Critical ethical questions arise concerning whether studies among adolescents of new behavioral and biomedical HIV preventive interventions such as Pre-Exposure Prophylaxis (PrEP) should obtain parental permission. This paper examines the relevant regulations and ethical guidance concerning waivers of parental permission, and arguments for and against such waivers. Opponents of such waivers may argue that adolescent decision-making is "too immature" and that parents always have rights to decide how to protect their children. Yet requiring parental permission may put adolescents at risk, and/or

PURPOSE: Research among adults shows benefits and low perceived burden of engaging in behavioral research. However, questions remain regarding the ethics of conducting behavioral research in pediatric populations during sensitive situations, including during a new life-threatening diagnosis or at end-of-life. We examined reactions to participating in a behavioral reproductive research study among male adolescents newly diagnosed with cancer and their parents, as a step towards optimizing fertility preservation utilization in a population where future infertility is common.
METHODS: Pediatric

BACKGROUND: The participation of vulnerable patients in clinical research poses apparent ethical dilemmas. Depending on the nature of the vulnerability, their participation may challenge the ethical principles of autonomy, non-maleficence, or justice. On the other hand, non-participation may preclude the building of a knowledge base that is a prerequisite for defining the optimal clinical management of vulnerable patients. Such clinical uncertainty may also incur substantial economic costs.

The growing evidence base for biomedical HIV prevention interventions - such as oral pre-exposure prophylaxis, microbicides, male circumcision, treatment as prevention, and eventually prevention vaccines - has given rise to concerns about the ways in which users of these biomedical products may adjust their HIV risk behaviors based on the perception that they are prevented from infection. Known as risk compensation, this behavioral adjustment draws on the theory of "risk homeostasis," which has previously been applied to phenomena as diverse as Lyme disease vaccination, insurance mandates, and

It is a central principle in modern research ethics that the rights, safety, and well-being of research participants are the most important considerations. This should also apply to third-party individuals, that is, individuals who are not research participants but may be affected by the research. Despite this, international research ethics guidelines give limited attention to protecting third parties. This gap became evident when we were involved in the design and conduct of a series of clinical trials involving individuals with sexual interest in children, where children may constitute an id

The global clinical research ecosystem faced highs and lows during the COVID-19 pandemic. Key issues included research waste from poor-quality trials and fragmented regulatory and ethical reviews. Streamlining and harmonising these review processes is crucial for efficient, robust, and timely multinational trials, ensuring rigorous scientific standards, proper safety, and ethical oversight. Robust regulatory and ethics review systems thrive on continuous learning and efficient processes, crucial for high-quality research. Enhancing trial design and implementation, and guiding innovative approa

OBJECTIVE: Pregnant persons historically have been excluded from clinical trials. Recently, there has been a shift from exclusion toward inclusion of pregnant persons in research while acknowledging the complexity of reproductive ethics and the intertwined interests of the pregnant person and fetus. Our objective was to use a principle-based approach to review the ethics of clinical research concerning pregnancy-related disorders that predominantly affect fetal well-being.
STUDY DESIGN: Ethical principles are applied to the design of interventional trials in two rare conditions of pregnancy,

Currently, a prognostic biomarker-based model is being developed to predict the onset and disease progression of Huntington's Disease (HD) and Spinocerebellar Ataxia (SCA) types 1 and 3, both late-onset genetic neurodegenerative diseases lacking a disease-modifying treatment (DMT). The need for more accurate predictions of onset and disease progression arises in the context of clinical trials evaluating the effectiveness of potential DMTs and identifying the optimal time to initiate such a DMT. Moreover, such a prognostic model may provide mutation carriers with personal utility. The aim of th

The COVID-19 pandemic brought about safe distancing measures that are unprecedented. This article details the impact of the pandemic across research studies involving older persons in Singapore, and assesses the different strategies used to adapt to their needs in the context of evolving public health measures. The pandemic exposed diverse experiences of vulnerability among older persons, pushing for critical reflections on ethics of participation and social inclusion as the new research normal. We emphasize the importance of practicing flexibility: consideration for differentiated approaches

While compensation for research participation is common practice in community settings, the type and amount of compensation provided in carceral settings varies substantially. This is notable given the potential for compensation to unduly influence participation in research activities, particularly for vulnerable populations, such as people who are incarcerated. We aimed to conduct community-engaged research to better understand the potential impacts of different compensation approaches. We conducted interviews (N = 21) concerning ethical research approaches and compensation practices in carce

BACKGROUND: HIV phylogenetic analyses provide insights into genetic relatedness and transmission patterns but cannot directly identify specific transmission events or the direction of transmission. However, if not well understood and handled carefully, phylogenetic results have the potential to harm individuals and groups.
OBJECTIVES: The study aimed to explore the ethical considerations and the social context of HIV phylogenetic research.
METHOD: A qualitative methods study on phylogenetics was embedded within a clinical trial on community-based biosocial HIV prevention and sexual reproduct

Traditional research ethics governance was designed for bounded protocols, identifiable investigators, and temporally limited encounters with human participants. Data science health research (DSHR) disrupts that architecture because health data, biological materials, computational representations, and models persist, travel, combine, and acquire new uses across time. In this viewpoint, we use DSHR broadly to include research using large health datasets and adjacent secondary uses of health data, models, and biological materials, including learning health systems, public health surveillance, qu

As human microbiome research is globalizing, it raises ethical concerns regarding the European and North American dominance in the field, which may reproduce a colonial bias and perpetuate inequities in global health research and outcomes. We suggest disentangling this ethical quandary into three main concerns: 1) scientific bias toward European and North American populations; 2) limited meaningful community inclusion, participation, and ownership, and 3) scant significant inclusion of diverse global researchers. We then formulate three recommendations for their resolution, deploying co-labora

This paper aims to reflect on ethical considerations for the well-being of researchers involved in the development of virtual reality (VR) scenarios for parents who emotionally maltreat their children. Ethical considerations, particularly potential emotional strain, arose throughout the VR scenario development, including topic selection and content creation. However, despite the significance of these issues, existing literature addressing ethical challenges in researching sensitive topics from the researcher’s perspective has focused solely on data collection or analysis, overlooking the well-

Adapted physical activity has been critiqued for its lack of attention to assumptions underlying both research and practice. In response to these critiques, the purpose of this paper was to perform a secondary analysis of a recent systematic literature review to examine the axiological and ethical assumptions of physical activity intervention research involving adults with Down syndrome since 1990 in light of the possibilities of critical disability studies. Findings revealed the dominance of normative movement ideologies and a focus on physical activity performance. Participants were not affo

BACKGROUND: As Next Generation Sequencing technologies are increasingly implemented in biomedical research and (translational) care, the number of study participants and patients who ask for release of their genomic raw data is set to increase. This raises the question whether research participants and patients have a legal and moral right to receive their genomic raw data and, if so, how this right should be implemented into practice.
METHODS: In a first step we clarify some central concepts such as "raw data"; in a second step we sketch the international legal framework. The third step prov

Existing scholarship on the experiments performed in concentration camps beginning in 1942 on the value of sulfonamides in treatment of wound infections, in which inmates were used as experimental subjects, maintains that not only were the experiments ethically and legally completely reprehensible and unacceptable, but that they were also bad science in the sense that they were investigating questions that had already been resolved by valid medical research. In contrast to this, the paper argues on the basis of contemporary publications that the value of sulfonamides in the treatment of wound

This case involves the examination of an ethically problematic event that arose while we were conducting an outreach/early intervention research demonstration project targeted at women who exchange sex for drugs. Subjects were drug abusers--primarily crack-cocaine smokers--recruited by outreach workers on "the street" and in jail. Participants were offered counseling, HIV and syphilis screening, and risk-reduction education. A woman who was a jail inmate tested positive for HIV. In order to obtain medical and other assistance, she would have to reveal her serostatus to jail officials. This cou
