The effects of prolonged exposure of rats to sweetened caloric (sucrose) and non-caloric (saccharin) solutions on subsequent sensitivity to the anorectic effects of naloxone and d-fenfluramine were investigated in a series of experiments. In Experiment 1, rats given 18 days exposure to 10% sucrose showed greater sensitivity to the anorectic effects of naloxone (0.125-1.0 mg/kg, IP) in a separate feeding test, than did controls or rats exposed to 0.2% saccharin. This effect was replicated in Experiment 2, and here rats exposed to a palatable quinine-sucrose solution that was less preferred than…

The combined administration of phentermine and fenfluramine (PHEN/FEN) has been used as a treatment for obesity. Recent evidence suggests that this drug mixture may also be an effective medication for substance abuse disorders, including cocaine dependence. It is well-established that repeated high-dose fenfluramine causes serotonin (5-HT) terminal degeneration in laboratory animals, and no studies have addressed possible interactions between phentermine and fenfluramine. The purpose of the present work was to examine the effect of phentermine coadministration on fenfluramine-induced depletion…

Physical and mental effects of a single oral dose of ephedrine (ephedrine HCl 30 or 40 mg), fenfluramine (fenfluramine HCl 15 or 20 mg), phentermine (7.5 or 11.25 mg) and prolintane (prolintane HCl 10 or 20 mg) were compared in a double-blind placebo-controlled study. Each group consisted of 16-43 healthy volunteer medical students. The subjects fasted for at least 3 hr before drug administration and further until the end of the experiment. All the parameters were measured just prior to giving the drug, and 1.5 hr and 2.5 hr afterwards. Ephedrine significantly increased systolic blood pressure…

BACKGROUND: Fenfluramine is an antiseizure medication approved by the Food and Drug Administration for the treatment of Dravet syndrome in patients older than 2 years. Fenfluramine is an amphetamine derivative. It cross-reacts with amphetamines in urine drug screen immunoassays.
PATIENT PRESENTATION: A 2-year-old patient with Dravet syndrome had a cardiorespiratory arrest and tested positive for amphetamines in a urine drug screen, raising concerns of child abuse. Liquid chromatography-mass spectrometry confirmed the presence of fenfluramine but did not detect amphetamines.
CONCLUSION: Fenfl…

We report the case of a patient referred to us for mitral and aortic valvular disease with a rheumatic appearance. The unusual macroscopic appearance on valve resection was not compatible with a rheumatic cause. A detailed review of this patient's clinical history (ie, a history of treatment with fenfluramine) suggested an iatrogenic cause, which was confirmed by histology. For the first time, a case of valvular heart disease that deteriorated was discovered 7 years after treatment with fenfluramine, whereas this iatrogenic disease classically resolves after discontinuation of treatment. This …

CONTEXT: Few published studies of the pathology of fenfluramine-phentermine (fen-phen) valvulopathy, described by Connolly and colleagues in 1997, have appeared.
OBJECTIVES: To define temporal changes in the morphology of the stuck-on plaques and to determine whether the plaques progress or regress after cessation of fen-phen.
METHODS: The available clinical information and pathology material from 35 aortic valves (AVs) and 43 mitral valves (MVs) from 64 patients were reviewed.
RESULTS: The valves fell into 3 groups: 17 AVs and 28 MVs had fen-phen lesions only, 2 AVs and 7 MVs had fen-phen …

BACKGROUND: Coadministration of phentermine and fenfluramine (phen/fen) effectively treats obesity and possibly addictive disorders. The association of fenfluramine and certain other anorexic agents with serious side effects, such as cardiac valvulopathy and primary pulmonary hypertension (PPH), limits the clinical utility of these drugs. Development of new medications that produce neurochemical effects like phen/fen without causing unwanted side effects would be a significant therapeutic breakthrough.
METHODS AND RESULTS: We tested the hypothesis that fenfluramine (and other anorexic agents)…

Fenfluramine hydrochloride and its tablets were determined with high performance liquid chromatography by using an analytical column (25 cm x 0.46 cm i.d.) packed with Hypersil BDS C18 (5 microns), a UV detector at 264 nm and V(acetonitrile): V(0.4 mol/L ammonium acetate): V(triethylamine) = 30:70:2 as eluent. The quantitative determination was performed with caffeine as the internal standard. The internal standard method showed good linearity in the mass concentration range of 0.1-0.5 g/L with r = 0.9999. The average recovery of fenfluramine tablets was 99.98%. This method is simple, rapid, s…

To investigate the satiety defect of hyperphagic genetically obese (ob/ob) mice, acute feeding responses to three differently acting anorectic agents were determined in 7-9 weeks old lean (+/+) and ob/ob mice habituated to a restricted (0900-1230 hr) daily feeding routine. Fenfluramine (10 mg/kg), cholecystokinin (100 U/kg) and neurotensin (500 micrograms/kg), administered intraperitoneally 15 min before feeding, each produced a rapid but transient suppression of food consumption in ob/ob mice, similar to lean controls. The results suggest that neural satiety mechanisms triggered via serotonin…

OBJECTIVE: In patients with Dravet syndrome (DS), fenfluramine reduced convulsive seizure frequency and provided clinical benefit in nonseizure endpoints (e.g., executive function, survival). In zebrafish mutant scn1 DS models, chronic fenfluramine treatment preserved neuronal cytoarchitecture prior to seizure onset and prevented gliosis; here, we extend these findings to a mammalian model of DS (Scn1a+/- mice) by evaluating the effects of fenfluramine on neuroinflammation (degenerated myelin, activated microglia) and survival.
METHODS: Scn1a+/- DS mice were treated subcutaneously once daily …

SCN8A-developmental and epileptic encephalopathy is caused by pathogenic variants in the SCN8A gene encoding the Nav 1.6 sodium channel, and is characterized by intractable multivariate seizures and developmental regression. Fenfluramine is a repurposed drug with proven antiseizure efficacy in Dravet syndrome and Lennox-Gastaut syndrome. The effect of fenfluramine treatment was assessed in a retrospective series of three patients with intractable SCN8A epilepsy and severe neurodevelopmental comorbidity (n = 2 females; age 2.8-13 years; 8-16 prior failed antiseizure medications [ASM]; treatment…

BACKGROUND: Dravet syndrome (DS) is an infantile-onset developmental and epileptic encephalopathy syndrome with limited treatment options. We aimed to evaluate the efficacy and tolerability of fenfluramine in patients with Dravet syndrome using meta-analytical techniques.
METHODS: We searched for relevant randomized controlled trials and non-randomized studies involving children with Dravet syndrome on fenfluramine therapy in MEDLINE, CENTRAL, EMBASE, Google Scholar and Web of Science database (31 July 2020). The primary outcome for the efficacy of fenfluramine was reduction in monthly convul…
