Study on the new strategy and key techniques for accurate prevention and treatment of nonalcoholic steatohepatitis based on intestinal target bacteria.

BACKGROUND: Nonalcoholic fatty liver disease (NAFLD) has emerged as a major health problem worldwide; according to statistics, 10% to 25% of patients with NAFLD can progress to nonalcoholic steatohepatitis (NASH). A link between the composition and metabolites of intestinal microbiota and the development of NAFLD is becoming clearer. It is believed that microbiota factors are driving forces of hepatic steatosis and inflammation. The formulated food that contains prebiotics and dietary fiber may improve NAFLD by altering the intestinal flora and its metabolites. METHODS: The study plan to recr

[Allergy to protein hydrolysates. Report of 30 cases].

AIM OF THE STUDY: To describe the population of children allergic to cow's milk protein and their usual substitutes made of protein hydrolysates, and who are efficiently taken care of by using Neocate, an amino acid based formula. PATIENTS AND METHODS: Allergy to protein hydrolysates was diagnosed in 30 infants, aged from 15 d to 13 months (median = 3 months), who remained symptomatic while receiving a protein hydrolysate for 15 d to 12 months (median = 1.8 months). After a complete clinical and biological evaluation, an amino acid based diet using Neocate was attempted, followed after one mo

Model-informed Drug Development (MIDD) Approach to Support Biopharmaceutical Development of Iberdomide.

Iberdomide is a BCS III CELMoD™ agent currently under development for treatment of multiple myeloma. Five formulations were used during clinical development, starting with active ingredient in gelatin capsule (AIC), followed by subsequent formulations in gelatin (F1), HPMC capsules (F2 and F3), and finally the intended commercial form (ICF). A food effect study with the Phase I AIC formulation showed no food effect and two relative bioavailability studies bridging from AIC to formulation F1 and F2 showed similar systemic exposure. Modeling and simulation, based on in vitro dissolution, was use

Elemental diet decreases inflammation and improves symptoms in adult eosinophilic oesophagitis patients.

BACKGROUND: Eosinophilic oesophagitis (EoE) is a chronic disease, driven by food allergens. Elemental diets are effective for the management of children with EoE, but studies on the effect of elemental diets in adults are scarce and poor palatability challenges dietary adherence. AIM: To assess the effects of an elemental diet (Neocate, Nutricia, Utrecht, the Netherlands) on the inflammation, symptoms and endoscopic signs in adult EoE patients. METHODS: In this prospective study, 21 patients with active EoE, confirmed by biopsies showing ≥15 eosinophils per microscopic high power field (HPF)

Supplements containing amino acids and keto acids in the treatment of chronic uremia.

Three mixtures containing varying proportions of threonine, tyrosine, and the ornithine, lysine, and histidine salts of branched-chain keto acids have been tested as dietary supplements to a 20- to 25-g mixed-quality protein diet in patients with severe chronic uremia. Two of the three supplements improved the abnormalities of plasma amino acid concentrations, and slowed or arrested progression of renal insufficiency. The second supplement, which contained less threonine and lysine, led to subnormal plasma concentrations of these two amino acids and aggravated hypophosphatemia. The third suppl

Azetidine-2-carboxylic acid contaminated dietary proline as a cause of urinary excretion of 4-amino-2-(S-cysteinyl)butyric acid in patients on oral treatment with a synthetic diet.

Three children with branched-chain ketoaciduria (maple syrup urine disease) were found to excrete an abnormal amino acid when they were on an artificial diet. This substance was identified as 4-amino-2-(S-cysteinyl)butyric acid with the use of column liquid chromatography, gas chromatography--mass spectrometry of various derivatives, and 360 MHz 1H-NMR spectroscopy. The same compound was detected in urine samples from subjects undergoing an oral loading test with L-proline. The chromatographic analysis of commercial proline from two sources indicated that one of the batches was contaminated (l

Development of low glycemic index food products with wheat resistant starch: a review.

With the growing recognition of the health benefits associated with GI foods in managing type 2 diabetes and alleviating oxidative stress, wheat-based low-GI foods enriched with resistant starch (RS) emerging as a promising alternative to refined grains in daily diets. This review provides a comprehensive overview of the health benefits of RS, delving into its types, characteristics, and the mechanisms underpinning its resistance to hydrolysis. To enhance the functionality of RS, various modification techniques-encompassing chemical, enzymatic, physical, and combinatory approaches-have been ex

Homemade formulas for nutrition support in chronic kidney disease: A narrative review of the opportunity for education, research, and innovation.

Protein-energy wasting is common in people with chronic kidney disease (CKD), especially in those undergoing kidney replacement therapy. Oral nutrition supplements and enteral nutrition are strategies that have been shown to improve nutrition status, and potentially outcomes. However, access to specialized commercial products for people with CKD is limited by factors including cost and regional availability. Homemade formulas represent a potentially cheaper, accessible, and more flexible option than commercial products, but they come with their own unique set of challenges. Furthermore, some a

What's in the Bottle? A Review of Infant Formulas.

Infant formulas are designed to be a substitute for breast milk. Since they are sole source of nutrition for growing and developing infants, they are highly regulated by the government. All ingredients in infant formulas must be considered "generally recognized as safe." Manufacturers are continually modifying their products to make them more like breast milk. Functional ingredients added to infant formula include long-chain polyunsaturated fatty acids, nucleotides, prebiotics, and probiotics. The most common breast milk substitutes are standard cow's milk-based term infant formulas, which inc

The reality of dietary compliance in the management of phenylketonuria.

In phenylketonuria (PKU), it is common for blood phenylalanine (Phe) concentrations to be outside optimal target ranges, particularly in teenagers and adults, indicating inadequate compliance. It is well known that significant noncompliance exists, and the situation in PKU would appear no different than other chronic conditions. In PKU, compliance is complex, being subject to diverse definitions, and factors influencing compliance include the nature and nurture of the patient, as well as the inconvenience, cost and availability of dietary treatment. It is also a dynamic process, with many pati

Effect of cholecystokinin and of its antagonist, of atropine, and of food on the release of immunoglobulin A and immunoglobulin G specific antibodies in the rat intestine.

Cholecystokinin has previously been shown to produce a rise in immunoglobulin A (IgA) antibodies in the intestinal fluid of human volunteers. Whether the source of these antibodies was pancreatic, biliary, or mucosal had not been defined. The object of the present investigation was to study factors involved in regulating the release of IgA and IgG antibodies from the intestinal mucosa. Hooded-Lister rats were sensitized to ovalbumin. After the sensitization protocol, which lasted 21 days, the rats were anesthetized and a segment of intestine 10 cm long was isolated and perfused in vivo. This s

Elemental diets in the management of Crohn's perianal fistulae.

Elemental diets have been used extensively in the routine management of patients with Crohn's disease during the past 2 years in this unit. We report our experience in 6 patients in whom perianal fistulae were present. Fistulae improved in 4 patients during elemental feeding; complete healing of fistulae occurred in one of them. No benefit was apparent in 2 patients, but in one the fecal stream had previously been diverted by transverse colostomy, and in the other fecal stasis was marked. The clinical significance of the observations and the difficulties of assessing diet therapy in this condi

Elemental diet therapy in the management of complicated Crohn's disease.

The purpose of this report was to study the types of complications of Crohn's disease which may benefit from the use of elemental diets. The elemental diet Vivonex was used in the management of patients with Crohn's disease with local complications but without nutritional deficiency. Sole treatment of this kind was found to be of value in promoting healing of perianal or anal ulceration and anal fissures; it was also of value in allowing subacute ileal obstruction to settle, thus postponing surgery and allowing better bowel and nutritional preparation for surgery. Patients with bile acid-induc

Prospective study of an amino acid-based elemental diet in an eosinophilic gastritis and gastroenteritis nutrition trial.

BACKGROUND: Eosinophilic gastritis/gastroenteritis (EoG/EoGE) are rare disorders with pathologic gastric and/or small intestinal eosinophilia lacking an approved therapy. An allergic mechanism is postulated but underexplored mechanistically and therapeutically. OBJECTIVE: We evaluated the effectiveness of a food allergen-free diet (elemental formula) in controlling gastrointestinal eosinophilia in adult EoG/EoGE. METHODS: Adults aged 18 to 65 years with histologically active EoG/EoGE (≥30 eosinophils per high-power field) in the stomach and/or duodenum and gastrointestinal symptoms within th

Lipemia retinalis in an infant treated for retinopathy of prematurity.

Lipemia retinalis is an unusual ocular finding associated with hypertriglyceridemia. We report the case of an infant treated for retinopathy of prematurity who later developed lipemia retinalis, with triglyceride levels of 4736 mg/dl. There was a paradoxical worsening of hypertriglyceridemia with the use of medium chain triglyceride supplement. On discontinuing the supplement, the triglycerides level drastically dropped, and retinal vasculature returned to a normal hue.
