The Usefulness of Surgical Titanium Microclips for Mucosal Repair in the Frontal Sinus Using ORBEYE: A Technical Note.

Bifrontal craniotomy frequently involves opening the frontal sinus and mucosal injury. We report a new technique for mucosal repair in the frontal sinus using surgical titanium microclips. Six consecutive patients who underwent bifrontal craniotomy with frontal sinus exposure and mucosal injury underwent mucosal repair using surgical titanium microclips between April 2019 and August 2022. In all cases, the frontal sinus mucosa was peeled from the inner walls of the frontal sinus to ensure sufficient mucosal margin for clipping using ORBEYE. The repair was accomplished with the microclips in al

Conduit/Wrap Repairs to Digital Nerves Provide Residual Strength After Peak Loading.

BACKGROUND: Many techniques are used for digital nerve repair, most commonly coaptation by sutures. Nerve repairs must be strong while offering an environment for nerve regeneration. Sutures can damage the nerve and thereby limit growth and regeneration. Sutures can rip and cause sudden catastrophic failure. Fibrin glue and conduit-wraps allow a good environment for growth, but neither provides much strength. A benefit to conduit repair would arise if the repair maintained integrity after the peak load so that the path for regrowth stayed in place. The goal for this study was to determine whet

Effect of application of fibrin glue on vocal fold healing after surgical manipulation in rabbits.

INTRODUCTION: A challenge in phonosurgery is achieving good vocal quality with minimal vocal fold fibrosis. Fibrin glue can be applied to minimize fibrosis; however, its use in the larynx is based primarily on clinical experience, particularly in extensive lesions. OBJECTIVE: The objective of this study was to evaluate the effect of application of fibrin glue on collagen concentration at the late phase of the healing process after detaching a pedicled flap of the vocal fold cover in rabbits. METHODS: In this prospective animal study, twelve adult male rabbits underwent laryngeal microsurgery

Interposing layer of fibrin glue: A new horizon in vesico-vaginal fistula repair.

AIM: In this study our idea is to compare the effectiveness of using interposing layer of fibrin glue to omental flap in reducing the failure of laparoscopic vesicovaginal fistula repair. METHODS: Forty patients with fairly large vesicovaginal fistula were enrolled and divided in two groups of 20 each. We have used fibrin glue in one group and omental flap in the other group. RESULT: Of 20 patients in fibrin glue group no failure was seen, while 5 patients out of 20 in omental flap group had failure. CONCLUSION: This result is statistically significant and hence use of fibrin glue to be con

[Anaphylaxis Induced by Fibrin Glue During Surgery for Pneumothorax].

We report here our experience with a case of anaphylactic shock caused by fibrin glue. A 51-year-old male underwent a thoracoscopic surgery for refractory pneumothorax under local anesthesia. Bullae were revealed, and subsequently covered with fibrin glue and a polyglycolic acid sheet. Twenty-minutes after application of the fibrin glue, sudden drop of blood pressure less than 80 mmHg and a skin rash appeared. Since the patient was not administered any other drugs prior to the reduction in blood pressure, anaphylactic shock was considered to be caused by fibrin glue. The patient recovered afte

Bile does not increase the aggressiveness of pancreatic fluid towards fibrin-based sealants in a short-term ex-vivo setting.

PURPOSE: Postoperative pancreatic fistulas are among the most serious complications of pancreatic surgery. Biliary fistulas, though usually less severe, can occur after pancreaticoduodenectomy and, when combined with pancreatic fistulas, increase morbidity and mortality. This study is the first to investigate, whether bile enhances the harmful effects of pancreatic fluid on sealant integrity in a short-term ex vivo model using a fibrin-based sealant, thereby contributing to fistula formation. METHODS: We developed and validated an improved ex-vivo setup to simulate the conditions during pancr

[Use of human fibrin glue for perforated trophic retinal ulcer].

A patient with a sterile trophic corneal perforation of 2 mm after cataract surgery underwent perforation closure with human fibrin glue. Whitening of the fibrin glue indicated a stable perforation closure 10 min after application. Perforation closure was successfully performed using human fibrin glue with complete epithelialization 2 weeks after surgery. Corneal perforation closure of sterile corneal ulcerations using human fibrin glue is a simple technique that may be successful in acute cases which have an increased risk of corneal transplant rejection.

Occlusion of a recurrent tracheoesophageal fistula with surgisis.

Recurrent fistula after the repair of esophageal atresia with a tracheoesophageal fistula is a difficult problem that requires a reoperation. Each subsequent operation poses greater risk and morbidity. Therefore, in this paper we describe a case in which a fistula recurred after the initial esophageal aresia repair, then recurred again after a thoracotomy to repair the recurrence. This recurrence was successfully treated with endobronchial obliteration of the fistula tract with a biosynthetic mesh.

Sealing effect of fibrin glue on the healing of gastrointestinal anastomoses: implications for the endoscopic treatment of leaks.

BACKGROUND: The adoption of advanced laparoscopic techniques for complex surgical procedures has raised the concern that the leak rate might be higher than for open surgery, particularly in the surgeon's early experience or in difficult cases. In this study, the sealing effect of fibrin glue on leaking gastrointestinal anastomoses was evaluated in an experimental swine model. METHODS: A standardized gastrojejunostomy was performed on 20 female pigs (mean weight, 47.7 +/- 5.7 kg). A leak was created on the anterior surface of the anastomosis. The animals were randomized to either fibrin glue o

Fibrin glue for anal fistulas: long-term results.

PURPOSE: The aim of this study was to evaluate the long-term success and complication rate of fibrin-glue treatment of anal fistulas. METHODS: Patients with an anal fistula presenting to a single surgeon over a three-year period were enrolled in this study. At their first operation, all 48 patients (26-72 years old) underwent anoscopy, biopsy, destruction of the internal gland, and placement of a draining seton. Approximately two months later after preoperative bowel preparation, the seton was removed, the internal opening closed with a single suture, and fibrin glue instilled by way of the e

Nerve anastomosis with glue: comparative histologic study of fibrin and cyanoacrylate glue.

The authors report the results of a comparative experimental nerve study, using a biologic tissue glue (fibrin) and a synthetic glue (2-cyanoacrylate) in a rat model. A tension-free repair is necessary with the use of fibrin glue, or gapping may occur, thus limiting the use of the agent in promoting re-neurotization. In addition, the human origin of fibrin and thrombin allow for the possibility of viral transmission. The aim of the study was to verify if the synthetic glue is a viable alternative, or whether it causes cellular and tissue lesions. Their main finding was that the cyanoacrylate c

Results of fibrin glue application therapy in testicular hydrocele.

Nine patients, mean age 55 years, with testicular hydroceles, were treated by aspiration and two-component fibrin glue injection. One patient was treated twice. The glue contains 70-110 mg fibrinogen and 500 IU human thrombin in 0.5- and 2-ml injections, respectively (Tisseel duo quick, Immune AB). The smaller glue volume was used in 4 cases and the larger volume in 6 cases. The average volume of hydrocele fluid was 77 (range 60-120) ml. Treatment caused no pain or discomfort other than puncture of the skin and no pain-relieving medication was required afterwards. In this series there were no

Fibrin glue for reanastomosis of the fallopian tube in the rabbit: adhesions and fertility.

Fertility and adhesions were examined in 41 rabbits in three treatment groups following the construction of microsurgical anastomoses with fibrin glue and by conventional suturing techniques. The three types of treatment were: isthmic anastomoses without resection, ampullary anastomoses, isthmic anastomoses following resection of oviduct segments. There were no significant differences between the two methods of performing the anastomoses in any of the three groups with respect to number of ovulations, number of implantations, nidation index, and pregnancy rate. The extent of the formation of a

Effects of Novel Cross-Linked Chondroitin Sulfate (SI-449) as a Postoperative Anti-Adhesion Barrier.

INTRODUCTION: Postoperative adhesion often develops as a natural physiological response following abdominal and pelvic surgeries. Although existing resorbable adhesion barriers have reduced the incidence of postoperative adhesion formation, their clinical efficacy requires improvement. In this study, we generated a novel cross-linked, powder-formed chondroitin sulfate (SI-449) as an effective postoperative anti-adhesion barrier. We evaluated its anti-adhesion effect in animal models of abdominal and pelvic surgeries and elucidated its mechanism of action. METHODS: Rats in the cecum-abraded ad

An injured tissue affects the opposite intact peritoneum during postoperative adhesion formation.

The pathophysiology of adhesion formation needs to be clarified to reduce the adhesion-related morbidity. The epithelial characteristics of the peritoneum suggest a protective role against adhesion formation, yet how the peritoneum is involved in adhesion formation is not well characterized. We microscopically observed an experimental model of adhesion formation to investigate the effects of an injured tissue on the opposite intact peritoneum. Adhesions were induced between injured and intact hepatic lobes, and the intact peritoneum opposite to the injured tissue was examined for 8 days. The o
