Surgical outcomes of styloidectomy, internal jugular vein fasciotomy, and C1 transverse process resection for venopathic intracranial hypertension.

OBJECTIVE: The role of extracranial venous compression of the internal jugular vein (IJV) in patients who have symptoms consistent with intracranial hypertension is a recently described phenomenon. If medical treatment fails to control symptoms, several surgical techniques have been described to address the IJV compression that typically occurs, as the IJV passes anterior to the C1 transverse process. Techniques include styloidectomy, IJV fasciotomy, IJV repositioning, C1 transverse process resection, and IJV stenting, in isolation or in various combinations. The aim of this study was to evalu…

Mini approaches fasciotomy combined with vacuum sealing drainage for acute compartment syndrome caused by fractures in children.

PURPOSE: Acute compartment syndrome (ACS) is an urgent, critical condition that requires immediate fasciotomy once diagnosed. Traditionally, fasciotomy of the forearms and lower leg involves one or two long approaches. Our previous study demonstrated that mini approaches fasciotomy was an effective method to treat ACS. This study is aimed at further evaluating the limb functions and complications of mini approaches combined with vacuum sealing drainage (VSD) for treating ACS caused by fractures in the forearms and lower legs. METHODS: This was a retrospective cross-sectional study, and after …

Lower Leg Lateral Chronic Exertional Compartment Syndrome: Prospective Surgical Treatment Outcomes for Isolated or Combined Lateral Fasciotomy.

BACKGROUND: Chronic exertional compartment syndrome involving the lower leg lateral compartment (lat-CECS) seldom occurs isolated but is usually combined with CECS of the anterior (ant-CECS) or deep posterior compartment (dp-CECS). Patient characteristics in lat-CECS and outcome after surgery are largely unknown. The aim of this prospective case series was to describe patient characteristics and symptoms and to report on outcome following a fasciotomy. METHODS: All patients diagnosed with lat-CECS based on exertional lateral lower leg symptoms and elevated intracompartmental pressure (ICP) me…

Forearm compartment syndrome due to acquired hemophilia that required massive blood transfusions after fasciotomy: A case report.

BACKGROUND: Acquired hemophilia is rare. In some cases, the bleeding in muscle causes compartment syndrome. However, it is not clear whether fasciotomy should be performed for the compartment syndrome caused by acquired hemophilia because of the risk of bleeding and the unknown functional results. CASE SUMMARY: A 75-year-old woman was admitted with severe pain of the right forearm with no preceding traumatic event. The right forearm was obviously swollen, and stretch pain was observed. Subcutaneous hematomas were suspected in various parts of the body. Compartment pressure was 110 mmHg on the…

Injectable Collagenase Versus Percutaneous Needle Fasciotomy for Dupuytren Contracture in Proximal Interphalangeal Joints: A Randomized Controlled Trial.

PURPOSE: Collagenase Clostridium histolyticum (CCH) injection was introduced commercially as a treatment for Dupuytren contracture following initial phase-3 investigations in 2009 with promising results. However, the efficacy of CCH has not been prospectively investigated in a direct comparison to other active treatments of Dupuytren contracture with more than 1-year follow-up, despite a wide and increasing clinical use. METHOD: In this prospective, independent, open-label, randomized controlled trial, (Clinicaltrials.gov; NCT 01538017), percutaneous needle fasciotomy (PNF) was directly compa…

Delayed Primary Closure of Fasciotomy Incisions in the Lower Leg: Do We Need to Change Our Strategy?

OBJECTIVES: The primary purpose of this study is to determine whether a strategy of bringing patients back to the operating room for successive debridements allows for the eventual delayed primary closure (DPC) of fasciotomy wounds. DESIGN: Retrospective cohort study. Data were collected from medical records and radiographs. SETTING: Two urban level 1 trauma centers. PATIENTS: One hundred four adult patients with acute compartment syndrome in the setting of a tibia fracture (open or closed). INTERVENTION: All patients underwent decompressive fasciotomies with closure by either DPC or split…

Virtual reality as a non-pharmacologic analgesic for fasciotomy wound infections in acute compartment syndrome: a case report.

BACKGROUND: Fasciotomy is a life-saving procedure to treat acute compartment syndrome, a surgical emergency. As fasciotomy dramatically improves wound pain, it should be performed as soon as possible. Moreover, delays in the use of fasciotomy can increase the rate of wound infections. Once the fasciotomy wound is infected, pain control is achieved via the long-term use of opioids or anti-inflammatory analgesics. However, the administration of high doses of opioids may cause complications, such as respiratory depression, over-sedation, and constipation. Therefore, treatment methods other than n…

Treatment of elephantiasis of the leg by fasciotomy and lymphangiectomy with skin preservation in a resource-limited setting: a case report and review.

The author reports the surgical management of a patient with elephantiasis of the leg in the Democratic Republic of Congo. A fasciotomy and lymphangiectomy with skin preservation, combined with compression therapy, resulted in significant cosmetic, functional, and social improvement. Although challenging in a resource-limited setting, development of surgical management may make it possible to reduce beliefs that elephantiasis is incurable or due to witchcraft and may reduce time to consultation.

Impact of clinical and sociodemographic variables in development of acute compartment syndrome requiring fasciotomy after tibia fractures.

BACKGROUND: Factors like age, sex, high-energy trauma, and complex fractures are factors for acute compartment syndrome (ACS), but the influence of sociodemographic factors remains unexamined. The goal was to assess whether sociodemographic factors were independently associated with ACS requiring fasciotomy after tibia fracture when accounting for available clinical and injury-related factors. METHODS: This is a retrospective study of patients diagnosed with tibia fractures between 2016 and 2021 using the Nationwide Inpatient Sample (NIS), a 20% weighted sample of 95% of the U.S. inpatient po…

Percutaneous Needle Fasciotomy Versus Limited Fasciectomy for Dupuytren Disease: A Linear Model Assessment of Short-term Efficacy.

BACKGROUND: There is no consensus about the best treatment for Dupuytren contracture in the hand. In particular, whether to use a percutaneous needle fasciotomy (PCNF) in preference to a limited fasciectomy (LF). METHODS: We performed a retrospective review of the outcomes of 74 joints treated with either PCNF or LF. Baseline characteristics, complications, and active extension deficit (AED) were assessed at 3 weeks and 3 months posttreatment. Reoperative procedures were analyzed to assess the effectiveness of repeated procedures. RESULTS: Our results suggest that there is no significant dif…
