BACKGROUND: In 2010, Kenya's Constitution created a devolved governance structure, with county governments assuming responsibility for health service delivery after 2013. Eye health carries a burden, with blindness prevalence at 0.37% and vision impairment affecting 11.0-37.5% across regions. Policy and financing mechanisms shaping service availability remain poorly understood. Kenya has advanced through seven strategic plans, the latest being the National Eye Health Strategic Plan (NEHSP, 2020-2025). While plans provide direction, they have operated without a policy framework, limiting financ

During the COVID-19 pandemic, countries adopted mitigation strategies to reduce disruptions to cancer services. We reviewed their implementation across health system functions and their impact on cancer diagnosis and care during the pandemic. A systematic search was performed using terms related to cancer and COVID-19. Included studies reported on individuals with cancer or cancer care services, focusing on strategies/programs aimed to reduce delays and disruptions. Extracted data were grouped into four functions (governance, financing, service delivery, and resource generation) and sub-functi

Development assistance has historically supported major gains in health and health-system development in sub-Saharan Africa, but recent reductions in development assistance have intensified financing and service-delivery pressures across the region. As governments seek more durable and domestically sustained strategies, the private health sector may play a larger role in expanding financing, specialised care, innovation, workforce retention, and public-private partnerships. However, private sector expansion alone is not sufficient and may worsen inequities if it remains disconnected from natio

OBJECTIVE: To assess the potential of opioid settlement dollars disbursed to state and local governments to replace or supplement federal substance use disorder (SUD) funding.
STUDY SETTING AND DESIGN: For each state, we calculated the percentage of Substance Use and Mental Health Services Administration (SAMHSA) SUD funding for each state that can be offset by settlement funds.
DATA SOURCES AND ANALYTIC SAMPLE: We estimated annual opioid settlement distributions (2022-2038) from KFF Health News and SAMHSA-funded SUD awards (2024) from SAMHSA's website and the Tracking Accountability in Gove

Family planning is commonly regarded as a highly cost-effective health intervention with wider social and economic benefits. Yet use of family planning services in Sierra Leone is currently low and 25.0% of married women have an unmet need for contraception. This study aims to estimate the costs and benefits of scaling up family planning in Sierra Leone. Using the OneHealth Tool, two scenarios of scaling up family planning coverage to currently married women in Sierra Leone over 2013-2035 were assessed and compared to a 'no-change' counterfactual. Our costing included direct costs of drugs, su

BACKGROUND: Fighting illness and poverty are intertwined objectives in global development. In recent decades, health financing reforms across many nations have enhanced financial protection for low-income populations and promoted health equity for all citizens. However, prior cross-national comparative studies predominantly focused on examining financing structures or social health insurance (SHI) schemes, neglecting financing schemes targeting the poor, such as medical financial assistance (MFA). This study comparatively explores the design of health financing schemes and financial protection

BACKGROUND: Improving maternal and child health (MCH) outcomes is a critical agenda item in global development. Health financing factors play a crucial role in affecting MCH outcomes, which vary substantially in the Middle East and North Africa (MENA) region. This study aims to examine the trends in maternal mortality rate (MMR), infant mortality rate (IMR), and under-5 mortality rate (U5MR) in the MENA region and the potential impact of health financing factors on them.
METHODS: We compiled data on MCH mortalities and potential determinants, including health financing factors, for all countr

Health system fragmentation is the division and lack of coordination among major health system functions such as financing, service delivery or governance. It is often claimed that fragmentation significantly affects the efficiency, equity and quality, often leading to inefficiencies and poor performance. However, the fragmentation of health systems, particularly from the structural and financing perspective, is conceptually underexplored. This article focuses on fragmentation of health system financing, a critical driver of health system performance and presents a framework and narrative lite

INTRODUCTION: Financial limitations have stalled the implementation of national surgical plans in low- and middle-income countries (LMICs), which significantly rely on development assistance for their health expenditures. This paper describes the challenges and opportunities in accessing development assistance from multilateral organisations and quantifies the trends of their financial contributions to surgical systems in LMICs.
METHODS: We analysed publicly available data from the Institute for Health Metrics and Evaluation Financing Global Health database and Organisation for Economic Co-op

Somalia continues to face multidimensional public health challenges driven by prolonged conflict, political instability, fragmented governance, climate-related disasters, displacement, poverty, and a fragile health system. The country carries a dual burden of communicable and non-communicable diseases (NCDs), alongside persistently high maternal and child mortality and recurrent nutrition and WASH emergencies. These pressures are intensified by workforce shortages, limited laboratory and surveillance capacity, unequal access to care, and heavy dependence on externally financed programmes. In t

Despite primary healthcare (PHC) being recognised in global declarations-Alma Ata in 1978 and Astana in 2018-and prioritised in national health strategies, chronic under-resourcing of PHC persists in most low-income and middle-income countries. More public spending is needed for PHC, but macrofiscal and political constraints often limit the ability of governments to allocate more public resources to PHC. Under-resourcing has been compounded by fragmented and rigid funding flows, which are inefficient and may erode equity, quality of care and public trust in PHC.This article explores the driver

As international development partners reduce funding for family planning (FP) programs, the need to estimate the financial resources devoted to FP is becoming increasingly important both at all levels. This cross-sectional assessment examined the FP financing sources, agents, and expenditures in two counties of Kenya for fiscal years 2010/2011 and 2011/2012 to guide local decision-making on financial allocations. Data were collected through a participatory process. This involved stakeholder interviews, review of financial records and service statistics, and a survey of facilities offering FP s

This paper examines how Brazil, Rwanda, and China have pursued universal health coverage (UHC) beyond classical health system models developed in high-income settings. We introduce the concept of institutional assembly under constraints, offering a complementary framework for understanding UHC pathways in developing countries, where no single trajectory universally applies. Brazil's health system ensures free access at the point of care through tax-based public financing, but limited government spending has produced a dualised system of public and private provision. Rwanda extends coverage to

The Previne Brasil program and the new 2024 methodology altered the federal co-financing of Primary Health Care (PHC) in Brazil. This study aimed to analyze the temporal trend of performance indicators and the number of registrations in PHC services and to compare the cost of PHC expenses from federal resources, after the co-financing changes, in the municipalities of Pernambuco. This is an ecological study with temporal trend analysis using Joinpoint regression, from 2018 to 2024. In this study, Pernambuco was used as an illustrative case to discuss the application of payment for performance

PURPOSE: This study characterizes the number of trainee positions allocated to residency programs across surgical and nonsurgical specialties from 2010 to 2024.
METHODS: Data from the National Resident Matching Program and San Francisco Match were collected on entry-level trainee positions across specialties from 2010 to 2024. Entry positions into either the plastic surgery integrated residency (PGY-1) and independent positions were summed to evaluate "plastic surgery (total)" positions. Linear regressions analyzed changes in position numbers and year-over-year percent changes. The relative s

STUDY QUESTION: What is the governmental fiscal impact of a new assisted reproduction subsidy scheme based on projected lifetime net taxes attributed to resulting live births in Taiwan?
SUMMARY ANSWER: We estimate that the new fertility reimbursement scheme has generated favorable lifetime fiscal gains for the Taiwanese government, resulting in a return on investment (ROI) of NT$5.6 for every NT$1.0 spent based on those families receiving public subsidies for fertility care under the new scheme.
WHAT IS KNOWN ALREADY: Globally, there is variation in the amount of public reimbursement for ass

As of 2024, herpes zoster (HZ) vaccination for older adults was not included in Japan's routine vaccination program. Two types of HZ vaccines are currently available at the individual's expense. In this study, we surveyed program details for HZ vaccination in Japanese municipalities. We further explored the subsidy amount and the percentage of newly vaccinated individuals among target populations who were vaccinated by local governments under the subsidy. In June 2023, we mailed the questionnaire to 56 municipalities about HZ vaccine subsidy programs in fiscal year 2022. Information on the det

While transition of donor programs to national control is increasingly common, there is a lack of evidence about the consequences of transition for private health care providers. In 2015, President's Emergency Plan for AIDS Relief (PEPFAR) identified 734 facilities in Uganda for transition from PEPFAR support, including 137 private not-for-profits (PNFP) and 140 private for-profits (PFPs). We sought to understand the differential impacts of transition on facilities with differing ownership statuses. We used a survey conducted in mid-2017 among 145 public, 29 PNFP and 32 PFP facilities reportin

The outbreak of the Ebola virus disease (EVD) in 2014 mobilized international efforts to contain a global health crisis. The emergence of the deadly virus in the United States and Europe among health care workers intensified fears of a worldwide epidemic. Market incentives for pharmaceutical firms to allocate their research and development resources toward Ebola treatments were weak because the limited number of EVD cases were previously confined to rural areas of West Africa. We discuss 3 policy recommendations to address the long-term challenges of EVD in an interconnected world.
