This case report documents the clinical, radiographic, and histologic findings in a 69-year-old obese man, who had subchondral insufficiency fracture both in the femoral head and medial femoral condyle. On plain radiographs, both lesions underwent subchondral collapse. Magnetic resonance images of the left hip showed a bone marrow edema pattern with associated low-intensity band on T1-weighted images, which was convex to the articular surface. The histopathologic findings in the hip and knee were characterized by the presence of a subchondral fracture with associated callus and granulation tis

This article describes the case of a 69-year-old patient with an occult acetabular fracture complicated by an ipsilateral femoral neck fracture occurring within 2 months. The acetabular fracture remained undiagnosed at examination due to insufficient clinical and radiographic data interpretation. The patient was assured of early mobilization that led to a fall and subsequent hip fracture. We focus on the potential reasons for the nondiagnosis of the acetabular fracture. Acetabular fractures in the elderly may occur after low-energy injuries. The lack of history of violent injury may lead to an

This is a report of treatment, complications and results of 209 fractures of the femoral shaft: 114 compressions plates, 63 medullary nails, four fixateur externe, 28 vertical tractions after Weber. The most frequent complication after compression plate was the delayed fracture healing (5.9%) and the malrotation after medullary nails (8%). 2% of the cases developed an osteitis, no pseudoarthrosis was observed. All complications could be successfully treated by second operation. 92.8% of the long-term results could be judged as good to very good. The fixateur externe was rarely used, the vertic

In general, all traumatic dislocations of the hip must be treated as surgical emergencies. Multiple attempts at closed reduction are contraindicated, particularly in Type V dislocations. Every effort must be made to recognize the dislocation, particularly in patients with other severe lower extremity trauma. Reduction within 24 hours gives better results than late reductions. Roentgenograms of the pelvis must include both hips after closed or open procedures as a check for a concentric reduction of the hip. Any abnormality, or failure to reduce the avulsed head fragment, demands an immediate h

INTRODUCTION: The skeleton is a common site for metastases. Prostate, breast, lung, renal and thyroid carcinomas account for 80 % of the original cancers, with the femur being the most affected long bone. With improved oncological treatments, prolonged patient survival leads to an increased prevalence of osseous metastases. This study examines the impact of preventive surgery for impending femoral pathological fracture (IFF), versus treatment of pathological femur fracture (PFF) on patient mortality and morbidity.
METHODS: Retrospective cohort of 174 patients undergoing surgery due to femoral

PURPOSE: We propose coronal shear fracture of the femoral neck (CSFF) as a new type of fracture that differs from a basicervical fracture. This study aimed to present the incidence of CSFF and compare its clinical characteristics and outcomes with those of basicervical fractures.
METHODS: In this multicenter retrospective cohort study, 2207 patients with hip fractures were identified using computed tomography (CT), 17 and 27 patients were diagnosed with CSFF (CSFF group) and basicervical fractures (basicervical fracture group), respectively. The primary outcome was reoperation, while the seco

Talar fractures associated with ipsilateral femoral shaft fractures have been occasionally reported, but an unexpectedly high potential for simultaneous occurrence of the fractures in head-on collisions has not been well documented. We treated three patients with ipsilateral femoral shaft and talar fractures in a 2-year period starting in 2000. The patients were male, aged 25, 34 and 40 years, polytraumatized with other injuries. In all cases, the fractures occurred in the right lower extremities of drivers involved in head-on collisions. The talar fractures were not detected at the initial di

Unstable intertrochanteric and subtrochanteric fractures in elderly patients are still a challenge in orthopedic surgery. The results of the present study show the importance of a rigid fixation to allow early mobilization and immediate weight bearing. The gamma nail has clearly brought an improvement to the treatment of such fractures, although there are complications associated to this implant which need to be reduced by further developments of the implant itself, the instruments, and/or the operative technique.

INTRODUCTION: the objective of this study was to analyze the incidence and predisposing factors in subtrochanteric fractures on hips previously treated with cannulated screws after suffering a subcapital fracture, based on eight cases registered in the last 20 years.
MATERIAL AND METHODS: this is a retrospective observational study on a series of patients with a diagnosis of subtrochanteric hip fracture, who have been treated in recent months for a subcapital fracture of the same hip by osteosynthesis with cannulated screws. The study period was 20 years (2000-2020).
RESULTS: of the eight ca

Secondary hip fractures (SHFs) rarely occur after intramedullary nailing (IMN) fixation without femoral neck fixation for atypical femoral fractures (AFFs). We report three cases of older Japanese women who sustained SHFs presumably caused by osteoporosis and peri-implant stress concentration around the femoral neck after undergoing IMN without femoral neck fixation for AFF. All cases were fixed with malalignment. In AFF patients, postoperative changes due to postoperative femoral bone malalignment may affect the peri-implant mechanical environment around the femoral neck, which can result in

OBJECTIVES: The goal of this study is to establish radiographic features and characteristics of patient injury in cases of femoral shaft fractures that predict the presence of ipsilateral femoral neck fractures (IFNFs).
METHODS: Patient data was retrospectively assessed from a single level I trauma center through the electronic health record using (Current Procedural Terminology) CPT codes for both isolated and combined ipsilateral femoral shaft and neck fractures. Demographic information, injury characteristics, and independently reviewed radiographic features were collected and compared aga

INTRODUCTION: Failure of proximal femoral fracture managed by proximal femoral nail (PFN) leads to a very difficult situation to handle with conventional techniques, and reversed distal femoral locking compression plate (DF-LCP) is of great benefit in these selective cases.
METHODS: Twelve patients with ununited proximal femoral fractures including subtrochanteric fractures with a failed PFN implant were included in the study. All patients with periprosthetic fractures and fractures treated by implants other than PFN were excluded from this study.
RESULT: All fractures went into union in an

INTRODUCTION: Anterior femoral notching during TKA is thought to weaken the distal femur, potentially increasing periprosthetic fracture risk, but its clinical impact remains debated. This study examines whether notching independently predicts such fractures and assesses related clinical outcomes using the Tayside classification system for cortical violations.
METHODS: A retrospective cohort study was conducted on 715 patients who underwent primary TKA. Patients were divided into Group 1 without notching (control group) and Group 2 with any presence of notching according to Tayside classifica

Subtrochanteric femoral fractures are one of the common fractures encountered in today's Orthopaedic practice. This area consists of mostly cortical bone with high stress generation thus heal slowly which leads implant failure. The inherent instability of this fracture and forces of the muscles with comminuted medial calcar is giving the fracture a tendency to varus collapse. Intramedullary nails are not suitable for short proximal segment and wide medullary canal while extramedullary implants are associated with higher rate of implant failure. Recently proximal femoral locking compression pla

Proximal femoral fractures, especially in elderly persons with osteoporosis, present a challenge for the traumatologist. While the dynamic hip screw (DHS) became the implant of choice for the treatment of stable fractures, the ideal implant for the treatment of unstable fractures remains an issue. In our experience, Proximal Femoral Nail Antirotation (PFNA) is an excellent device for osteosynthesis as it can be easily inserted, it provides angular and rotational stability and allows early weight bearing on the affected limb. Between February 2007 and August 2009, 76 patients underwent the PFNA

BACKGROUND: Diaphyseal and distal femoral fractures are painful injuries that are frequently seen in patients requiring a trauma work-up in the hospital emergency department prior to definitive management. The purpose of this study was to determine whether a femoral nerve block administered in the emergency department could provide better pain relief for patients with femoral fractures than currently used pain management practices.
METHODS: Patients who presented with an acute diaphyseal or distal femoral fracture were identified as potential candidates for this study. Eligible patients were

BACKGROUND: This biomechanical study was designed to evaluate the predictive ability of dual-energy x-ray absorptiometry, cortical bone geometry as determined with computed tomography, and radiography in the assessment of torsional load to failure in femora with and without notching.
METHODS: Thirteen matched pairs of cadaveric femora were randomized into two groups: a notched group, which consisted of femora with a 3-mm anterior cortical defect, and an unnotched group of controls. Each pair then underwent torsional load to failure. The ability of a number of measures to predict femoral torsi

BACKGROUND: While periprosthetic femoral fracture (PFF) is not the most common reason for revision of a primary total hip arthroplasty (THA), it represents a common problem in the acute postoperative period. During this time, undergoing revision surgery has been found to have increased risks of complications. The goal of this study was to evaluate the outcome of patients who experienced an isolated femoral revision for a PFF acutely after primary THA.
METHODS: We performed a retrospective chart review of patients from 2004 to 2022 using an institutional database. We included patients who unde
