Endoscopic transnasal cauterization of the anterior ethmoidal artery.

CONCLUSIONS: In our experience, the technique described can be applied independently according to the specific patient's anatomy and disease, allowing simple, easy, and safe identification of the anterior ethmoidal artery (AEA). OBJECTIVES: The aim of this study was to provide the anatomic rationale for endoscopic cauterization of the AEA and to present our surgical approach to AEA in the treatment of severe nasal bleeding. METHODS: A retrospective study reviewed 300 endoscopic transnasal AEA cauterizations, over a 20-year period from 1991 to July 2010, at the Instituto Felippu, Sao Paolo, B

Congenital vomeral bone defect in two thalassemia trait cases.

The nasal septum is composed of a perpendicular plate of ethmoid bone, quadrilateral septal cartilage, membraneous septum, crest of palatine and maxillary bone and vomer. Defects of the nasal septum may be due to a variety of causes such as trauma, infection, inhalant irritants and neoplasia. To our knowledge, up until now, congenital defect of the vomer has been presented in six cases without any nasal symptoms. We present two cases of congenital defect of vomer with thalassemia trait, the patients complaining of nasal obstruction.

Delayed diagnosis and surgical repair of a chronic post-traumatic cerebrospinal fluid fistula with encephalocele in a child.

BACKGROUND: Encephalocele is a pathological condition characterized by the herniation of intracranial contents through a cranial bone defect. Although most cases are congenital, traumatic encephaloceles can occur following blunt head injury, surgical interventions, or other acquired causes. These lesions are clinically significant due to their association with cerebrospinal fluid (CSF) leakage and the risk of life-threatening meningitis. Traumatic encephaloceles in children are exceedingly rare, particularly those involving the fronto-ethmoidal region. CASE DESCRIPTION: We report the case of

[A comparative study between the results of antro-ethmoidal orbital decompression isolated and associated with orbital fat removal].

PURPOSE: To compare the results of isolated antral-ethmoidal orbital decompression and that associated with orbital fat removal in patients with dysthyroid orbitopathy. METHODS: Nineteen isolated antral-ethmoidal orbital decompressions were performed in a group of 12 patients (19 orbits) with the diagnosis of dysthyroid ophthalmopathy in the quiescent stage. The same procedure, but in association with orbital fat removal, were performed in another group of 8 patients (10 orbits) also with the diagnosis of dysthyroid ophthalmopathy in the quiescent stage. The results of both groups were compar

Orbitoethmoid aneurysmal bone cyst. Case report and literature review.

Aneurysmal bone cyst is a benign, vascular, variable growing and expansive lesion that can occur in any part of the skeletal system, but mainly in long bones and vertebrae. We present a case of orbitoethmoid aneurysmal bone cyst in a 62-year-old female presenting epiphora. Nasal endoscopy was normal. Sinus CTscan revealed an expansive mass in the right ethmoid sinus extending and destroying the right lamina papiracea. The tumor was completely resected through paralateral rhinotomy. Histological analysis showed fibrous septa containing multinucleated giant cells and bone tissue surrounding bloo

Paranasal sinus infection due to atypical mycobacteria in two patients with AIDS.

Atypical mycobacteria, which are common opportunistic pathogens in patients with AIDS, have not been previously implicated in the pathogenesis of paranasal sinus infections; we describe two such patients. Clinical and radiographic evidence of bilateral maxillary and ethmoid sinusitis was observed for one patient; his infection proved resistant to therapy with conventional antimicrobials and decongestants. Endoscopic ethmoid sinus biopsy yielded a specimen containing acid-fast bacilli (AFB) that were later identified as Mycobacterium kansasii. Antimycobacterial therapy had not resulted in ameli

The Bony Cartilaginous Unit.

To achieve an acceptable result in septorhinoplasty, sufficient septal cartilage is needed. There are many cases in which septal cartilage is insufficient, especially in revision surgery or in some races such as Asian. To obtain an ideal outcome, a bony cartilaginous unit (septal cartilage/ethmoid bone composite graft) is proposed as a versatile graft for different purpose such as caudal extension graft.

[Esthesioneuroblastoma of nasal cavity and ethmoid sinus].

This paper presents a rarely occurring tumor of nasal cavity and ethmoid sinus (esthesioneuroblastoma--2nd grade of the pathological grading system proposed by Hyams). It was observed in a woman aged 68. Moreover, there were described surgical therapy and repair of cerebral spinal fluid leaks using composite bone/mucosal graft of the lower turbinate as a donor material. The treatment was successful.

[Mucoceles of the maxillary sinus. Report of a case].

Mucoceles are retention cysts of the paranasal sinuses, specially developed on the anterior ethmoid and frontal sinuses and seldom seen on the maxillary cavity, as was the reported case. The authors underline the bone deformities sometimes seen and the difficulties when interpreting the meaning of x-ray records, both circumstances leading, from time to time, to the suspicion of a malignant tumor.

Giant cell reparative granuloma of the ethmoid sinus.

This article represents the fourth and fifth reported cases of GCRG that involve the ethmoid sinus. The problems encountered in establishing the diagnosis are emphasized, as are the features that differentiate it from the giant cell tumor, aneurysmal bone cyst, and hyperparathyroidism, the conditions with which it is most commonly confused. The unusually aggressive behavior, as demonstrated by these two cases, is highlighted.

Anatomical Study of Ethmoidal Foramina and Optic Canal in Endoscopic Trans-Nasal Approach to Anterior Skull Base.

BACKGROUND: Surgery to anterior skull base is challenging since complex structures are involved. Injury of olfactory fila and optic nerve may result in postoperative complications. In our study, the authors aim to use computer topographic image to provide a comprehensive anatomical information of anterior skull base and set up a new classification of optic canal (OC) in ethmoid sinus by the degree of bony defect, so as to help surgeons in endoscopic transnasal approach to anterior skull base. METHOD: Computer topographic angiography images of 112 individuals were reviewed, the measurement was

Minimal endoscopic approach to subperiosteal orbital abscess.

OBJECTIVE: To limit endoscopic abscess drainage to the opening of the ethmoid cells involved, adjacent to the subperiosteal orbital abscess. DESIGN: Prospective study. PATIENTS: Twenty children with subperiosteal orbital abscess complicating acute sinusitis. INTERVENTION: Endoscopic opening of the medial wall of the bulla ethmoidalis and of the lamina papyracea. RESULTS: The limited endoscopic approach allowed subperiosteal orbital abscess drainage in all patients with positive clinical outcomes. Extensive ethmoidectomy was not necessary to achieve drainage. CONCLUSIONS: Endoscopic subper

Tuberous sclerosis and fibrous dysplasia.

We present the case of an 11-year-old female patient with tuberous sclerosis who had a right nasal mass. CT examination revealed fibrous dysplasia involving the frontal, ethmoid, sphenoid, and vomer bones. Biopsy findings of the mass confirmed this diagnosis, and follow-up revealed marked expansion of these lesions. The authors emphasize the association of bone abnormalities and tuberous sclerosis and discuss the consideration of fibrous dysplasia as a component of this syndrome.

Skull Base Invasion Patterns of Malignant Head and Neck Tumors: A Neurosurgical Perspective.

Objective  Craniofacial resection (CFR) and temporal bone resection (TBR) on malignant head and neck tumors (MHNTs) invading skull base require accurate and precise determination of the tumor invasion. We investigated tumor skull base invasion patterns and surgical results in CFR and TBR cases. Methods  We performed either CFR or TBR for 75 selected patients with the possibility of en bloc resection over the period between 2011 and 2018. The medical charts of the selected patients were reviewed. Results  Primary tumor onset site (TOS) groups were: (1) nasal cavity/ethmoid sinus, 20 cases; (2)

A Case of Childhood-Onset Giant Cell Tumor that Caused Optic Nerve Atrophy in Both Eyes.

PURPOSE: The purpose of this study was to report the case of a female patient who had a giant cell tumor in the paranasal sinus during childhood, and while undergoing multiple resection surgeries experienced optic atrophy in both eyes. CASE PRESENTATION: This study involved a 35-year-old woman who was previously diagnosed with a giant cell tumor of the paranasal sinus bone at age 13. A CT scan revealed a large tumor extending from the sphenoid sinus to the ethmoid sinus. At age 14, a tumor resection was performed in conjunction with radiation therapy. However, after resection and radiation th

Patterns of facial fractures in children.

Morbidity and mortality among children is usually the result of trauma. Because a child's face is retruded relative to the protecting skull, has a thicker layer of adipose tissue, more elastic bones, flexible sutures lines, the presence of tooth buds within the jaws, and the lack of pneumatisation of the sinuses, the facial bones fracture less commonly than in adults. Our aim was to assess the patterns of such fractures in children who presented to the department of Oral and Maxillofacial Surgery, King Edward Medical University/Mayo Hospital Lahore, Pakistan. All 535 eligible children between

Chondroma in the Area of the Spheno-Ethmoid Junction.

Chondroma in the area of the spheno-ethmoidal junction is very rare. A 29-year-old male patient with chronic rhinosinusitis with nasal polyps was arranged for a preoperative computed tomography scan, and a lesion was accidentally found in his spheno-ethmoidal junction and involved the skull base. Combined with MRI, the lesion was misdiagnosed as fungal sinusitis. However, no fungal lesions were found during the operation, and cartilage tissue was confirmed only after some bone was ground away under the guidance of a surgical navigation system. Our case indicates that chondroma is easily misdia

Management of a Partially Treated Case of Chronic Osteomyelitis of the Hard Palate and Maxillary Sinus.

Osteomyelitis affects bones, including both cortex and medulla. It influences the mandible more frequently than the maxilla. Probable etiologic factors include foci of infection or trauma; however, the hematogenous spread of infection from a distant site is also a causative factor. Staphylococcus is the common organism involved in the causation. Clinical symptoms include signs of inflammation, pus drainage, fistulous or sinus tracts, wound disintegration, erythema, and raised local temperatures. Laboratory diagnosis with the evaluation of white blood cell count, rate of erythrocyte sedimentati
