Differential reinforcement using matched examples and nonexamples expedites the practice needed to learn concepts.

Conceptual learning refers to correctly identifying novel examples and nonexamples of a concept. Concepts can be taught by presenting stimuli and differentially reinforcing responses to examples but not nonexamples. However, it is unclear which stimuli should be used and whether specifically designed stimuli can result in correct responding even in the absence of differential reinforcement. Stimulus selection can be informed by identifying features that a stimulus must have to be an example and can have without altering whether the stimulus is an example or a nonexample. Developing and Piloting a Standardized Thermography Protocol to Expedite Rheumatology Referrals for Possible Inflammatory Arthritis.

OBJECTIVE: Inflammatory arthritis, especially rheumatoid arthritis (RA), imposes substantial morbidity that diminishes quality of life and escalates health care costs when left untreated. Timely diagnosis and treatment are pivotal. However, limited access to rheumatologists underscores the importance of triaging referrals based on symptom severity. We developed a protocol for using infrared thermography (IRT) to detect joint inflammation and implemented it in an academic primary care clinic as a screening tool for rheumatology referrals. MATERIALS AND METHODS: We enrolled people with RA, oste

Occurrence of Pharmaceuticals and Other Anthropogenic Compounds in the Wastewater Effluent of Arctic Expedition Cruise Ships.

Cruise ship traffic in polar regions is increasing, but we lack a good understanding of the emissions from these ships to sensitive marine environments. Wastewater discharges may result in the release of contaminants of emerging concern into such environments. Treated wastewater from three expedition cruise ships was collected and analyzed with a focus on pharmaceuticals, personal care products, and industrial chemicals. Samples were screened using data-dependent acquisition using liquid chromatography-high-resolution mass spectrometry (LC-HRMS). More than 160 compounds were identified at Schy

Beresford, Maxwell Oliver, Rhona Loxton, and George W. Rodway. Everest 1924: Observations and impressions from the Journal of Expedition Physician and Naturalist R.W.G. Hingston. High Alt Med Biol. 26:416-423, 2025.-The 1924 British expedition to Mount Everest was the second serious attempt to summit the mountain, following on from the expedition of 1922. Initially led by General Charles Bruce, the party was composed of some of the most prestigious climbers of its day. Major Richard W.G. Hingston, although not a mountaineer by trade, acted as medical officer on the expedition. Hingston maintai

Vibration tailored to jawbone density with near-infrared light expedites orthodontic tooth movement.

Malocclusion, a prevalent oral health issue, requires prolonged orthodontic treatment due to inefficiency of orthodontic tooth movement (OTM). Non-invasive physical stimulation has been increasingly applied to accelerate OTM. This study investigated how age-related variations in jawbone density affect the efficacy of vibration and near-infrared (NIR) light in accelerating OTM. Human computed tomography (CT) data and rat models revealed higher jawbone density in young adults than in adolescents. A ten-channel vibration-light integrated platform (VLIP) was developed to precisely control the stim

Bedside Aspiration for Workup of the Pediatric Septic Hip: Avoid Trips to the OR and Expedited Time to Diagnosis.

BACKGROUND: Septic arthritis of the hip is a pediatric orthopaedic emergency. Joint synovial fluid aspiration is a critical step of diagnosis, which may be delayed due to limited personnel or operating room (OR) availability. To expedite diagnosis, orthopaedic residents perform ultrasound-guided bedside arthrocentesis in the emergency department (ED). This study aims to evaluate the impact of this practice on time to diagnosis and definitive treatment of septic arthritis of the hip and minimizing trips to the operating room. METHODS: This is a retrospective study of patients presenting to our

Expedited Exome Reanalysis Following Deep Phenotyping and Muscle Biopsy in Suspected Mitochondrial Disorder.

BACKGROUND: Exome sequencing (ES) is a useful tool in diagnosing suspected mitochondrial disease but can miss pathogenic variants for several reasons. Additional testing, such as muscle biopsy or biochemical testing, can be helpful in exome-negative cases. METHODS: We report a patient who presented with repeated episodes of lactic acidosis and failure to thrive. RESULTS: ES and mitochondrial sequencing were initially negative but clinical suspicion for mitochondrial disease remained high. After muscle biopsy showed evidence of mitochondrial dysfunction, the ES was reanalyzed and revealed nov

Audit of a Revised Pathway Aimed at Expediting Diagnosis and Treatment for Suspected Achilles Tendon Rupture.

Treatment outcomes for Achilles tendon ruptures depend upon prompt diagnosis and management. A local study in 2018 highlighted inefficiencies in patient management, and a revised protocol was introduced allowing investigation and referral to be initiated by other healthcare professionals. This retrospective audit evaluates the impact of this on the timescale from presentation to treatment. It analyzes all suspected Achilles tendon ruptures within one District General Hospital from April 2021 to March 2022. Data regarding patient timelines was compared to the 2018 study. Over 12 months, 99 pati

Reducing Barriers for Expedited Partner Treatment in Adolescents and Young Adults: A Quality Improvement Initiative.

BACKGROUND: Adolescents and young adults (AYAs) face significant barriers to screening, testing, and treatment of sexually transmitted infections (STIs). Expedited partner therapy (EPT) streamlines partner treatment of STIs, but use among adolescents is low. We aimed to increase EPT offering and provision at 2 adolescent medicine clinics (AMCs) and the emergency department (ED) in an urban children's hospital. We addressed barriers at provider, pharmacy, and patient levels. We compared EPT offering and provision for chlamydia ( Chlamydia trachomatis [CT]) and trichomonas ( Trichomonas vaginali

Using the Electronic Health Record to Implement Expedited Partner Therapy in the Pediatric Emergency Department.

OBJECTIVES: Expedited partner therapy (EPT) is a partner treatment strategy for sexually transmitted infections (STIs) including gonorrhea and chlamydia as well as trichomoniasis in some states. The process allows healthcare providers to write prescriptions for STI treatment among partners of infected patients without a previous medical evaluation. The Centers for Disease Control (CDC) has recommended EPT as a useful option to facilitate partner treatment, particularly male partners of women with chlamydia or gonorrhea infections. Our institution implemented EPT in 2016 after Ohio legislation

Expedited brain cooling: Persistent temperature management from first aid to interhospital treatment.

Selective brain cooling is a promising technique for improving outcomes in ischemic stroke in the area of reperfusion. A recent study described the efficacy of a new method of selective brain cooling via active conductive head cooling. This is a major step forward in the administration of hypothermic treatment during pre-hospital transfer. However, to enhance the benefits of selective therapeutic cooling, a more comprehensive strategy preventing delay in hypothermic induction and increasing the accuracy of selectivity in the brain should be considered to mitigate the side effects related to th
