Elephantiasis Nostras Verrucosa of the Lower Limb: A Case Report and Literature Review.

Elephantiasis nostras verrucosa (ENV) is a rare complication of chronic lymphedema, leading to progressive cutaneous hypertrophy and disfigured lower extremities. We present an unusual case of a 79-year-old man with ENV, who had an episode of erysipelas treated and cured with antibiotics. The patient presented with progressive painless lymphedema of the lower limb years ago, complicated with skin changes. The biology and radiology test does not objective any causes. The compression therapy was proposed to the patient. ENV is characterized by lymphedema and skin anomaly. Management of ENV is of…

Diagnosis of elephantiasis nostras verrucosa as a clinical mani-festation of Kaposi's sarcoma.

Elephantiasis nostras verrucosa, a rare manifestation of Kaposis sarcoma, is a progressive cutaneous hypertrophy caused by chronic non-filarial lymphedema secondary to obstruction of the lymphatic system that can lead to severe disfigurement of parts of the body that have gravity-dependent blood flow, due to edema, fibrosis, and hyperkeratosis, especially lower extremities. Among the various conditions that can induce chronic lymphedema are tumors, trauma, radiotherapy, obesity, hypothyroidism, chronic venous stasis, and AIDS-related Kaposis sarcoma. Kaposis sarcoma is a vascular tumor associa…

Elephantiasis nostras verrucosa in the setting of lymphoedema tarda and neurofibromatosis type 1.

Elephantiasis nostras verrucosa (ENV) comprises an uncommon skin disease characterised by dermal fibrosis with hyperkeratotic, verrucous and papillomatous lesions that usually occur after chronic lymphoedema. We describe the case of a 56-year-old-man with neurofibromatosis type 1, no known surgical history, no chronic medication and no travel history, presenting with worsening non-pitting oedema and impressive foul-smelling mossy plaques and cobblestone-like nodules in both legs and feet, especially on the right, compatible with the diagnosis of ENV.

Giant elephantiasis neuromatosa in the setting of neurofibromatosis type 1: A case report.

Elephantiasis neuromatosa (EN) can arise from a plexiform neurofibroma of the superficial and deep nerves developing from a hyperproliferation of the perineural connective tissue infiltrating adjacent fat and muscles. To date, the clinical association between EN and neurofibromatosis type 1 (NF1) has been poorly defined, particularly with regard to the role of lymphatic alterations and the consequent lymphedema. The present study reports the clinical and biomolecular features of EN in a NF1 patient with the clear clinical diagnostic criteria of multiple cafè-au-lait macules, neurofibromas, EN,…

Elephantiasis of non-filarial origin (podoconiosis) in the highlands of north-western Cameroon.

Lymphoedema, a condition of localized fluid retention, results from a compromised lymphatic system. Although one common cause in the tropics … area and dissected was found to harbour any stage of W. bancrofti. These findings indicate that the elephantiasis seen in the North West province of Cameroon is of non-filarial origin.

Non-filarial elephantiasis in the Mt. Elgon area (Kapchorwa District) of Uganda.

Following reports of a high frequency of elephantiasis in Kwen County (Kapchorwa District) on the slopes of Mt. Elgon in Uganda, a baseline survey for lymphatic filariasis was carried out in three villages in the affected area. Individuals aged 1 year and above were examined for chronic manifestations of lymphatic filariasis, and for specific circulating filarial antigens and microfilariae of Wuchereria bancrofti. Elephantiasis was observed in all age groups from 10 years and above. The overall prevalence was 4.5%, and the prevalence among individuals aged >/=20 years was 8.2%. Males and femal…

A Case of Chronic Elephantiasis Nostra Verrucosa Highlighting the Progression of Chronic Lymphedema.

Elephantiasis nostra verrucosa (ENV) is a rare form of secondary, nonfilarial chronic lymphedema, typically linked to conditions such as morbid obesity, congestive heart failure, and chronic venous insufficiency (CVI). Bacterial infection is thought to initiate a vicious cycle of inflammation, limb swelling, and fibroblast proliferation that results in the "cobblestone" appearance of the affected extremity. A 67-year-old male with a past medical history of end-stage renal disease (ESRD) on hemodialysis, morbid obesity, type 2 diabetes mellitus, and ENV presented to our ED with cellulitis of hi…

The Effect of Lymphoedema Exercises and Foot Elevation on the Quality of Life of Patients with Elephantiasis.

Filariasis is a chronic infectious disease caused by filarial worms. Swelling in the legs in patients with filariasis can result in a significantly lower quality of life. The recommended treatments for patients who experience swelling or lymphoedema are lymphoedema exercises and foot elevation. This research is a quantitative study with a quasi-experimental design including pre- and posttreatment tests with a control group. This study used a cluster sampling method, which is a nonprobability sampling technique. The samples in this study were 48 respondents divided into two groups: 24 responden

The Successful Treatment of Elephantiasis Nostras Verrucosa With Ablative Carbon Dioxide Laser.

BACKGROUND AND OBJECTIVE: Elephantiasis nostras verrucosa (ENV) is a disfiguring skin condition that is difficult to treat. Existing treatment modalities serve to improve cosmesis or treat symptoms. Herein, we report a case of ENV with lymphocutaneous fistula successfully treated with ablative carbon dioxide laser. STUDY DESIGN/PATIENTS AND METHODS: A 57-year-old woman with biopsy-proven ENV with lymphocutaneous fistula was treated with ablative carbon dioxide laser to the symptomatic area of her right thigh in 3 treatment sessions over 6 months. RESULTS: The patient had resolution of lympho…

Elephantiasis Nostras Verrucosa (ENV): a complication of congestive heart failure and obesity.

Congestive heart failure (CHF) and obesity are common medical conditions that have many complications and an increasing incidence in the United States. Presented here is a case of a disfiguring skin condition that visually highlights the dermatologic consequences of poorly controlled CHF and obesity. This condition will probably become more common as CHF and obesity increase in the US.

[A case of scrotal elephantiasis 30 years after treatment of penile carcinoma].

A 67-year-old man visited our hospital with complaints of scrotal swelling associated with occasional febrile episodes. Physical examination … mination of the biopsy specimen from the scrotal mass demonstrated lymphangiectasia consistent with elephantiasis of the scrotum. Surgical excision of this huge scrotal mass was performed in August 1997. The resected tissue weighed 1,400 g. Convalescene was uneventful. He subsequently underwent perineal urethrostomy in place of the suprapubic cystostomy.

Predominant CD8+ infiltrate in limb biopsies of individuals with filarial lymphedema and elephantiasis.

In 34 individuals with a spectrum of clinical manifestations of Bancroftian filariasis, we investigated whether immunoperoxidase-stained, random, superficial dermal biopsies could further elucidate the nature of the diffuse damage to superficial lymphatics that had been recently demonstrated by radionuclide lymphoscintigraphy. A total of 78% and 68% of limbs from patients with clinical disease and asymptomatic microfilaremia, respectively, contained EN4+PAL-E- lymphatic vessels that were abnormally dilated. The majority of subjects, regardless of clinical classification, had a CD3+ perivascula

[Elephantiasis of the vulva and the legs. Medical and surgical treatment of a case of lymphatic filariasis].

After reviewing the main causes of elephantiasis in tropical countries, the results of combined medical and surgical treatment of a case of lymphatic filariasis are reported. Medical treatment consists of a cycle of high-dose mebendazole (40 mg/kg/die) and subsequent cycles of diethylcarbamazine at a habitual dose (6 mg/kg/die). In the interval, the elephantiasic right lip was removed surgically. The overall clinical results was good. Noteworthy was an unwanted, leukopenising side-effect of high-dose mebendazole.

[Lymphoedema and elephantiasis due to filariasis. Pathogenesis and clinical aspects (author's transl)].

Adult lymphatic filariae (Wuchereria bancrofti, Brugia malayi) can cause blocking of lymphatics producing obliterating endolymphitis lesions. The subsequent extravasation of lymph (or chyle when the obstruction is canal) is at the origin of the formation of lymphedema or elephantiasis, in which the main histological finding is great hypertrophy of collagen elements. This theory involving filaria only is not the full picture, and bacterial infection, mainly by streptococci, is an important factor. The association of filaria with microbes is particularly dangerous because the presence of the lat…

Reduction perineoplasty for scrotal elephantiasis: a method of surgical reconstruction of the penis and scrotum.

Scrotal elephantiasis can be insidious in onset, idiopathic as to cause and debilitating in progress. The massive scrotal enlargement results in a sac-like structure that first envelopes and then completely covers the penis, testes and the regional spermatic cord structures, causing loss of topographic identification. The urethra leads into a stratified, squamous epithelial-lined canal, which traverses the edematous subcutaneous tissue to open irregularly at the inferior aspect of the scrotal bag. The difficulty of acquiring sustained periods of cleanliness, the incontinence of urine despite a…
