Necrotizing enterocolitis: factors affecting mortality in 101 surgical cases.

One hundred one infants requiring operative treatment for necrotizing enterocolitis were evaluated for prenatal, perinatal, therapeutic, clinical, laboratory, radiographic, and operative factors as they relate to mortality. Infants less than 28 weeks gestation and those with birth weight less than 1500 gm had an increased mortality rate. The use of umbilical arterial catheters, high-density premature formulas, and aminophylline (for apnea) had an adverse effect on survival. Persistent acidosis, oliguria, and abdominal wall erythema resulted in a significantly worse outcome. The presence of por

The complex interaction of enteral feeding in the pathogenesis of necrotizing enterocolitis.

NEC is a multifactorial gastrointestinal disorder affecting medically fragile infants in the neonatal intensive care unit (NICU) and is associated with significant morbidity and mortality. Although the precise pathogenesis of NEC is not well understood, prematurity remains a significant risk factor. Immature intestinal immunity, altered microbial colonization (dysbiosis), and dysregulated inflammatory responses contribute to NEC susceptibility in the preterm intestine.1 During NEC, an exaggerated inflammatory response can lead to intestinal tissue damage and in severe cases, surgical resection

Sepsis in neonates with necrotizing enterocolitis: outcomes and predictive model.

Sepsis after neonatal necrotizing enterocolitis (NEC) onset was associated with substantially adverse outcomes. However, effective tools for predicting sepsis risk in this special population remain lacking. This study aimed to investigate the influence of sepsis after NEC onset on the outcomes and develop a prediction model for early identification of sepsis. Retrospective review of 530 patients diagnosed with NEC was reviewed with clinical data. In this study, the overall proportion of sepsis was 56.2% (298/530). The rates of surgical intervention (63.09% vs. 18.10%) and mortality (17.11% vs.

Gut microbiota in necrotizing enterocolitis: a bibliometric and visualization analysis.

BACKGROUND: Necrotizing enterocolitis (NEC) is a severe gastrointestinal disease affecting neonates, especially preterm and very low birth weight infants. Recent studies highlight the role of the gut microbiota in neonatal health, with microbial dysbiosis linked to NEC onset. However, bibliometric analyses in this field remain relatively scarce. This study aims to conduct a comprehensive analysis of the literature related to gut microbiota and NEC published between 1996 and 2024, to outline the development of this research area, identify emerging trends, and provide insights for future investi

Unusual Presentation of Necrotizing Enterocolitis Presenting in a Term Infant.

Necrotizing enterocolitis (NEC) is a gastrointestinal emergency causing inflammation and necrosis of the intestine, most commonly in premature formula-fed infants. There exists limited conflicting data on the pathophysiology behind NEC development and the underlying intestinal bacteria behind the disease. We report a case of a two-week-old female patient who was born full term. During her birth hospitalization, she was transferred to the neonatal intensive care unit for late-onset tachypnea and apnea, where she received ampicillin and gentamicin intravenously through a peripheral line for a to

Sacrococcygeal Teratoma With a Unique Gastrointestinal Presentation of Necrotizing Enterocolitis.

A neonate with an antenatally detected mass in the coccygeal region experienced sudden abdominal worsening, with abdominal distension and bloody aspirates on day 4 of life. The abdominal radiograph revealed pneumatosis intestinalis (necrotizing enterocolitis stage IIa), requiring discontinuation of feeds and initiation of intravenous antibiotics. Computed tomography of the mass and abdomen showed that the coccygeal mass had an intra-abdominal extension, pushing the bowel loops and likely representing a Type II sacrococcygeal teratoma. Given the lesion's large size and vascularity, we hypothesi

Neonatal Necrotizing Enterocolitis: An Update on Pathophysiology, Treatment, and Prevention.

Necrotizing enterocolitis (NEC) is a life-threatening disease predominantly affecting premature and very low birth weight infants resulting in inflammation and necrosis of the small bowel and colon and potentially leading to sepsis, peritonitis, perforation, and death. Numerous research efforts have been made to better understand, treat, and prevent NEC. This review explores a variety of factors involved in the pathogenesis of NEC (prematurity, low birth weight, lack of human breast milk exposure, alterations to the microbiota, maternal and environmental factors, and intestinal ischemia) and r

Potential role of bile acids in the pathogenesis of necrotizing enterocolitis.

Necrotizing enterocolitis (NEC) is one of the most common acute gastrointestinal diseases in preterm infants. Recent studies have found that NEC is not only caused by changes in the intestinal environment but also by the failure of multiple systems and organs, including the liver. The accumulation of bile acids (BAs) in the ileum and the disorder of ileal BA transporters are related to the ileum injury of NEC. Inflammatory factors such as tumor necrosis factor (TNF)-α and interleukin (IL)-18 secreted by NEC also play an important role in regulating intrahepatic BA transporters. As an important

A Nomogram for Predicting Surgical Timing in Neonates with Necrotizing Enterocolitis.

OBJECTIVE: To explore the surgical risk variables in patients with necrotizing enterocolitis (NEC) and develop a nomogram model for predicting the surgical intervention timing of NEC. METHODS: Infants diagnosed with NEC were enrolled in our study. We gathered information from clinical data, laboratory examinations, and radiological manifestations. Using LASSO (least absolute shrinkage and selection operator) regression analysis and multivariate logistic regression analysis, a clinical prediction model based on the logistic nomogram was developed. The performance of the nomogram model was eval

A protocol for the induction of experimental necrotizing enterocolitis in neonatal mice.

Recapitulating human NEC using animal models has been insightful in dissecting the signaling pathways, immune-mediated mechanisms, genetic signatures, and the intestinal architecture of NEC. This protocol describes an in vivo murine NEC model, using hypoxia and formula containing lipopolysaccharide and enteric bacteria derived from an infant with NEC. With this mouse model, we aim to further dissect NEC pathogenesis and develop new therapeutic strategies. For complete details on the use and execution of this protocol, please refer to Mihi et al. (2021).

Sumac (Rhus coriaria) for the prevention and treatment of necrotizing enterocolitis.

Leukotrienes, free oxygen radicals, tumor necrosis factor-alpha, and inflammatory mediators play major roles in the development of necrotizing enterocolitis (NEC). Rhus coriaria (RC: sumac) extracts may enhance cell viability by reinforcing defenses against free radical species in several progressive diseases as well as inflammatory diseases. The aim of the present study was to evaluate the effects of RC in a rat NEC model in terms of intestinal damage. Newborn pups were separated into three groups: control, NEC, and NEC treated with RC. Mortality and clinical sickness scores were evaluated. A

Acquired-B phenomenon in a neonate presenting with necrotizing enterocolitis.

OBJECTIVE: We are reporting a case of acquired B phenomenon in a 14 days old pre-term baby presenting with necrotizing enterocolitis (NEC) where it was detected as an ABO discrepancy. METHOD: The forward and reverse grouping was done using tube technique as well as column agglutination technique (ABD card and LISS Coombs AHG gel cards, Biorad, Switzerland). The direct antiglobulin test (DAT) was also done on gel card. Further work up was done using acidified (0.1 N HCl) anti-B (pH 6-6.5) polyclonal anti-B (from group A donors) for resolution of ABO discrepancy. RESULTS: The forward grouping

Topiramate for Seizures in Preterm Infants and the Development of Necrotizing Enterocolitis.

Neonatal seizures represent a significant health burden on the term and preterm neonatal population and are linked to poor long-term neurodevelopmental outcomes. Currently, there are no US Food and Drug Administration-approved antiepileptic drugs for neonates, and authors of the medical literature have yet to reach a consensus on the most adequate approach to neonatal seizures. Topiramate is readily used in the adult and older pediatric population for the management of migraines and partial-onset seizures. Topiramate continues to gain favor among pediatric neurologists who often recommend this

Characteristics and incidence of transfusion-associated necrotizing enterocolitis in the UK.

Background and aims: The etiology of necrotizing enterocolitis (NEC) is unclear and postulated as being multifactorial. It has been suggested that one causative factor is the transfusion of packed red blood cells (PRBCs) leading to the disease entity commonly referred to as transfusion-associated NEC (TANEC). TANEC has been reported in North America but its incidence has not been formally investigated in the UK. Our aims were to identify the incidence of NEC and TANEC in tertiary-level UK neonatal units and to describe characteristics of TANEC cases.Materials and methods: Using strict case def

Outcome in neonates with necrotizing enterocolitis and patent ductus arteriosus.

BACKGROUND: There is no agreement of the influence of patent ductus arteriosus (PDA) on outcomes in patients with necrotizing enterocolitis (NEC). In this study, we assessed the influence of PDA on NEC outcomes. METHODS: A retrospective study of 131 infants with established NEC was performed. Outcomes (death, disease severity, need for surgery, hospitalization duration), as well as multiple clinical parameters were compared between NEC patients with no congenital heart disease (n=102) and those with isolated PDA (n=29). Univariate, multivariate and stepwise logistic regression analyses were p
