Ulcerative duodenitis with luminal constriction in two horses.

Proximal enteritis was diagnosed on the basis of clinical signs of disease in 2 Standard-bred yearlings from the same farm. Large volumes of fluid were repeatedly obtained during gastric decompression. The horses did not respond to treatment with IV administered fluids, antimicrobial agents, cimetidine, and metoclopramide. Postmortem findings confirmed severe focal chronic active duodenitis with stricture formation. An etiologic agent was not identified.

Increased incidence of duodenitis in chronic renal failure.

Forty-six consecutive patients with chronic renal failure (CRF) were assessed radiographically, endoscopically and histologically for duodenal inflammation. Their gastric secretion was also examined. Thirty-eight patients with non-ulcer dyspepsia served as controls. Histologically established duodenitis was more common in CRF patients than in controls. Among the non-dialyzed patients its prevalence reached 28% (9/32), while in the controls it was 5% (2/38) (p less than 0.05). The CRF patients with duodenitis had upper GI symptoms, hypersecretion of acid (p less than 0.05) and duodenal ulcer di…

Ulcer associated and nonspecific duodenitis treatment by cimetidine.

Of a total of 230 patients subjected to endoscopy for various reasons 16 were found to have duodenal ulcer and duodenitis (group A) and 16 duodenitis but without an ulcer (group B). Duodenal mucosal biopsy was done in both groups and histological confirmation of duodenitis was noted in 9 of the group A and in 10 of group B. Endoscopy after short-term cimetidine treatment in group A patients revealed ulcer healing in 87.5 percent and improvement of duodenitis in 50 percent. In group B the duodenitis improved endoscopically in 43.7 percent. Histology confirmed the improvement of duodenitis in 31…

Natural history of chronic duodenitis: incidence in Nigerians.

Chronic peptic duodenitis accounted for four percent of endoscopic diagnoses of upper gastrointestinal disorders in lbadan, Nigeria. This clinicopathologic entity was a frequent cause of x-ray negative dyspepsia and was a cause of significant bleeding in two of the 26 patients included in this study. None progressed to frank duodenal ulceration during the follow-up period. It is emphasized that accurate assessment of dyspepsia must include endoscopic and histologic examinations.

Endoscopic duodenal biopsies in coeliac disease and duodenitis.

Endoscopic duodenal biopsies were taken from 27 patients with suspected coeliac disease and compared with intubation capsule jejunal biopsies. The specimens were reported without knowledge of the patients' names or symptoms. In 24 patients (89%), coeliac disease could either be diagnosed or excluded with 100% accuracy, despite the inability to orientate the biopsies correctly. Six biopsies were considered technically unsatisfactory, but only in three (11%) was it impossible to exclude coeliac disease. Duodenal biopsies were also taken from 118 consecutive patients attending for routine upper g

Previously Unreported Pseudomembranous Duodenitis: A Case Report With Histopathology.

The pseudomembranous inflammatory process is a process characterized by the formation of a white membrane-like exudate over colonic mucosa and is mainly caused by Clostridium difficile toxin. The stool culture is considered to be the gold standard and is technically challenging and is not performed routinely. There are some reports of duodenitis and proximal jejunitis in horses attributed to Clostridium difficile infection. Hereby, we report a case of pseudomembranous duodenitis in a seven-year-old boy with a complaint of severe abdominal pain. Upper endoscopy revealed patchy ulceration and a …

Two cases of diffuse duodenitis associated with ulcerative colitis.

The upper gastrointestinal tract is not generally considered a target organ in ulcerative colitis (UC). However, several cases showing upper gastrointestinal involvement in UC have been reported. In this report, we present 2 rare cases of diffuse duodenitis accompanying pancolonic UC. Case patient 1 was a 44-year-old man who developed diffuse duodenitis shortly after colectomy and was successfully treated with mesalazine. Case patient 2 was a 25-year-old woman who developed diffuse duodenitis under a steroid-free condition and was successfully treated with prednisolone. The 2 patients had Heli…

Symptomatic CMV duodenitis. An important clinical problem in AIDS.

Duodenal disease resulting from cytomegalovirus (CMV) infection is unusual in the absence of other gastrointestinal disease. We report two cases of CMV infection symptomatically isolated to the duodenum. One patient presented with ulcer-type dyspepsia, and the other with severe gastrointestinal bleeding. Ganciclovir therapy resulted in both a clinical and endoscopic remission. The importance of endoscopic evaluation is discussed given the necessity of mucosal biopsy for diagnosis and the availability of specific and effective therapy.

Phlegmonous duodenitis complicating multiple myeloma: a successfully treated case.

A patient receiving chemotherapy for multiple myeloma suddenly developed an acute abdomen, fever, and neutrophil leukocytosis. At laparotomy, the distal two-thirds of the duodenum was swollen and hemorrhagic and was surgically excised. The specimen displayed an acute phlegmonous (suppurative) duodenitis with submucosal and transmural acute inflammation and edema despite an intact mucosa. Blood cultures grew Group B, beta-hemolytic streptococci and gram-positive cocci were present histologically. The patient recovered uneventfully following the surgery and a course of broad spectrum antibiotic …
