A Method for Cranial Nerve XI Silencing During Surgery of the Foramen Magnum Region: Technical Case Report.

BACKGROUND AND IMPORTANCE: Skull base surgery involves the microdissection and intraoperative monitoring of cranial nerves, including cranial nerve XI (CN XI). Manipulation of CN XI can evoke brisk trapezius contraction, which in turn may disturb the surgical procedure and risk patient safety. Here we describe a method for temporarily silencing CN XI via direct intraoperative application of 1% lidocaine. CLINICAL PRESENTATION: A 41-yr-old woman presented with symptoms of elevated intracranial pressure and obstructive hydrocephalus secondary to a hemangioblastoma of the right cerebellar tonsil

Vertebral Artery Transposition Via an Extreme-Lateral Approach for Anterior Foramen Magnum Meningioma or Craniocervical Junction Tumors.

OBJECTIVE: Vertebral artery (VA) transposition in the extreme-lateral transcondylar approach can minimize the manipulation of the low cranial nerves and the brain stem. The authors describe the surgical technique of VA transposition. METHODS: From March 2000 to December 2014, 28 of 48 patients underwent VA transposition for anterior foramen magnum meningioma (16 patients) and craniocervical junction (CCJ) tumors (12 patients). Tumor was resected via an extreme-lateral approach with partial condylectomy to expose the anterior portion of the brain stem. For intradural tumors, the VA was mobiliz…

Foramen magnum arachnoid cyst induces compression of the spinal cord and syringomyelia: case report and literature review.

It is very rare that a foramen magnum arachnoid cyst induces compression of the spinal cord and syringomyelia, and currently there are few treatment experiences available. Here we reported the case of a 43-year-old male patient who admitted to the hospital due to weakness and numbness of all 4 limbs, with difficulty in urination and bowel movement. MRI revealed a foramen magnum arachnoid cyst with associated syringomyelia. Posterior fossa decompression and arachnoid cyst excision were performed. Decompression was fully undertaken during surgery; however, only the posterior wall of the arachnoi…

Computed Tomography-Aided Morphometric Analysis of the Foramen Magnum for Neurosurgical Insights: A Pilot Study in the Pakistani Population.

BACKGROUND: The foramen magnum (FM) is a critical structure at the craniovertebral junction (CVJ), influencing surgical access, radiological evaluation, and forensic sex estimation. Although international reference values exist, computed tomography (CT) based FM morphometry remains underreported in the Pakistani population. This study provides population-specific high-resolution computed tomography (HRCT) measurements, evaluates gender and age-related variation, and classifies FM shape using the Martin and Saller index. MATERIALS AND METHODS: A retrospective analysis of 166 adult HRCT scans (…

Management of Unique Basilar Invagination Combined with C1 Prolapsing into the Foramen Magnum in Children: Report of 2 Cases.

BACKGROUND: Basilar invagination (BI) is a developmental anomaly and commonly presents with neurologic findings. The incidence of BI associated with other osseous anomalies of the craniovertebral junction is high, including incomplete ring of C1 with spreading of the lateral masses, atlanto-occipital assimilation, hypoplasia of the atlas, basiocciput hypoplasia, and occipital condylar hypoplasia. However, BI combined with C1 prolapsing into the foramen magnum (FM) is an extremely rare condition. CASE DESCRIPTION: A previously healthy 6-year-old girl (case 1) presented with extremities numbnes…

The formation of the foramen magnum and its role in developing ventriculomegaly and Chiari I malformation in children with craniosynostosis syndromes.

OBJECT: Craniosynostosis syndromes are characterized by prematurely fused skull sutures, however, less is known about skull base synchondroses. This study evaluates how foramen magnum (FM) size, and closure of its intra-occipital synchondroses (IOS) differ between patients with different craniosynostosis syndromes and control subjects; and whether this correlates to ventriculomegaly and/or Chiari malformation type I (CMI), intracranial disturbances often described in these patients. METHODS: Surface area and anterior-posterior (A-P) diameter were measured in 175 3D-CT scans of 113 craniosynos…

The pathogenesis of syringomyelia associated with lesions at the foramen magnum: a critical review of existing theories and proposal of a new hypothesis.

Syringomyelia is frequently accompanied by an extramedullary lesion at the foramen magnum, particularly a Chiari I malformation. Although syringomyelia associated with foramen magnum obstruction has characteristic clinical, radiological, and neuropathological features, its pathogenesis remains unclear. Currently prevalent hydrodynamical theories assert that obstruction of the subarachnoid space at the foramen magnum interferes with flow of cerebrospinal fluid (CSF) between the spinal and the intracranial subarachnoid compartments. As a result, spinal CSF is driven into the spinal cord through …

Comparison of Foramen Magnum and Foramen of Magendie Dredging Versus Posterior Fossa Decompression With Duraplasty in Adults With Chiari I Malformation-Syringomyelia: A Propensity-Matched Study.

BACKGROUND AND OBJECTIVES: While neurosurgical posterior fossa decompression with duraplasty (PFDD) may provide clinical and radiological improvement for Chiari malformation-I (CM-I)-syringomyelia, the comparative efficacy and safety of PFDD without vs with intradural tonsillar manipulation (posterior fossa decompression with tonsil resection) has remained controversial for over 5 decades. METHODS: We conducted a retrospective cohort study of 1231 CM-I-syringomyelia patients treated at our institution from 2003 to 2024, comparing 2 techniques: standard PFDD and foramen magnum and foramen of m…

Syringomyelia in the Course of Resection of Foramen Magnum Pathology: A Case Report with an Intriguing Therapeutic Solution and Systematic Review.

INTRODUCTION: Syringomyelia is a rare disease of the spinal cord, and postoperative syringomyelia is an extremely rare complication. There are very few scientific reports on this subject in the literature, which is why we decided to review them and report our case of a patient after resection of a meningioma of the foramen magnum; we diagnosed syringomyelia in the course of postoperative complications. METHODS: PubMed MEDLINE, Scopus, and Web of Science were searched. The PRISMA checklist was followed for a systematic structure. We paid special attention to publications describing syringomyel…

Sex and Age Determination by Measurement of Foramen Magnum Dimensions on Computer Tomography Scanning Study in the Iranian Population.

BACKGROUND: Individual distinguishing evidence may be an imperative field of measurable investigation which demonstrates higher correct expectation rates. This process of recognizable Evidence is facilitated by the assurance of sex and age. In circumstances where there are fragmented and mangled skeletal remains, sex assurance is moderately troublesome, and it becomes important to set up the precision of cadaver bones. Therefore, this study aims to evaluate sexual dimorphism and age determination by measuring foramen magnum (FM) dimensions in the Iranian population using digital computed tomog…

Surgical management of ventral and ventrolateral foramen magnum meningiomas: report on a 64-case series and review of the literature.

Foramen magnum meningioma poses a challenge for neurosurgeons. Prognosis has generally improved with diagnostic and surgical advances over the past two decades; however, it may ultimately depend more on the surgeon's ability to tailor the approach and interpret intraoperative risks in single cases. The series comprised 64 patients operated on for ventral and ventrolateral foramen magnum meningioma. All patients underwent preoperative magnetic resonance imaging and received surgery via the dorsolateral route, rendering the series homogeneous in neuroradiological workup and surgical treatment. P…

Stenosis of the foramen magnum and rostral spinal canal, with spinal cord deformity, in Jeune's asphyxiating thoracic dystrophy.

Ventilatory compromise in Jeune's asphyxiating thoracic dystrophy (JTD) generally is attributed to abnormalities in thoracic configuration that produce restrictive pulmonary disease and pulmonary hypoplasia. We present our findings following dissection and roentgenography of the base of the skull and the vertebral column in a premature infant with JTD. They suggest that malformations of the vertebral laminae in JTD may lead to compression of the distal medulla and rostral cervical spinal cord. Similar compression is known to contribute to ventilatory dysfunction in other osteochondrodysplasias

Posterior C1-C2 screw-rod salvage for Chiari I malformation with atlantoaxial instability after failed foramen magnum decompression.

BACKGROUND: Chiari I malformation secondary to atlantoaxial instability requires reduction and stabilization. When initial foramen magnum decompression (FMD) is performed without recognition of this instability, subsequent salvage surgery is technically demanding and carries substantial risk. OBJECTIVE: The objective of this study was to evaluate the clinical and radiological outcomes of posterior C1-C2 screw-rod salvage surgery in patients with Chiari I malformation and atlantoaxial instability following failed FMD. MATERIALS AND METHODS: We retrospectively analyzed 21 consecutive patients …

Resolution of tension pseudomeningocele complicating foramen magnum decompression for Chiari I malformation after ventriculoperitoneal shunt: A case report.

INTRODUCTION: Tension pseudomeningocele is a rare complication that arose after foramen magnum decompression in a patient with Chiari I malformation and syringomyelia. This article presents a case that was effectively managed through cerebrospinal fluid diversion methods, highlighting the mechanisms involved and the treatment option used. CASE REPORT: A 15-year-old girl came to our clinic with allodynia in the right anterior chest wall that had lasted for six months. An MRI showed a type 1 Chiari malformation along with syringomyelia from C2 to C6. After performing foramen magnum decompressio…

Neurenteric cysts at foramen magnum in children: presentation, imaging characteristics, and surgical management-case series and literature review.

INTRODUCTION: Neurenteric cysts (NEC) are rare benign tumors of the central nervous system. Even more rare are the NEC at the ventral foramen magnum in children that are misdiagnosed. They cause distortion of the vertebrobasilar vascular system and cervicomedullary neural structures and are adherent to the latter. The inferior portion lies ventral to the upper most dentate ligament. This has implications in the surgical approach for complete excision. METHODS AND CASE SERIES: We reviewed our series of five children with ventral foramen magnum NEC in the MRI era and analyzed presentation, radi…

Spontaneous C1 anterior arch fracture as a postoperative complication of foramen magnum decompression for Chiari malformation type 1.

BACKGROUND: C1 fracture accounts for 2% of all spinal column injuries and 10% of cervical spine fractures, and is most frequently caused by motor vehicle accidents and falls. We present a rare case of C1 anterior arch fracture following standard foramen magnum decompression for Chiari malformation type 1. CASE DESCRIPTION: A 63-year-old man underwent standard foramen magnum decompression (suboccipital craniectomy and C1 laminectomy) under a diagnosis of Chiari malformation type 1 with syringomyelia in June 2009. The postoperative course was uneventful until the patient noticed progressive pos…
