Acute Hemophilus parainfluenzae epiglottitis in an adult.

A 60-year-old man, who had non-insulin-dependent diabetes but was otherwise in good health, was hospitalized for sudden upper airway obstruction and respiratory arrest. Findings on fiberoptic laryngoscopy were consistent with acute epiglottitis. Blood cultures and factor analysis later revealed that the responsible pathogen was Hemophilus parainfluenzae. Acute epiglottitis in the adult is no longer considered rare and is usually attributed to H influenzae. The case described here and two other cases reported elsewhere indicate that H parainfluenzae may also be a cause of this serious and often…

Adult epiglottitis, cellulitis, and Streptococcus pneumoniae bacteremia.

Described is an adult with acute epiglottitis, cellulitis of the neck and chest, and Streptococcus pneumoniae bacteremia. Although this syndrome is usually associated with Haemophilus influenzae type b, this case illustrates that in adults a similar syndrome can be produced by S. pneumoniae. The failure to determine an etiologic agent in approximately two thirds of reported adult cases of epiglottitis raises the question as to whether S. pneumoniae, a common adult respiratory pathogen, might fill this etiological void. To establish such an association between S. pneumoniae and acute epiglottit…

Management of acute epiglottitis by nasotracheal intubation.

In 40 cases of acute epiglottitis in children, intubation was the chosen method for the management of airway obstruction. Six patients were treated without the establishment of an artificial airway, and no tracheostomies were done. No patients who were admitted to the hospital died of airway obstruction, although one sustained irreversible brain damage before admission, and two died of overwhelming infectionmthe average duration of intubation was 2.days and the average hospital stay was 5.days. Two children developed subglottic granulation tissue that was removed successfully and did not recur…

Vaping-induced acute epiglottitis: a case report.

BACKGROUND: E-cigarette use, or vaping, is an alternative nicotine delivery system that is becoming increasingly prevalent in adolescents and young adults. There is currently a lack of comprehensive research on the adverse effects of vaping on the upper airway. Acute epiglottitis is a potentially life-threatening condition that can lead to airway obstruction. It is commonly caused by bacterial infections such as streptococci, staphylococcus, and Moraxella. Adult patients with acute epiglottitis mainly present with odynophagia, dysphagia, and respiratory difficulties. The diagnosis of epiglotti…

Bilateral empyema secondary to Hemophilus influenzae epiglottitis.

An empyema is a collection of pus in the pleural space, often due to aerobic bacteria, that requires drainage typically done with a small bore chest tube and fibrinolytics. The large majority of empyemas are unilateral. Bilateral empyemas are rare, usually non-infectious and typically require surgical management. We report a case of Hemophilus influenzae epiglottitis complicated by bilateral culture proven empyema successfully managed without surgery. The aim of this report is to present a rare complication of epiglottitis and discuss the theoretical issues with management of multiple infected…

Acute Epiglottitis Due to COVID-19 Infection.

UNLABELLED: A 53-year-old man presented acutely to the Accident and Emergency department with a 2-day history of progressive odynophagia and shortness of breath. The patient had stridor at rest and acute epiglottitis was suspected. The patient was transferred urgently to theatre for intubation but due to a severely oedematous airway, this was unsuccessful and emergency tracheotomy was performed by the ENT team. Throughout admission the only positive microbiological sample was a nasopharyngeal swab for SARS-CoV-2 infection. In the absence of other positive microbiology, it is highly likely that…

Airway intervention in cases of acute epiglottitis.

UNLABELLED: Airway intervention in cases of acute epiglottitis.Problem/objectives: In cases of acute epiglottitis, indications for airway intervention have not been established. In the present study, we reviewed patients with acute epiglottitis to identify clinical factors, which suggest airway intervention should be performed. METHODOLOGY: Patients with acute epiglottitis admitted to The Jikei University Daisan Hospital (Tokyo) from 2004 to 2013 were identified. Patients' characteristics, histories, laryngoscopic findings and laboratory findings were reviewed and analysed. RESULTS: Of the 8…

Symptoms and signs differentiating croup and epiglottitis.

AIM: To determine differentiating symptoms and signs of epiglottitis and laryngotracheobronchitis (croup). METHODS: Contemporaneous interview of parents and clinical examination of children with acute upper airway obstruction presenting to the intensive care unit of a paediatric hospital. RESULTS: Two hundred and three children were examined over a 40-month period. One hundred and two had croup, of whom 49 had the diagnosis confirmed at intubation and another six by direct laryngeal inspection without intubation. One hundred and one had epiglottitis of whom 95 were diagnosed by direct inspec…

[Acute epiglottitis in Iceland from 1983-2005].

OBJECTIVE: To describe the changes in the epidemiology of acute epiglottitis in Iceland from 1983-2005. METHODS: All patients with discharge diagnosis of epiglottitis during the study years were identified and diagnosis confirmed by chart review. Main outcome measures were age, gender, month/year of diagnosis, microbiology, airway management, ICU admissions, choice of antibiotics, length of hospital stay and major complications/mortality. RESULTS: Fifty-seven patients were identified (annual incidence 0.93/100.000). The mean age was 33.3 years (1-82). Childhood epiglottitis disappeared after…

[Epiglottitis: a children's disease seen in adults].

Two women, aged 58 and 62, developed epiglottitis with acute symptoms of sore throat, respiratory distress, fever, and stridor on inhalation. Both developed an upper respiratory tract obstruction requiring acute intubation to secure an open airway. The symptoms disappeared after administration of intravenous antibiotics and corticosteroids. Both women were discharged without any residual signs of infection. The incidence of epiglottitis in adults is most likely rising. The illness used to be seen almost only in children. A clear explanation for this rise in incidence cannot be given. It is imp…

[Antimicrobial immunity in patients with acute epiglottitis].

Fifty-three patients with catarrhal epiglottitis and 31 patients with epiglottic abscess aged 16-60 years were examined. It was determined that development of epiglottitis is tightly related to abnormalities in reactivity of phagocytic and humoral arms of immunity. Decreased affinity of produced antibodies, opsonizing properties of serum as well as phagocytic and biocide activity of neutrophils were revealed in patients. In patients with catarrhal and necrotic epiglottitis similar abnormalities of immunoreactivity were observed although in patientswith necrotic epiglottitis they were more pron…
