Outcomes of Anti-Vascular Endothelial Growth Factor Treatment for Choroidal Neovascularization in Fellow Eyes of Previously Treated Patients With Neovascular Age-Related Macular Degeneration.

IMPORTANCE: Neovascular age-related macular degeneration (nvAMD) is a leading cause of vision loss. The optimal screening protocol to detect choroidal neovascularization (CNV) in fellow eyes of patients undergoing treatment for unilateral CNV has not been determined. OBJECTIVE: To compare the visual outcomes of eyes with established, active nvAMD in index eyes with outcomes of fellow eyes that subsequently developed CNV during the management protocol. DESIGN, SETTING, AND PARTICIPANTS: In this retrospective single-center case series conducted at a private vitreoretinal practice, data were co

Orexin-A and BDNF in the hippocampus of middle-aged rats: beneficial effects of integrating voluntary physical activity with intermittent fasting.

Aging is associated with cognitive decline, reduced hippocampal function, and decreased levels of brain-derived neurotrophic factor (BDNF) and orexin A -key regulators of neuroplasticity and energy balance. This study examined the chronic effects of intermittent fasting (IF), both alone and in combination with voluntary physical activity, on hippocampal orexin A, orexin receptor 1, and BDNF levels in middle-aged male Wistar rats. Over eight weeks, rats were divided into control (Con), IF, and IF with voluntary wheel running (IF+VA) groups. Behavioral assessments (open field, rotarod), biochemi

From Venom to Vein: Factor VII Activation as a Major Pathophysiological Target for Procoagulant Australian Elapid Snake Venoms.

Australian elapid snake venoms are uniquely procoagulant, utilizing blood clotting enzyme Factor Xa (FXa) as a toxin, which evolved as a basal trait in this clade. The subsequent recruitment of Factor Va (FVa) as a toxin occurred in the last common ancestor of taipans (Oxyuranus species) and brown snakes (Pseudonaja species). Factor II (prothrombin) activation has been stated as the primary mechanism for the lethal coagulopathy, but this hypothesis has never been tested. The additional activation of Factor VII (FVII) by Oxyuranus/Pseudonaja venoms has historically been considered as a minor, u…

So-called "central retinal vein occlusion". II. Venous stasis retinopathy.

28 patients (29 eyes) with venous stasis retinopathy (VSR) were studied. This study indicates that VSR is a self-limited, chronic and comparatively benign condition as compared to hemorrhagic retinopathy. No patient with VSR progressed to hemorrhagic retinopathy. The main complication which required management in VSR was the deterioration of central visual acuity (VA) due to development of macular edema which, if untreated, ended in cystoid macular degeneration and permanent central scotoma. Thus the indication for treatment in these cases was the fall of central VA. Ten eyes showed no deterio

Extracellular Vesicles Are Associated With Outcome in Veno-Arterial Extracorporeal Membrane Oxygenation and Myocardial Infarction.

Background: Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is being increasingly applied in patients with circulatory failure, but mortality remains high. An inflammatory response syndrome initiated by activation of blood components in the extracorporeal circuit may be an important contributing factor. Patients with ST-elevation myocardial infarction (STEMI) may also experience a systemic inflammatory response syndrome and are at risk of developing cardiogenic shock and cardiac arrest, both indications for VA-ECMO. Extracellular vesicles (EV) are released by activated cells as med

Hospital frailty risk score and 90-day mortality in patients receiving venoarterial extracorporeal membrane oxygenation.

BackgroundFrailty is increasingly recognized as a key determinant of outcomes in critically ill patients, yet its utility among patients receiving venoarterial extracorporeal membrane oxygenation (VA ECMO) remains unclear. We evaluated the hospital frailty risk score (HFRS) as a risk factor of 90-day mortality in VA ECMO patients.MethodsWe conducted a retrospective analysis of VA ECMO patients at Johns Hopkins Hospital from 2016 to 2024. HFRS was derived from ICD-10 codes. An optimal frailty cut point was determined using maximally selected rank statistics, and patients were subsequently strat

Application of intraoperative infrared thermography in bypass surgery for adult moyamoya syndrome: A preliminary study.

BACKGROUND AND OBJECTIVES: Cerebral revascularization surgery is the mainstay of treatment for moyamoya syndrome (MMS) today, and intraoperative determination of the patency of the revascularized vessel is a critical factor in the success of the procedure. Currently, major imaging modalities include intraoperative indocyanine green (ICG) videoangiography (ICG-VA), digital subtraction angiography (DSA), and vascular ultrasound Doppler. Infrared thermography is a modern imaging modality with non-contact devices for the acquisition and analysis of thermal data. We aimed to investigate the feasibi

Bioaccumulated provitamin A in black soldier fly larvae is bioavailable and capable of improving vitamin A status of gerbils.

The aim was to study whether provitamin A (proVA), which can bioaccumulate in black soldier fly larvae (BSFL), is bioavailable and can restore VA status in mammals. A model for studying the metabolism of this vitamin, the gerbil, was either fed a standard diet (C+ group), a diet without VA (C-), a diet in which VA was provided by β-carotene (β-C) from sweet potatoes (SP), or a diet in which VA was provided by β-C from BSFL that had been fed sweet potatoes (BSFL). The animals were killed at the end of the supplementation period and β-C, retinol and retinyl esters were measured in plasma and liv

The Fate of Hemodialysis Vascular Access After Kidney Transplantation: Why Ligation Should Not Be Systematically Performed?

BACKGROUND: After renal transplant, the haemodialysis (HD) vascular access (VA) is often maintained in the event of graft failure but evidence to recommend the systematic ligation/maintenance are lacking. We investigate the effects of VA preservation/ligation in successfully transplanted patients. METHODS: A retrospective analysis of all patients undergoing successful renal transplantation after HD were conducted. Patients with a functioning VA, those undergoing ligation or presenting with post-transplantation spontaneous thrombosis were included. Demographics, comorbidities, reasons for liga

Acquired von Willebrand syndrome in patients treated with veno-arterial extracorporeal membrane oxygenation.

Veno-arterial extracorporeal membrane oxygenation (VA ECMO) is a powerful device for treatment of patients with life-threatening heart failure. Although bleeding is often associated with VA ECMO and sometimes results in a fatal outcome, its precise causes remain unknown. On the other hand, excessive high shear stress in the cardiovascular system causes acquired von Willebrand syndrome (aVWS), characterized by loss of von Willebrand factor (vWF) large multimers. vWF large multimers of five consecutive patients treated with VA ECMO were quantitatively evaluated using the vWF large multimer indic
