External transscleral posterior chamber lens fixation.

To avoid the complications associated with anterior chamber intraocular lenses and iris-fixated posterior chamber lenses when challenged with inadequate capsular or zonular support, we have developed a new method of measured external transscleral suture fixation of posterior chamber intraocular lenses within the ciliary sulcus. This technique differs from most current techniques of transscleral fixation, which involve passing needles through the corneoscleral wound into the ciliary sulcus without direct visualization and out through the sclera. Our technique offers a more predictable placement

External fixation of Colles' fractures. An anatomical study.

In a prospective, controlled study 58 patients aged under 60 years with Colles' fractures were treated either by a forearm plaster or by the application of an external fixator. In 94% of those treated by a fixator it was possible to insert the distal pins of the frame into the fracture fragment, the fixation obtained being sufficient to forgo additional splintage. The external fixator proved more effective at holding the manipulated position, and the radiological loss of position during fracture union was minimal compared with that seen in patients treated in plaster.

Rupture of the calcaneal tendon. Treatment by external fixation.

Thirty-three consecutive patients with complete ruptures of the calcaneal tendon were treated by external fixation. The patients were assessed both objectively and subjectively, and the results classified as excellent, good, fair and poor. In 30 patients the result was excellent or good. There were no infections or re-ruptures. Two patients with fair results had sural nerve injury. The only patient with a poor result had Sudeck's atrophy. It is felt that this operation satisfies the need for a new technique which is simple and combines the advantages of both surgical and non-surgical treatment

External fixation of complex hand and wrist fractures.

From 1977 to 1980 fixation with an external fixateur was performed in 33 injuries of the upper extremity: ten fractures of the hand, 22 Colles' fractures, and one osteotomy for Madelung's deformity. Application was primary in 19 patients and after another type of fixation (cast or Kirschner wire) had failed to hold reduction in 14 patients. All ten hand fractures healed in 4 to 12 weeks, and the distal radius fractures united in 6 to 10 weeks. The external fixateur for unstable fractures of the hand and wrist with bone or soft-tissue loss provides rigid skeletal stabilization, accessibility fo

Alternative indications for external fixation according to Wagner.

Wagner's technique of external fixation was originally developed for limb-lengthening procedures. With its one-dimensional configuration and great stability, however, it is also highly suitable for application to the upper extremity and thigh for other purposes and several alternative applications are described. The more important indications are infected defect-pseudarthroses of the femur, temporary immobilization of the knee-joint, and 'unmanageable' fractures of the distal radius.

Ankle fusion with the Hoffmann external fixation device.

For over 100 years, many different types of external fixation have been used to immobilize fracture fragments. The greatest acceptance of external fixation has been with open fractures of the tibia. Three ankle fusions, all complex cases, were performed using the double-framed Hoffmann external fixation device. The deformities treated were a severe post-traumatic equinus deformity of the ankle and forefoot, a painful nonunion of a previous ankle fusion, and an equinus deformity secondary to a transmetatarsal amputation.

Biomechanical Comparison between Volar Plate Fixator and Nonbridge External Wrist Fixator.

Intra-articular distal radius fractures are difficult to reduce and maintain by nonoperative means. ORIF leaves implants in the patient long after the fracture is healed. External fixation can stabilize the reduced fracture and leaves no long-term implants. The nonbridging fixator (NBX) will provide better reduction and comparable rigidity of fixation to a volar plate for a 5-fragment, OTA 23 C3.2 distal radius fracture. A 5-part distal radius fracture was created in 5 pairs of cadaver arms. One arm was randomly fixed with the NBX fixator; the matched pair was fixed with a volar plate (VPS). F

The Anterior Subcutaneous Pelvic Ring Fixator: No Biomechanical Advantages Compared with External Fixation.

BACKGROUND: Subcutaneous fixation of the anterior pelvic ring is an emerging surgical technique for trauma patients. The aim of this study was to biomechanically evaluate 2 internal fixation devices for stabilizing a disrupted pelvic ring and compare them with traditional external fixation. METHODS: Thirty-six synthetic pelvises with a simulated unstable ring fracture (anteroposterior compression type III) were divided into 3 groups. Group A underwent fixation with a supra-acetabular external fixator; group B, with an internal fixator using the USS II polyaxial system; and group C, with an in

New Fixation Methods for the Treatment of the Diabetic Foot: Beaming, External Fixation, and Beyond.

Surgical correction of complex foot and ankle deformities secondary to Charcot neuroarthropathy remains a significant surgical challenge. New technological advancements in hardware have allowed for the use of augmented fixation techniques in midfoot deformity correction, including the use of indication-specific locking plates and beaming techniques that offer enhanced stability. Severe hindfoot deformity management can employ the use of internal fixation, including intramedullary hindfoot nails and circular external fixation frames for limb salvage.

Fixation of extra articular distal radial fractures with non bridging external fixator.

Severely comminuted fractures of distal radius often lead to impairment of wrist function. A prospective study was performed on 30 patients aged between 35-65 years of extra-articular fractures of distal radius. All patients were treated with closed manipulation of the fracture an application of non-bridging (radio radial) external fixator under radiological control. At six months after the injury functional results were good with near normal movements of the wrist and hand. Radiological fracture alignment was satisfactory with no significant shortening of the radius. There were no re-displace

Rigid versus semirigid fixation for condylar fracture: experience with the external fixation system.

PURPOSE: Although mandibular condylar fractures are very common, the treatment remains controversial. Many techniques of reduction and many devices have been suggested. The purpose of this study was to evaluate the advantages and drawbacks of semirigid fixation compared with rigid fixation using an external fixation system. PATIENTS AND METHODS: Between 1990 and 2005, 137 patients (83 males, 54 females; median age, 24.2 years; 116 with monocondylar fracture and 21 with bicondylar fracture) were treated with an external fixation system (Mand-X-Fix, Leibinger, Germany). In these cases, the dist

Evaluation of Physical and Mental Health in Adults Who Underwent Limb-Lengthening Procedures with Circular External Fixators During Childhood or Adolescence.

BACKGROUND: Lower limb length discrepancy (LLD) in children and adolescents, often due to congenital or acquired conditions, is treated to achieve limb equality and alignment, optimizing function and minimizing cosmetic concerns for an active adulthood. This study evaluated the Health-Related Quality of Life (HRQoL) and physical functioning of adults who underwent unilateral limb lengthening with circular external fixators (EFs) in childhood. METHODS: Fifty patients treated at a median age of 14.9 years completed the Short Form 36 (SF-36) and Stanmore Limb Reconstruction Score (SLRS) question…

The use of circular external fixators in the management of lower limb trauma in Dublin: a single surgeon's 20-year experience.

INTRODUCTION: It has been estimated that approximately 520,000 injury presentations are made to Irish accident and emergency departments each year. Fractures account for 20 % of these injuries. Circular external fixators (frames) have been shown to be a safe and effective method of treatment for long bone fractures where internal fixation is impossible or in-advisable. We present the outcomes of all frames applied at our institution for stabilisation of acute fractures over a 20-year period. METHODS AND METHODS: We retrospectively reviewed a prospectively compiled database of all frames appli…

Spinopelvic fixation with retention of external fixation in a lateral position for unstable pelvic fracture.

Anterior external fixation (EF), as the primary treatment for unstable pelvic fractures, is performed with patients in the supine position. In most cases, however, definitive surgery for posterior fixation is performed first in the prone position without EF. We report the case of a patient with unilateral and vertically unstable pelvic fracture whom we had treated with minimally invasive spinopelvic fixation, with retention of the anterior EF in a lateral position. Reduction of the residual displacement was performed with percutaneous spinal instrumentation, and acceptable reduction was achiev

High-energy tibial plateau fractures: external fixation versus plate fixation.

OBJECTIVE: Whether external or plating fixation is more appropriate for high-energy tibial plateau fractures is still being disputed, our aim was to test the hypothesis whether external fixation can provide a fair outcome with fewer complications, when compared to the results with previously reported data of plating fixation for high-energy tibial plateau fractures. METHODS: An Ovid of Medline, Embase, and Cochrane Library search was conducted for the relevant English orthopedic journals, and eligible studies, including twenty-four case series and one comparative study containing 885 patients

Tibial lengthening: technique for speedy lengthening by external fixation and secondary internal fixation.

BACKGROUND: The conventional Ilizarov technique for tibial lengthening is frequently time-consuming, causes suffering, and is associated with many complications. This study takes a retrospective approach to investigate the outcome of applying a slightly speedier procedure using an Ilizarov apparatus and secondary internal fixation. METHODS: Twelve adult patients displaying tibial shortening (median, 4.0 cm; mean, 4.6 cm; range, 3.0-12.0 cm) with various causes were treated with or without tibial osteotomy and stabilized using an Ilizarov lengthening apparatus. Postoperatively, lengthening of
