The role of subthalamic nucleus (STN) deep brain stimulation (DBS) is well established in Parkinson's disease, but experience with STN DBS in other movement disorders is limited. We report on a patient with medically refractory cervical dystonia and essential tremor resulting in dystonic head tremor and action tremor of the hands who obtained complete tremor suppression and near resolution of her cervical dystonia with bilateral STN stimulation. The results in this case demonstrate that STN DBS can dramatically improve dystonia and tremor in nonparkinsonian movement disorders.

This case report focuses on withdrawal dystonia, a movement disorder associated with neuroleptics. Its occurrence in a patient with Tourette's disorder complicated the clinical picture. A misinterpretation of the symptoms led to ineffective management of the movement disorder. The presence of increased blinking with facial pain, dystonic movements, and other facial movements at each neuroleptic dose reduction pointed toward withdrawal dystonia rather than toward a worsening of Tourette's disorder. Implications for diagnosis and treatment are discussed.

Pathogenic variants of the IRF2BPL gene have been mostly associated with early onset epileptic encephalopathy. Movement disorders such as dystonia and ataxia were also reported, with symptoms mainly developing between childhood and adolescence. Here we describe a family with several members affected by a late onset dystonic and ataxic progressive syndrome, caused by a novel heterozygous pathogenic variant in the IRF2BPL gene.

BACKGROUND: Sensory tricks (also termed "alleviating maneuvers") are voluntary maneuvers that transiently alleviate dystonic postures or movements and represent a hallmark clinical feature of dystonia with diagnostic and mechanistic significance. Despite extensive phenomenological description, the evidence base has not been systematically synthesized.
OBJECTIVES: To characterize the prevalence, phenomenology, clinical effects, predictors, neurobiological mechanisms, and translational relevance of alleviating maneuvers in dystonia, and to prospectively evaluate the feasibility of quantitative

BACKGROUND: Generalized dystonia is a complex and heterogeneous movement disorder characterized by sustained or intermittent muscle contractions that lead to abnormal postures and movements.
PATHOPHYSIOLOGY: It primarily arises from dysfunction within basal ganglia circuits and involves a range of underlying pathophysiological mechanisms, including disrupted dopamine synthesis, altered calcium signaling, and impaired stress response pathways. Advances in genetics have identified key causative mutations-most notably in KMT2B, VPS16, EIF2AK2, PRKRA, and AOPEP- that contribute to disease onset a

The adult-onset focal dystonias are a group of clinically heterogeneous disorders that affect different regions of the body. Although they affect different regions with different clinical manifestations, there is evidence that etiopathogenesis is shared at the anatomical, physiological, and genetic levels. However, there is also evidence that etiopathogenesis varies. This chapter summarizes the evidence for lumping or splitting these apparently different clinical phenotypes. It also includes some potential explanations to explain the similarities and differences.

Dystonia is probably the most common form of movement disorder encountered in the clinical practice. It is characterized by sustained muscle contractions, usually producing twisting and repetitive movements or abnormal postures or positions. Dystonias can be classified in several ways, including primarily by the clinical phenomenology or by the underlining etiology, in particular to understand if the presentation is genetically determined. By advances of genetics, including contemporary genomic technologies, there is a growing understanding of the molecular underpinnings of genetically determi

Primary monogenic forms of dystonia manifest solely or mainly with dystonia; they have been linked to a number of genes and loci and assigned "DYT" numbers. The pure dystonia syndrome early-onset primary dystonia (DYT1) manifests with dominantly-inherited generalized dystonia, often with focal onset in a limb. DYT1 is caused by a GAG deletion in the TOR1A gene. Mutations in the THAP1 gene cause DYT6, a form of pure dystonia that primarily involves cranio-cervical and upper limb muscles. Patients with the dystonia plus syndrome DYT5 display levodopa-responsive dystonia sometimes associated with

PURPOSE OF REVIEW: This article reviews current evidence on etiology, diagnosis and clinical management of patients with a challenging movement disorder referred to as focal hand dystonia (FHd).
RECENT FINDINGS: Patients who present to a rhematologist with a history of repetitive overuse, weakness, pain and involuntary, end-range posturing of the digits when performing a target task may have FHd. The etiology is considered idiopathic and multifactorial. There are no specific laboratory or clinical tests to 'rule in' or 'rule out' the diagnosis. Comparative neuroimaging studies report inadequa

Paralysis and movement disorders unexplained by identifiable neurological disease are common clinical presentations in neurological outpatient clinics. Despite their frequency and potential importance from a neuroscientific viewpoint these disorders have received surprisingly little attention in clinically oriented neuroscientific research. In this review we summarize the available literature that specifically relates to the functional neuroanatomy of psychogenic movement disorders. We discuss the aetiology, clinical phenomenology, the current concepts of the dynamic neuroanatomical networks u
