Combined Lung and Cranial Ultrasound for Early Identification and Severity Stratification of Neonatal Respiratory Distress Syndrome.

Early identification and severity stratification of neonatal respiratory distress syndrome (NRDS) are essential for timely respiratory support and prevention of complications. This prospective observational study evaluated the clinical utility of a combined lung ultrasound (LUS) and cranial ultrasound (CrUS) framework for multidimensional bedside risk assessment in 137 neonates with respiratory distress. Baseline LUS and CrUS examinations were performed within 6 h of admission, followed by dynamic LUS monitoring at 24 h and 72 h. The integrated model (Clinical + LUS + CrUS) demonstrated superi

Prognostic value of cranial ultrasound findings in infants aged <90 days with bacterial meningitis: a single-centre retrospective cohort study.

BACKGROUND: Bacterial meningitis (BM) poses a serious threat to infant health. We assessed cranial ultrasound (CUS) changes in infants with BM as possible predictors of the neurological sequelae of BM. METHODS: We retrospectively assigned 132 infants diagnosed with BM from 2007 to 2021. Neuroimaging characteristics and cerebral blood flow (CBF) profiles identified using CUS were analysed and compared between the groups during the acute and postacute phases of BM. RESULTS: Overall, 102 infants with CUS and outcome data were recruited. 37/102 (36.3%) infants with neurological developmental imp

Utility of Cranial Ultrasound to Investigate Brain Injury in Hypoxic-Ischemic Encephalopathy.

BACKGROUND: With increasing availability of brain magnetic resonance imaging (MRI) in high-income countries, cranial ultrasound (cUS) is used less frequently to evaluate infants with hypoxic-ischemic encephalopathy (HIE). This study aimed to correlate findings of brain injury on early postnatal cUS with brain injury on neonatal brain MRI performed as part of routine clinical care for near-term and term infants with moderate to severe HIE. METHODS: This was a retrospective cohort study comparing early postnatal cUS and later neonatal brain MRI using scoring systems with prognostic validity to

Cranial Ultrasound Findings in Infants With Congenital Cytomegalovirus Infection in a Universal Newborn Screening Study in Minnesota.

BACKGROUND: Congenital cytomegalovirus (cCMV) is the most common infectious cause of neurodevelopmental deficits in US children. To inform patient management, it is important to define whether central nervous system (CNS) manifestations are present at birth. This study characterized neuroimaging findings in infants with cCMV identified by a universal screening study in Minnesota during February 2016-December 2022. METHODS: Newborns with cCMV infection (confirmed by urine CMV polymerase chain reaction [PCR] testing, obtained following a positive screening saliva and/or dried blood spot result)

Transcranial Doppler series part IV: case studies.

Transcranial Doppler (TCD) is considered one of the most difficult ultrasound studies to perform. Knowledge of cerebral hemodynamics is essential to the interpretation of the TCD study. The most common uses for TCD are the evaluation for vasospasm after subarachnoid hemorrhage and for sickle cell vasculopathy. Other uses are the detection of vessel narrowing or occlusion, collateral flow secondary to internal carotid artery occlusion, arteriovenous malformations, and to confirm cerebral circulatory arrest. Case studies are presented to demonstrate the hemodynamic parameters that are used to in

Periventricular leukomalacia and prenatal methamphetamine exposure: a case report.

Periventricular leukomalacia (PVL) is a complication of prematurity that carries a high risk of long-term neurologic morbidity. We report the first case, to our knowledge, of unexpected PVL associated with in utero methamphetamine exposure in a 30-week gestation premature infant with a benign hospital course, who subsequently developed cerebral palsy by 24 months of life.

Sonographic diagnosis of congenital brain malformations: the Ilorin experience.

A retrospective study was done to determine the incidence of congenital malformation of the brain among African infants over a 5 year period. Transfontanelle ultrasound (US) was performed on 98 infants consisting of 45 males and 53 females. The mean age was 2.6 months (range 1-11 months). Congenital malformation was present in 25.5% of the 98 cases studied, most of which were associated with hydrocephalus. Cerebral aqueduct stenosis was the commonest congenital anomaly encountered and was found in 24% of case. This study shows that transfontanelle US is a useful tool for diagnosis of congenita

[Post mortem brain MRI: an alternative for pathology examination in Bourneville tuberous sclerosis of the fetus?].

Antenatal discovery of cardiac rhabdomyomes evokes the diagnosis of Bourneville's disease. Antenatal brain exploration with ultrasonography and magnetic resonance imaging (MRI) can highlight cerebral localizations. In the event of termination of pregnancy, confirmation of the cerebral lesions can be achieved with post mortem MRI as well as pathology examination. MRI can be usefully employed in the event pathology examination is not feasible.

Reproducibility of transcranial Dopplersonography: a validation study.

Transcranial Doppler-sonography (TCD) is a useful noninvasive technique for the measurement of intracranial arterial blood flow velocity. The present study evaluated the reproducibility of this method for repeated measurements of flow velocity in the anterior, middle and posterior cerebral, and basilar arteries. Thirty-six patients (23 males and 13 females) were studied. Three measurements were made by the same examiner on each patient with a time interval of 1 h between the first and the second examination, and about 24 h between the first and the third examination. The statistical evaluation

Osteopetrosis: brain ultrasound and computed tomography findings.

A case of osteopetrosis presenting with rare features of dysmorphism with proptosis due to hypoplasia of the orbits and the temporal bone is described. The case also had calcifications in the periventricular regions, the falx cerebri and the corpora colliculi. These features and sonographic findings of osteopetrosis have not been reported previously. The sensitivity and specificity of imaging modalities in the diagnosis of intracranial calcifications is discussed.

[Cerebral blood flow in healthy newborn infants in the early neonatal period].

Dynamic Doppler scanning studies of brain blood flow were carried out in 78 healthy newborns in the first days of their lives. The measurements were carried out in the internal carotid, basilar , anterior, median, and posterior cerebral arteries at the age of 30 min, 2 h, 1, 2, 3, and 4 days. Vascular resistance of brain vessels was assessed and the resistance index estimated. This index was found to increase by the second hour of life (p less than 0.001) in the anterior, median, and internal carotid arteries, whereas no noticeable changes in this index value were recorded in the basilar and p

Cerebral Doppler studies in the fetus and newborn infant.

The goal of developing reliable commercial Doppler systems for measuring vessel diameter and velocity changes during the cardiac cycle appears to be near. Reaching this goal would enable us to obtain volume-flow information continuously. In animal experiments, continuous measurements of Doppler velocity, pressure, and flow add important insights into hemodynamic measurements. Incorporating Doppler methods in microcirculatory research could also provide a link between the microcirculatory and the macrocirculatory hemodynamic research. Although Doppler methods have been validated, Doppler findin

[Differential diagnosis of periventricular pseudocysts in the neonatal period].

Periventricular pseudocysts are important sequelae of different noxious insults in the developing brain. Accurate diagnosis of those pathologic entities during early life has therefore become of direct concern to the clinician. When present at birth, they suggest an intrauterine pathology. It has only been with the advent of real-time cranial ultrasonography that subependymal cystic lesions can be detected antemortem. 25 infants were postnatal diagnosed in ultrasound as having periventricular cystic lesions. They belonged to different pathological entities: focal paraventricular pseudocysts (5
