Radiation pneumonitis prediction model with integrating multiple dose-function features on 4DCT ventilation images.

PURPOSE: Radiation pneumonitis (RP) is dose-limiting toxicity for non-small-cell cancer (NSCLC). This study developed an RP prediction model by integrating dose-function features from computed four-dimensional computed tomography (4DCT) ventilation using the least absolute shrinkage and selection operator (LASSO). METHODS: Between 2013 and 2020, 126 NSCLC patients were included in this study who underwent a 4DCT scan to calculate ventilation images. We computed two sets of candidate dose-function features from (1) the percentage volume receiving > 20 Gy or the mean dose on the functioning zon…

Four-Dimensional CT Analysis of Normal Syndesmotic Motion.

BACKGROUND: The syndesmosis ligament complex stabilizes the distal tibiofibular joint while allowing for small amounts of physiologic motion. When injured, malreduction of the syndesmosis is the most important factor that contributes to inferior functional outcomes. Syndesmotic reduction is a dynamic measure, which is not adequately captured by conventional computed tomography (CT). Four-dimensional CT (4DCT) can image joints as they move through range of motion (ROM). The aim of this study was to employ 4DCT to determine in vivo syndesmotic motion with ankle ROM in uninjured ankles. METHODS:

Prospective Validation of Two 4D-CT-Based Scoring Systems for Prediction of Multigland Disease in Primary Hyperparathyroidism.

BACKGROUND AND PURPOSE: Patients with multigland primary hyperparathyroidism are at higher risk for missed lesions on imaging and failed parathyroidectomy. The purpose of this study was to prospectively validate the ability of previously derived predictive score systems, the composite multigland disease score, and the multiphase multidetector contrast-enhanced CT (4D-CT) composite multigland disease score, to identify patients with a high likelihood of multigland disease. MATERIALS AND METHODS: This was a prospective study of 71 patients with primary hyperparathyroidism who underwent 4D-CT an

Four-dimensional quantitative analysis of the gait of mutant mice using coarse-grained motion capture.

To analyze an abnormal gait pattern in mutant mice (Hugger), we conducted coarse-grained motion capture. Using a simple retroreflective marker-based approach, we could detect high-resolution mutant-specific gait patterns. The phenotypic gait patterns are caused by extreme vertical motion of limbs, revealing inefficient motor functions. To elucidate the inefficiency, we developed a musculoskeletal computer model of the mouse hindlimb based on X-ray CT data. By integrating motion data with the model, we determined mutant-specific musculotendon lengths, suggesting that three major muscles were in

A comparison of two methods for segmentation of functional volumes in radiotherapy planning of lung cancer patients.

BACKGROUND: In radiotherapy (RT) of lung cancer, dose to functional lung (FL) volumes segmented with two different methods (perfusion SPECT (Q-SPECT) and 4D-CT (4D) ventilation (V)) have been shown to correlate with the incidence of radiation pneumonitis (RP). This study aims to compare the FL volumes identified by both methods. MATERIAL AND METHODS: Thirty lung cancer patients had a 4D and Q-SPECT prior to treatment. Seventeen of these patients also had a ventilation SPECT (V-SPECT). FL sub-volumes were segmented automatically, using cut-off values. The volumes were compared in terms of over

Evaluation of 4D cone-beam CT reconstruction methods for lung images acquired using rapid cone-beam CT acquisition: a phantom study.

Objective. Cone-beam CT (CBCT) technological advances for linear accelerators (Linacs) have led to CBCT imaging in <20 s, which can reduce radiation therapy treatment times. However, these rapid CBCT scans only allow for 3DCBCT images. In this paper we evaluate 4DCBCT reconstruction methods for rapid acquisition 3DCBCT protocol scans using an anthropomorphic breathing phantom.Approach. We evaluate two previously developed motion-compensated Feldkamp-Davis-Kress (MCFDK) methods, using a prior-motion model (MCFDK-Prior) and a data-driven MCFDK method (MCFDK-DD), on CBCT images of the phantom usi

Usefulness of four dimensional (4D) PET/CT imaging in the evaluation of thoracic lesions and in radiotherapy planning: Review of the literature.

In the past decade, Positron Emission Tomography (PET) has become a routinely used methodology for the assessment of solid tumors, which can detect functional abnormalities even before they become morphologically evident on conventional imaging. PET imaging has been reported to be useful in characterizing solitary pulmonary nodules, guiding biopsy, improving lung cancer staging, guiding therapy, monitoring treatment response and predicting outcome. This review focuses on the most relevant and recent literature findings, highlighting the current role of PET/CT and the evaluation of 4D-PET/CT mo

Three-Dimensional Computed Tomography Reconstruction for Revision Parathyroidectomy.

A 57-year-old man presented with persistent hyperparathyroidism following primary parathyroidectomy. A four-dimensional computed tomography scan with three-dimensional reconstruction showed two parathyroid glands (one right and one left) and anatomic variation from previous surgery. Revision surgery was performed revealing the parathyroid glands as expected from the preoperative three-dimensional reconstruction. After surgery, the patient recovered well, and preoperative symptoms resolved. The use of three-dimensional computed tomography reconstruction provided accurate localization of the par…

Motion and anatomy dual aware lung ventilation imaging by integrating Jacobian map and average CT image using dual path fusion network.

BACKGROUND: Deep learning-based computed tomography (CT) ventilation imaging (CTVI) is a promising technique for guiding functional lung avoidance radiotherapy (FLART). However, conventional approaches, which rely on anatomical CT data, may overlook important ventilation features due to the lack of motion data integration. PURPOSE: This study aims to develop a novel dual-aware CTVI method that integrates both anatomical information from CT images and motional information from Jacobian maps to generate more accurate ventilation images for FLART. METHODS: A dataset of 66 patients with four-dim

Comparison between target margins derived from 4DCT scans and real-time tumor motion tracking: insights from lung tumor patients treated with robotic radiosurgery.

PURPOSE: A unique capability of the CyberKnife system is dynamic target tracking. However, not all patients are eligible for this approach. Rather, their tumors are tracked statically using the vertebral column for alignment. When using static tracking, the internal target volume (ITV) is delineated on the four-dimensional (4D) CT scan and an additional margin is added to account for setup uncertainty [planning target volume (PTV)]. Treatment margins are difficult to estimate due to unpredictable variations in tumor motion and respiratory pattern during the course of treatment. The inability t

Patient Positional Uncertainty and Margin Reduction in Lung Stereotactic Ablative Radiation Therapy Using Pneumatic Abdominal Compression.

PURPOSE: Motion management presents a significant challenge in thoracic stereotactic ablative radiation therapy (SABR). Currently, a 5.0-mm standard planning target volume (PTV) margin is widely used to ensure adequate dose to the tumor. Considering recent advancements in tumor localization and motion management, there is merit to reassessing the necessary PTV margins for modern techniques. This work presents a large-scale analysis of intrafraction repositioning for lung SABR under forced shallow breathing to determine the margin requirements for modern delivery techniques. Ectopic Parathyroid Adenoma of the Piriform Sinus on Parathyroid 4D-CT and 99mTc-MIBI SPECT/CT.

Ectopic parathyroid adenomas are a common cause of postsurgical persistent primary hyperparathyroidism. Our case highlights a patient with a negative bilateral 4-gland exploration with follow-up parathyroid 4-dimensional CT and Tc-MIBI SPECT/CT, yielding focal uptake in the right piriform sinus. Subsequent direct laryngoscopy revealed a mass in the piriform sinus, which was resected with surgical pathology yielding parathyroid adenoma.

Comparison of supine and upright postures for liver respiratory motion management in radiotherapy.

BACKGROUND: Understanding respiratory motions of liver and its surrogate organs is crucial for precise dose delivery in liver cancer radiotherapy. Although these motions have been studied for respiratory motion management in the supine posture, few studies have quantified them and evaluated their correlations in the upright posture. PURPOSE: This study quantified the respiratory motions of liver and surrogate organs and evaluated the correlations between the liver motions and surrogate signals for respiratory motion monitoring in both the supine and upright postures. METHODS: 4D dynamic volu
