients are overrepresented in the group of polysensitized patients. IgE-mediated sensitization as well as an early onset of AD and duration of the disease have been identified as possible risk factors for skin sensitization to contact allergens. A defective permeability barrier with increased epidermal water lo

Immunotherapy of allergic contact dermatitis.

The term 'immunotherapy' refers to treating diseases by inducing, enhancing or suppressing immune responses. As allergy is an excessive, detrimental immune reaction to otherwise harmless environmental substances, immunotherapy of allergic disease is aimed at the induction of tolerance toward sensitizing antigens. This article focuses on the historical developments, present state and future outlook for immunotherapy with haptens as a therapeutic modality for allergic contact dermatitis. Inspired by the effectiveness of immunotherapy in respiratory allergies, attempts were undertaken at curing a

Contact dermatitis to Vicks VapoRub.

Vicks VapoRub (VVR) is a commonly used inhalant ointment that helps relieve symptoms of upper respiratory tract infections. It contains several plant substances, including turpentine oil, eucalyptus oil, and cedar leaf oil, which can potentially irritate or sensitize the skin, as well as camphor, menthol, nutmeg oil, and thymol. Although many reports describe allergic contact dermatitis (ACD) to the various constituents in VVR ointment, there are no cases of VVR directly causing ACD. We present a case of a patient who developed an ACD secondary to application of her VVR.

Allergic contact dermatitis from cigarettes.

Cigarettes are widely known to contain potent carcinogens, and their smoke contributes to many chronic and potentially fatal diseases. Cigarettes may also represent an underreported and underrecognized cause of allergic contact dermatitis (ACD). Potential allergens from cigarettes can be found in the filters, paper, and tobacco. This article reviews the current literature on ACD from cigarettes to understand the clinical manifestation of ACD from cigarettes, to recognize components in cigarettes as potential sources of ACD, and to describe how to patch-test patients with suspected ACD from cig

Allergic and irritant contact dermatitis.

Irritant and allergic contact dermatitis are common inflammatory skin diseases induced by repeated skin contact with low molecular weight chemicals, called xenobiotics or haptens. Although both diseases may have similar clinical presentations, they can be differentiated on pathophysiological grounds. Irritant contact dermatitis (ICD) is a non-specific inflammatory dermatitis brought about by activation of the innate immune system by the pro-inflammatory properties of chemicals. Allergic contact dermatitis (ACD) corresponds to a delayed-type hypersensitivity response with a skin inflammation me

[Allergic contact dermatitis to cosmetics].

INTRODUCTION: Contact dermatitis to cosmetics is a common problem in the general population, although its prevalence appears to be underestimated. We reviewed cases of allergic contact dermatitis to cosmetics diagnosed in our dermatology department over a 7-year period with a view to identifying the allergens responsible, the frequency of occurrence of these allergens, and the cosmetic products implicated. METHODS: Using the database of the skin allergy department, we undertook a search of all cases of allergic contact dermatitis to cosmetics diagnosed in our department from January 2000 thro

Allergic contact dermatitis: the future.

This article discusses the future of allergic contact dermatitis (ACD) diagnosis and management. Due to early landmark discoveries in this disorder, there is a knowledge base that can be refined. Improved patch-test technology, patch-test strategy, and patch-testing clinical relevance are emphasized. Other factors for improvement include complete ingredient identification on Material Safety Data Sheets, knowledge of the relationship of extracutaneous allergic disease to ACD, understanding of the evolutionary implications of ACD, increased chemical identification of allergens in publication, an

Allergic contact dermatitis from carmine.

A 28-year-old woman developed allergic contact dermatitis within 6 to 24 hours exclusively after using carmine-containing eyeshadows and lipsticks. She had both a positive patch test result and a positive antecubital repeated open application test result with carmine 2.5% in petrolatum. Thirty other patients had negative patch test results. Carmine is a widely used pigment derived from gravid cochineal insects. Carminic acid is the source of its color. Only two previous publications describing allergic contact dermatitis from carmine could be found. The ingredient in carmine causing these dela

Medical management of contact dermatitis.

Allergic and irritant contact dermatitis are important dermatological problems. Although the frequencies of positive reactions to a number of allergens have decreased during last 30 years because of better avoidance (and at least in part due to improved legislation), contact allergy to other agents is rising. The medical treatment starts from a correct identification of triggers of contact dermatitis which could allow patients to reduce or avoid exposure to these agents in future. A good clinical history, examination and immunological tests including patch testing are of crucial importance at

Irritant contact dermatitis: a review.

Irritant contact dermatitis is the most common form of contact dermatitis, and yet is often overlooked. Recent progress in understanding the pathogenesis has reignited the interest of clinicians in this area of dermatology. Irritant contact dermatitis is not a homogenous entity, but rather a number of subtypes contributing to different clinical presentations. The diagnosis of irritant contact dermatitis is often clinical, and may only be possible after the exclusion of allergic contact dermatitis with patch testing. There is no readily available diagnostic test. There is an incomplete understa

Contact dermatitis from a prosthesis.

Patients wearing a prosthesis face a wide variety of medical problems. Skin complications have long been recognized, but their prevalence is still unknown. The most frequently reported disorders are allergic contact dermatitis (ACD), acroangiodermatitis, epidermoid cysts, epidermal hyperplasia, follicular hyperkeratosis, verrucous hyperplasia, bullous diseases, hyperhidrosis, infections, malignancies, and ulcerations. Contact dermatitis represents one-third of the dermatoses in amputees wearing prostheses. All patients who are suspected of having ACD should be patch tested with standard allerg

Occupational carprofen photoallergic contact dermatitis.

BACKGROUND: The nonsteroidal anti-inflammatory drug carprofen was used in humans in the 1980s, before its withdrawal due to adverse effects. It re-emerged for veterinary uses, for which it is still widely prescribed, in the 1990s. There has been one previous report published of photoallergic contact dermatitis (PACD) in a pharmaceutical factory worker exposed to carprofen. OBJECTIVES: Investigation of carprofen as a cause of PACD in pharmaceutical factory workers presenting with facial dermatitis. METHODS: Photopatch testing to carprofen dilutions in two pharmaceutical factory workers and th

[Contact dermatitis caused by foods].

Cutaneous adverse reactions to foods and food additives are a growing public health problem which can occur both in the avocational and occupational settings. This article reviews different reaction patterns which can occur upon contact with foods and discusses some clinically important food-associated metal and fragrance allergens responsible for these adverse effects. As ultimately, education and guidance can minimize the morbidity of food allergy and enhance the quality of life of the affected individual; this article highlights this growing problem.

Allergic contact dermatitis to chloroxylenol.

Chloroxylenol, also known as p-chloro-m-xylenol (PCMX), is a compound that has been used as a preservative in cosmetics and as an active agent in antimicrobial soaps. We present two patients with allergic contact dermatitis from PCMX, confirmed by positive (+++) patch-test reactions at 48 and 72 hours, identification of PCMX in a soap and in a hand cream used by the patients, and improvement following withdrawal of the incriminating products. The mechanism of action, structure, antimicrobial activity, and dangers of PCMX are reviewed.
