Diaphragmatic hernia in adults often presents with overlapping respiratory and abdominal symptoms. They may be simple diaphragmatic eventrations or undiagnosed Bochdalek's hernias or may even be of post traumatic variety. Diaphragmatic hernias may be asymptomatic, present only with respiratory symptoms, or may present with obstruction and strangulation of involved bowel loops with faeco-pneumothorax. The index case was operated for open diaphragmatic hernia repair six years back and admitted for breathlessness with absence of abdominal signs and symptoms. Patient subsequently developed hydro-p…

Gastric volvulus has been rarely associated with diaphragmatic paralysis or eventration. In this article, we present the case of a patient with idiopathic paralysis of the left hemidiaphragm that underwent treatment with a robotic thoracoscopic diaphragm plication, which was complicated by massive gastric volvulus resulting in such significant intra-abdominal hypertension that the ipsilateral diaphragm ruptured anterior to the plication suture line.

Intrathoracic heterotopic liver tissue is an exceedingly rare clinical entity. In most patients, it is associated with other congenital defe … ymal hamartoma arising from an intrathoracic heterotopic liver that was found incidentally during a diaphragmatic eventration repair. Association of ectopic liver tissue with mesenchymal hamartoma has not previously been reported in the literature.

We report a case series of seven patients aged 24 to 78 years who presented with unilateral elevation of the diaphragm secondary to diverse underlying mechanisms. The diagnoses included sarcoidosis-related pulmonary inflammation, gallbladder adenomyomatosis with sepsis and metabolic derangement, pulmonary tuberculosis with Chilaiditi's syndrome, acute pulmonary embolism with diaphragmatic eventration, superior vena cava syndrome due to non-small cell lung carcinoma, cryptogenic organizing pneumonia with an incidental thymic cyst and pyogenic liver abscess. This series emphasizes that unilatera…

A rare case of dextrocardia in association with eventration of left hemidiaphragm who failed to thrive is presented. Growth retardation could not be attributed to these congential anomalies.

Elevated diaphragm can be due to diaphragmatic eventration or diaphragm paralysis. Diaphragm paralysis is a rare condition that can be congenital or acquired. Acquired diaphragmatic paralysis can result from injury to the phrenic nerve. Subsequently, there is loss of contractility of the diaphragm muscle leading to progressive atrophy and, hence, distension of the dome of the diaphragm leading to elevated right, left, or both copula of the diaphragm. Diaphragm plication aims to return the abdominal contents back to their normal position and allow for greater lung expansion by reducing the abun…

A full term neonate was operated for diaphragmatic eventration through the chest. Postoperatively the baby developed gastric outlet obstruction. Ultrasonography and barium meal examination were suggestive of extrinsic compression in the region of the pylorus. At laparotomy a gall bladder perforation was found producing a biloma just abve the pylorus. Drainage of the bilioma and temporary cholecystostomy cured the gastric outlet obstruction.

A two-month-old girl was operated for a left-sided eventration of the diaphragm. At surgery, a mass representing an extralobar pulmonary sequestration (EPS) was found inferior to the left lobe of the lung and was excised. Histopathological examination of the resected specimen showed presence of a type II congenital cystic adenomatoid malformation (CCAM) within it. A review of the relevant literature is presented.

Diaphragmatic palsy leads to a permanent ascension of one or both hemi-diaphragms with highly variable functional impact. The underlying mechanisms can be divided into two main categories: neurological or muscular disorder leading to peripheral dysfunction; defective or non-transmitted central command causing central dysfunction. A complete morphological and functional work-up is required to determine the circumstances leading to diaphragmatic palsy and the uni- or bilateral nature of the paralysis. The entire phreno-diaphragmatic transmission chain from the cranium to the diaphragmatic muscle

Gastric volvulus may present acutely with Borchardt's triad or with chronic vague abdominal symptoms. Predisposing factors include lax ligaments, bands, adhesions, paraesophageal diaphragmatic hiatus hernia, and eventration of the diaphragm. The goals of surgery are to detorse the stomach, correct conditions associated with volvulus and prevent a recurrence. Anterior gastropexy with or without colon displacement is mandatory. Gastric resection may be required for strangulation and necrosis.

The case of a seven-month-old infant is reported who had an eventration of the left hemidiaphragm associated with two bronchopulmonary foregut anomalies, duplication of the esophagus, and intralobar sequestration of the left upper lobe. The literature indicates that eventration of the diaphragm is associated with pulmonary sequestration in up to 50% of patients. Since the eventration itself obscures roentgenographic evidence of other foregut anomalies, the concomitant diagnosis and treatment of possible foregut anomalies is another justification for surgical repair of eventration of the diaphr

PURPOSE: Congenital diaphragmatic herniae (CDH) may require patch closure in 50% of the cases. We assessed a biologic and composite mesh in a porcine CDH model.
METHODS: Left sided thoracotomy was performed in 20 pigs. Approximately, 30% of the diaphragm was excised and the patch (Surgisis® or Parietex®) inserted to close this defect. The pigs were killed at 6 months and the diaphragm was harvested for biomechanical and histological assessment.
RESULTS: The mean weight of the pigs at surgery and killing were 6.1 kg (4.2-8.4 kg) and 94.1 kg (80-131 kg), respectively. There were two recurrence

Repair of a large diaphragmatic defect in congenital diaphragmatic hernia (CDH) and eventration of the diaphragm (DE) is difficult, especially when this is an unexpected finding at surgery. A patch of synthetic material may not be available at short notice, especially in developing countries. We describe the repair of nine such defects by using an abdominal muscle flap comprising the transversus abdominis and internal oblique muscles based on the intercostal and subcostal vessels. Although reports of similar flap repairs have been published in the literature [1-5], these employed a subcostal i
