[Isolated dislocation fracture of the trapezium (author's transl)].

As a very rare injury, isolated fracture, dislocation or rarely dislocation fracture of the trapezium can be connected with difficult diagnostical problems. Routine X-rays of the hand are not always sufficient, because of overlying trapezoid-contours. Special X-rays are additionally necessary. Concerning the therapy, there is a balance in conservative and surgical treatment, depending on the kind and nature of fracture and dislocation.

Cervical spine fracture dislocation with mild neurological deficits.

Cervical fracture dislocation often leads to neurological deficits, manifesting with sensory and motor symptoms, which may persist even after surgical intervention. We presented two cases with mild neurological deficits following such injuries. In Case 1, the patient presented with left-hand numbness 1 month after a car accident. Despite this, he exhibited full muscle power in all four limbs. Cervical spine MRI revealed a posterior column tear, facet fracture dislocation and spinal cord indentation at the C5/C6 level. The patient underwent open reduction with anterior and posterior fixation. A…

Bilateral Fracture-Dislocation of the Shoulder After Defibrillation.

BACKGROUND: There are multiple reported injuries associated with cardiopulmonary resuscitation, most of them caused by the force of compressions, like sternal and rib fractures, abdominal organ injuries like splenic rupture, liver lacerations, and injuries to the upper airway and skin. Injuries related to defibrillation and cardioversion are rare, mostly related to skin and muscle injuries on where the defibrillation paddles were placed. CASE REPORT: A 52-year-old man presented to the Emergency Department with crushing chest pain. The patient was suffering from a myocardial infarction, and du

Dorsal Radiocarpal Joint Fracture-Dislocation: A Case Report.

CASE: A 19-year-old right-hand-dominant man sustained a right dorsal radiocarpal dislocation. He presented with instability secondary to displacement of the volar ligamentous structures and an avulsed fragment of the distal aspect of the radius within the radiocarpal articulation. The patient was treated with open reduction and internal fixation, as well as a capsuloligamentous repair. At the 18-month follow-up, he had no pain and excellent functional recovery. CONCLUSION: Our experience emphasizes the importance of looking critically at the radiographs when there is an irreducible radiocarpa

Posterior Wall Fracture-Dislocation With Traumatic Abductor Avulsion.

The typical injury pattern of a posterior hip dislocation includes posterior wall fracture, capsular injury, and muscular strain. This report presents an example of a posterior wall acetabulum fracture-dislocation associated with complete hip abductor tendon and capsular avulsion from the femur. The patient was found to have an unstable hip despite a small posterior wall fragment due to the extensive soft tissue injury. This was repaired. The patient had improvement in function initially, but ultimately required repair of the hip abductor tendons. To the best of the authors' knowledge, this is

Trans-Scaphoid Perilunate Fracture Dislocation; A Technical Note.

Carpal fracture-dislocation is regarded as an unusual orthopedic injury and, thus, orthopedic surgeons are less experienced in dealing with … pectively. The combined approach had favorable results for treatment of this unusual type of carpus fracture dislocation. However longer follow up is need to evaluate the arthritis and degenerative changes in wrist.

Bilateral talus fracture dislocation: is avascular necrosis inevitable?

Bilateral simultaneous fracture dislocation of the talus is a very rare injury. It occurs usually following high-velocity injuries. We report a 45-year-old man who presented with bilateral talar fracture dislocation following a heavy stone falling on him. On his right side, he had open talus dislocation with lateral process fracture and on his left side he had closed talar neck fracture dislocation. He underwent immediate debridement and external fixator application for his right side and open reduction and internal fixation for his left talus. At the end of 34 months' follow-up, he was able t…

Isolated tarsal navicular fracture dislocation: a case report.

UNLABELLED: Injuries to the midtarsal joints are rare. They occur in the form of various combinations such as fracture, fracture subluxation, and fracture dislocation. The largest series of 71 cases has been reported by Main and Jowett in 1975. Isolated tarsal navicular dislocations are rare injuries. Being rare, these injuries remain poorly understood. Only few case reports exist, which describe the probable mechanism of injury and optimal treatment. Of the few case reports, only one describes closed reduction with external fixator and percutaneous fixation as the treatment modality. This cas…

An irreducible ankle fracture dislocation: the Bosworth injury.

Irreducible fracture dislocations of the ankle are rare and represent true orthopedic emergencies. We present a case of a fracture dislocation that was irreducible owing to a fixed dislocation of the proximal fibular fragment posterior to the lateral ridge of the tibia. This particular type of injury, known as a Bosworth fracture dislocation, was not appreciated on the initial radiographs taken in the emergency room but was identified at urgent surgical management. The trauma mechanism, radiographs, treatment, and relevant published data are discussed in the present report.

Bilateral neglected posterior fracture-dislocation of the shoulders.

Posterior dislocation of the shoulder is an uncommon injury. Diagnosis is difficult and often missed. Once diagnosed, management must be individualized depending on the amount of the defect of the humeral head and the time from injury. This article presents a case of a 40-year-old man with a 4-month history of bilateral locked posterior fracture-dislocation of the shoulders after a grand mal seizure. Imaging showed loss of the glenohumeral joint lines congruency, reverse Hill-Sachs lesions, and articular defects of 35% and 40% of the humeral heads. A modified McLaughlin technique was performed

Fracture dislocation of carpometacarpal joints: a missed injury.

Fracture dislocation of the carpometacarpal joints on the ulnar side of the hand is an uncommon injury. These are high-energy injuries seen in motorcyclists and boxers. The mechanism of injury involves violent, forceful dorsiflexion of the wrist combined with longitudinal impact on the closed hand. This article reports a case of fracture of the base of the middle finger with dislocation of the ring and little finger carpometacarpal joints. On first examination, a diagnosis of isolated, minimally-displaced, middle-metacarpal base fracture was made and deemed suitable for nonoperative management…
