A case of aphonia following a long-time endotracheal intubation is presented. The paramedian position of the left and the incomplete adduction of the right vocal cord were caused by a complete, respectively partial ankylosis of the cricoarytenoid joints. The ankylosis was the result of an arthritis during the intubation period. A nearly normal mobility of the vocal cords and a normal voice was achieved by translaryngeal injections of Fludrocortisone and Hyaluronidase into the cricoarytenoid joints and by their passive mobilisation.

Autosomal recessive renal tubular dysgenesis (RTD) is a rare genetic disorder caused by defects in the renin-angiotensin system, with the most common outcomes being foetal or neonatal death from renal failure, pulmonary hypoplasia and/or refractory arterial hypotension. A small proportion of patients survive past the neonatal period. We present the case of a toddler with RTD due to compound heterozygous variants in the ACE gene that codes for ACE, who has not required renal replacement therapy to date and in whom fludrocortisone has achieved electrolyte and acid/base balance.

Cerebral salt wasting is characterized by inappropriate natriuresis and volume contraction in the presence of cerebral pathology. Diagnosis can be difficult and therapy is challenging. We report two children with tuberculous meningitis and hydrocephalus who developed cerebral salt wasting following neurosurgical intervention. The first patient was managed with rigorous salt and water replacement whereas the second patient required the addition of fludrocortisone for control of salt-wasting.

Orthostatic hypotension is encountered frequently in everyday practice. It is often very disabling because of its troublesome symptomalogy. Orthostatic hypotension may be due to several causes and can be easily diagnosed by measuring blood pressure and heart rate after 3 minutes in upright posture. Its treatment comprises in all cases non-pharmacological interventions which can be associated whenever required by medications, in particular midodrine and fludrocortisone.

Primary adrenocortical insufficiency is described in 3 dogs. Clinical signs included marked depression, vomiting, diarrhoea, weakness and bradycardia. Diagnosis was based on clinical signs and biochemical findings of hyperkalaemia, hyponatraemia and a lowered plasma Na+ : K+ ratio. Treatment during the acute illness consisted of intravenous fluids and hydrocortisone sodium succinate. Fludrocortisone acetate has maintained all dogs in good health. One dog was on treatment for 4 years.
